Neonatal morbidity and mortality upon administration of corticosteroids and beta-mimetic drugs were observed. Remarkable was the low rate of maternal side effects and that their seriousness was quite inferior to that found by other authors. The fetal and neonatal side effects or symptoms were very few and it was impossible to ascertain whether they were due to the therapy, a consequence of maternal pathological events, the kind of delivery, gestational age or some other unknown factor. In contrast, an increased capacity is associated with an immune response to placental antigens. In humans, repeated miscarriages seem to be associated to a higher frequency of HLA allele BW35. Twenty couples with repetitive spontaneous abortions of unknown etiology and 20 control couples with normal fertility were HLA typed. The sera of wives of infertile couples were tested against the husbands lymphocytes in complement dependent lympholysis test in order to detect anti-paternal HLA antibodies. The data show an HLA significant compatibility between the husbands and the wives with a history of repetitive spontaneous abortions. The best probability of evaluating different loci is that of HLA-A (p less than 0.01). The frequency of HLA-BW35 in women with repeated miscarriages is higher than in control women or than in the general Italian population. In this paper our studies on the biosynthesis of prostaglandins in human amnion focused on the role of prostaglandins in initiating and maintaining labor. First of all, the source of prostaglandins released in amniotic fluid was investigated. The amnion was demonstrated to possess the highest activity of generating prostaglandins among amnion, chorion laeve, and decidua vera, obtained after normal vaginal delivery. Subsequently, the bioconversion of arachidonic acid into prostaglandins and related compounds using the homogenate of human amnion was carried out without the addition of cofactors in the incubation mixture to show the natural spectrum of various types of PGs formed. PGE2 was revealed to be a main product of the amnion; its conversion rate was 5% of incubated radioactivity. Other compounds catalyzed by cyclooxygenase were PGF2 alpha (0.2%), TXB2 (0.3%), and HHT (1-2%), whereas 15-HETE (2-3%) and a mixture of 9, 11, 12-HETE (1-2%) were formed by lipoxygenase in the amnion. And third, the activity to produce PGE2 in the amnion obtained at normal vaginal delivery was greater than that obtained at cesarean section prior to the onset of labor. Thus, these results suggested that the amnion played a significant role in prostaglandin biosynthesis in uterine tissues. Agglutination of the latex reagent by the antiserum occurs within 3 min. The agglutination, however, was completely inhibited by 100 ng/ml or more of estriol. There was a good correlation in results obtained by LAIR and by colorimetry, RIA and GLC. Urine samples (3329) obtained at various gestational weeks were assayed. The samples from 94% of the normal pregnant patients in the third trimester revealed 10 micrograms/ml or more of estrogens. We studied the influence of Cd in various doses (0.5-4 mg/kg body wt) and at different survival intervals on the placental uptake and transfer to the fetus of vitamin B12 (vit. B12), zinc (Zn), alpha-aminobutyric acid (AIB), and deoxyglucose (DOX). These were chosen to represent various mechanisms of membrane transport. We show that vit. B12, which is accumulated in the placenta by a presumed receptor-mediated mechanism, is most easily disturbed by Cd. Thus, a significant decrease in transfer to the fetus was seen already 1 h after a high dose (4 mg/kg body wt) of Cd, and also at longer intervals (24 h) after low doses (0.5 mg/kg body wt). The transport of Zn (chemically similar to Cd) was also disturbed, but its inhibition was probably due in part to a decreased maternal serum concentration. The transport of AIB and DOX was largely unaffected. We conclude that inhibition of nutrient transfer to the fetus may be the underlying mechanism of growth retardation and possibly of the malformations produced by Cd. The technique involves retrograde insertion of a double catheter into the thoracic inferior vena cava from the right jugular vein. The outer catheter serves as a guide for the inner catheter which is advanced into either ovarian vein. The overall rate of success was 45% in 40 animals near term. Sampling of blood from the ovarian vein in the conscious animal remained possible for up to 17 days after surgery. In this report the significance of PGI2 on fetal platelets was studied. Platelet aggregation induced by ADP, collagen, and adrenaline was found to be augmented in maternal blood but suppressed in umbilical cord blood. The plasma beta-thromboglobulin levels were higher in maternal and umbilical cord blood compared with that of a control group. Plasma 6-keto-PGF1 alpha, TxB2, cyclic AMP, and cyclic GMP values were significantly higher in umbilical cord blood than in maternal blood. A much larger amount of PGI2-like substance was released from the umbilical vessels than from the placental vascular tissues. The generation of this substance was reduced in the umbilical vessels in patients with severe pre-eclampsia associated with intrauterine fetal growth retardation. These results indicate that various factors, such as PGI2, TxA2, and cyclic nucleotides, may co-exist at high levels in the fetus. A balance is needed to maintain physical interaction between platelets and the vascular wall in the fetal blood vessels. The average plasma caffeine was 6.5 (SD 2.8-14.8), plasma theophylline 2.9 (SD 1.3-6.7), plasma theobromine 2.1 (SD 0.9-4.9) mumol/l.
