{"title":"Epidemiology of soft-tissue sarcoma in the locomotor system. A retrospective population-based study of the inter-relationships between clinical and morphologic variables.","authors":"A. Persson","doi":"10.1111/J.1699-0463.1984.TB04416.X","DOIUrl":"https://doi.org/10.1111/J.1699-0463.1984.TB04416.X","url":null,"abstract":"From all soft-tissue malignancies reported to the Swedish National Cancer Registry in Southern Sweden (1.3 mill. inhabitants) from 1964 through 1978, 278 cases were accepted as sarcomas after histologic re-examination. All these were malignancy-graded on a four-grade scale, without knowledge of the clinical course. A number of clinical and morphological variables were recorded and subjected to uni-, bi- and multi-variate analysis. Follow-up was available in all patients. The annual incidence rate over all ages was 1.4/100,000. The mean age was 58 years and males dominated (1.3/1). Malignant fibrous histiocytoma, liposarcoma and leiomyosarcoma were the most common histologic groups. Three-fourths of the tumors were high-grade malignant (Grade III-IV). Sixty per cent were deep-seated, having a median size of 8 cm compared to 4 cm for the superficial tumors. One third of all tumors were located in the thigh. The histologic groups were characterized by age, tumor depth and size, the occurrence of pain, malignancy grade and five-year survival; it was seen that each group, with respect to at least one of the variables, differed significantly from all the other groups. Thus histologic classification seems to identify different tumor entities. Out of several pair-wise associations the strongest were as follows: histologic groups versus depth, size, malignancy grade and age; depth versus size; and size versus malignancy grade (Grade IV tumors being larger). The proportion of superficial and small tumors in this series is high, compared to several reported series, probably owing to \"referral selection\" in the previous studies. A study of three Lundehunds with typical symptoms, including wasting, diarrhea and ascites, revealed segmental distention of lymphatics resembling primary lymphangiectasia in man. Atrophy, fusion and balloon-like swelling of villi with occasional rupture of lacteals were seen. Subepithelial fluid accumulation caused bulging of the epithelium. Microvilli showed partial or complete atrophy. Thin and thick myofilaments and Z-lines or Z-like densities were found in all. Intermediate filaments were found in 4 alveolar and 4 embryonal rhabdomyosarcomas. A correlation was noted between the light- and electron microscopic appearance of the tumours regarding the degree of differentiation. A wide differentiation spectrum of the tumour cells was observed which could be correlated partly with normal fetal myogenesis. Ultrastructural differences were demonstrated in the alveolar and embryonal types. The typical histological mixture of Antoni type A and B tissue correlated well with the cytological findings. In the smears, the Antoni type A tissue was represented by tissue fragments with a fibrillar ground substance and slender, spindle-shaped cells forming obvious Verocay bodies in 24 cases, and the Antoni type B tissue by loose, microcystic fragments with single, elongated nuclei sometimes looking like fish-hooks and with indistinct, slender, cytoplasmic processes. The ancient neurilemomas, a variant of the ordinary neurilemomas, in this series were characterised both histologically and cytologically by nuclear hyperchromasia and polymorphism, which it is important to be aware of in order to avoid an erroneous diagnosis of sarcoma or probable sarcoma. The differential diagnosis is discussed and it is concluded that diagnosis of neurilemoma by fine-needle aspiration biopsy is possible, particularly when Verocay bodies and/or a fibrillar ground substance are present in the smears. The ages at onset for the four men and two women range between 16 and 54 years. The lesions were well circumscribed, rounded or oval, and had a diameter of about 1 cm. Light-microscopically the lesions consisted of a dense fibrocollagenous tissue with a variable degree of collagenization and cellularity, the peripheral zone often being most cellular and containing slit-like vascular spaces. Electron-microscopically two main cell types were identified: myofibroblasts and fibroblasts, the former dominating in cellular areas, the latter dominating in collagenized areas. The histochemical analysis indicated the presence of sulphated glucosaminoglycans within the collagenous matrix. Immunoperoxidase staining for factor VIII RAG-related antigen gave prominence to the endothelium of capillary-like blood vessels. The differential diagnosis against giant cell tumor of tendon sheath is discussed. We have used four different murine tumors with different abilities to metastasize and with different metastasis patterns. The elimination of the tumor cells was determined by gamma-countings from different organs. The results were compared with the elimination of heat killed and x-ray sterilized tumor cells. We found a poor correlation between on the one hand the clearance of tumor cells from the blood and distribution of radiolabelled cells and on the other hand the ability of the tumors to metastasize and the metastasis distribution upon i.v. injection. Patients with metastasis at the time of diagnosis were not included. All histologic material was re-evaluated and histologic malignancy-grading (four-grade scale) performed without knowledge of the clinical course. The surgical procedures were classified as marginal and broad excisions. Patient follow-up ranged between 3- and 18 years. Multivariate analysis of the data by Coxs proportional hazard regression techniques disclosed seven negative prognostic variables of primary significance; high malignancy-grade (IV and III), pain at rest, male sex, increasing age and tumor size, a marginal excision and an extracompartmental tumor site, in order of decreasing relative risk (5.9-1.9) as regards survival. A secondary variable, that of local recurrence, was then included in the model and was found to have a stronger influence on survival than any of the other variables. Patients with local recurrence had a mortality risk which was 8.3 times that of patients without local recurrence. A risk curve based on the prognostic variables having primary significance was constructed. By this risk curve, patients with very good or very bad prognosis could be identified. The results are important when evaluating the efficiency of different therapies in non-randomized trials. In such studies the prognostic variables could be used to identify patients having comparable prognoses. Pair fed rats served as controls. Changes in aortic morphology were examined by light- and transmission-electron microscopy (TEM). In addition, the endothelial permeability and the penetration through the aortic wall of albumin were studied 10 minutes, 24 and 48 hours after i. v. injection of human serum 131I-albumin (131I-HSA). TEM revealed extensive elastolysis in the arterial wall of D-pen-treated rats, consistent with an inhibitory effect on crosslink formation. In experimental animals excess deposition of collagen and glycoaminoglycans was observed in the subendothelial and medial layer of the aortic wall, together with prominent basal membrane substance around aortic smooth muscle cells. The aorta/serum-ratio and the radioactive build-up 24 and 48 hours after injection of 131I-HSA was reduced in animals treated with D-pen for 42 days, indicating an impeded transmural transport of tracer which may be caused by a steric exclusion effect of abundant hyaluronate. The endothelial ultrastructure was unaffected by D-pen, and no differences in aortic 131I-HSA radioactivity or aorta/serum-ratio were recorded between experimental and control groups 10 minutes after tracer injection, indicating that the permeability of the endothelial barrier to albumin remained unaffected by D-pen treatment. Twelve women had histologically benign, 4 had borderline and 3 malignant CP. Recurrent CP was diagnosed in 9 women within 1 to 15 years after initial treatment. Of the 19 women, 5 had associated in situ or invasive BC, 2 with the primary CP and 3 with its recurrences. In 1 case the BC was located within the confines of the CP, whereas in 4 women the CP and BC were separate lesions.
