Hallgrímsson","doi":"10.1111/J.1699-0463.1982.TB00079_90A.X","DOIUrl":"https://doi.org/10.1111/J.1699-0463.1982.TB00079_90A.X","url":null,"abstract":"All primary tumours from the urinary bladder, ureter and urethra submitted for a histological diagnosis in Iceland during the 20 years, 1955-1974, and available for review, were typed histologically according to the World Health Organization Classification published in 1973. The series included 237 tumours, and of these 94.5 per cent were from the bladder. Transitional cell carcinomas constituted the largest group, or 120 in males and 77 in females. For transitional cell carcinomas there was a good correlation between growth patterns, grades and pathological stages. Most of the papillary carcinomas were of grades 1 and 2 and all the purely infiltrating carcinomas were of grade 3. The papillary carcinomas were predominantly superficial and the purely infiltrating were deeply invasive and the degree of cellular anaplasia increased with the depth of invasion from the lamina propria to perivesical tissue and other organs. The incidence of transitional cell carcinoma of the urinary bladder is rising in Icelanders and the incidence in females is the highest reported among European nations. Industrial chemical carcinogens are an unlikely cause of bladder carcinoma in Icelanders but cigarette smoking, and a high consumption of coffee and salted food may play an important role.","PeriodicalId":77652,"journal":{"name":"Acta pathologica, microbiologica, et immunologica Scandinavica. It was convincingly demonstrated that gynecomasty begins in an active proliferating phase and ends in a fibrous inactive phase. Contrary to previous authors we found it justified that the formation of lobules, indistinguishable from what is seen in the reproduceable age in the female breast, is not necessarily related to the administration of exogenic hormones. In five (6.5%) cases, typical multicentric foci of intraductal epithelial hyperplasia were demonstrated--of the atypical hyperplastic or early intraductal carcinoma type. The number of cysts for each cm of mucosa analyzed (cystic index) was calculated. Intramucosal cysts occurred in 34 per cent of the patients. The resultant cystic index was 0.19 (S.D. +/- 0.40). The cystic index was not correlated to age or sex. Comparative studies demonstrated significantly higher (p less than 0.001) proportions of intramucosal cysts in the non-tumoral mucosa from patients with early gastric cancer (cystic index 3.13 - S.D. +/- 3.55). The possible association between benign intramucosal cystic dilatation of the gastric glands and the occurrence of gastric carcinoma deserves further investigation. Out of 314 malignant epithelial tumours 86.0 per cent were squamous carcinomas (subtypes: 6% microcarcinomas, 30% keratinizing, 45% non keratinizing and 5% small cell carcinomas), 9.0 per cent adenosquamous carcinoma, 3.5% adenocarcinoma and 1.5 per cent undifferentiated carcinoma. A mass screening for uterine cancer in the population, ages 25-59, led to an increased incidence of cervical carcinoma, due to the finding of early tumours (clinical stages IA and IB). The increase was most marked in the first five years of screening (1965-69). The screening did not increase the incidence of the more advanced stages and a longer observation will be needed to determine whether a decline will occur in such tumours. All the major histological types of cervical carcinoma appear to progress at the same rate through the clinical stages, from the early to the far advanced.","PeriodicalId":77652,"journal":{"name":"Acta pathologica, microbiologica, et immunologica Scandinavica. In each individual the shortest osteon diameter (D) and the corresponding Haversian canal diameter (d) were measured in a minimum of 20 completed secondary osteons by means of a micrometer eyepiece. Among normal males the areas of bone resorbed and formed increased with age (p less than 0.01), owing to an increase in the thickness of bone resorbed (p less than 0.01) and an unchanged thickness of bone formed. Among the females, both the areas of bone resorbed and formed decreased with age (p less than 0.05), owing to a reduction in the thickness of bone resorbed (p less than 0.05) as well as formed (p less than 0.001). Resorbed and formed areas were reduced in the epileptic (p less than 0.01) and acromegalic (p less than 0.01) groups but increased in the hypothyroid group (p less than 0.01) compared to sex- and age-matched controls. Neither the osteoporotic nor the hyperparathyroid group showed any alterations in osteon size. The Haversian canal diameter was slightly increased in the epileptic group but normal in the other patient groups. The observed changes reflect variations in the amount of work performed by osteoclasts and osteoblasts during bone remodelling and may be explained by variations in cellular activity and bone turn-over rates.","PeriodicalId":77652,"journal":{"name":"Acta pathologica, microbiologica, et immunologica Scandinavica. In electron microscopy, some tumour cells showed irregularly orientated sarcomere formation with distinct Z-bands. All cells contained rod-like inclusions with attached thin cytoplasmic filaments. When stained with antibodies against desmin many tumour cells showed a diffuse peroxidase reaction, but the tumour also contained cells with cross striations. After phosphotungstic acid hematoxylin staining, dense cytoplasmic inclusions were seen; these structures showed strong affinity to antibodies against desmin. Our results apparently confirm the suggestion that the rod-like inclusions in rhabdomyoma cells are modified Z-bands. The light microscopic appearance made a rhabdomyosarcoma strongly suspected because of the presence of polymorphous, acidophilic, rhabdomyoblast-like cells. The clinical course was that of an ordinary recurring inflammatory nasal polyp. The ultrastructural and immunohistochemical analysis showed the fibroblastic and histiocytic nature of the atypical cells as being of a probably reactive nature. On all tumours a histopathological grading was performed, and all except two were found to be high-grade malignant. The follow-up is from 2 to 20 years, and only two of the living patients have not yet been followed for 5 years. One patient has been lost during the follow-up period. In the remaining 27 patients the 5-year survival is 48%. The factors essential to the prognosis are: the tumour localization, size, grade of malignancy and radical surgical treatment. The coexistence of Recklinghausen's disease and malignant schwannoma seems not in itself to give a worse prognosis; it is rather the large tumours and unfavourable localizations in this group of patients that shortens the survival, the 5-year survival being 37.5%.","PeriodicalId":77652,"journal":{"name":"Acta pathologica, microbiologica, et immunologica Scandinavica. Nielsen","doi":"10.1111/J.1699-0463.1984.TB04427.X","DOIUrl":"https://doi.org/10.1111/J.1699-0463.1984.TB04427.X","url":null,"abstract":"Eight cases of primary synovial chondrometaplasia have been evaluated with respect to 1) number of metaplastic nodules, 2) their location in relation to the surface of the synovium, 3) the histochemical composition of the matrix of the nodules; number of nodules with 4) cystic degeneration, 5) osteoclastic/chondroclastic activity at the margin, 6) invasion of connective tissue and 7) calcification and/or ossification. Furthermore, the pleomorphism and hyperchromatism of the chondroid cells and their nuclei and number of mitoses were evaluated. The number of metaplastic nodules varied greatly, from a few to more than a hundred. Most nodules were located at the level of the original synovial surface or in the pedunculated prominences. Histochemically, two types of nodules were found and in about equal numbers. Connective tissue invasion, calcification and ossification were the most frequent changes; whereas cystic degeneration, which has not been described before, was the rarest. Only one of the cases showed all the different changes, and these within a considerable number of the nodules. Variation of the chondroid cells and their nuclei was considered as an expression of degenerative changes, and mitoses were not found.
