{"title":"[An epidemiological study of oral cancer in Taiwan].","authors":"C H Tsay, C H Chiu","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Southeast Asia, which includes Taiwan, has one of the highest incidences of oral cancer in the world. From a public health or a clinical dental viewpoint, it is important to study the epidemiologic characteristics of oral cancer in Taiwan. This paper presents epidemiologic data on oral cancer in Taiwan from 1981-1986. The data, gathered from Cancer Registry Center, Department of Health, Executive Yuan, ROC, is studied using the descriptive method. The age-standardized incidence rates for males and females were 4.45/100,000 and 1.42/100,000, respectively. The cumulative incidence rate (0-79 years) was 0.59% for males and 0.19% for females, respectively. The average age was 52.0 years for males and 53.4 years for females. The male/female ratio was 3:1. The most frequently affected sites were the tongue for males and major salivary glands for females. The incident rates were found to be significantly higher in central Taiwan, especially for males. {"title":"Bilateral mesiodentes of identical twins--a case report.","authors":"W K Choi, R C Chang, S T Chuang","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A pair of identical 10-year-old Chinese twins with bilateral mesiodentes of opposite orientation are presented. Three mesiodentes were conical and impacted, while the other one had erupted and was incisiform in shape. All roots of the mesiodentes were completely formed. Intrapair twin comparison showed a high degree of genetically determined concordance, with minor differences subject to environmental influences. The prevalence of mesiodens and rate of monozygotic twins in various ethnic groups are reviewed. {"title":"Assessment of periodontal disease in an adult population survey in Taipei city using CPITN and GPM/T indices.","authors":"T K Peng, J H Yao, K S Shih, Y J Dong, C K Chen, L Pai","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The community periodontal index of treatment needs (CPITN) has been recommended for epidemiological surveys of periodontal status and treatment needs. The prevalence of gingivitis, periodontitis and missing teeth in a study population has been determined using the CPITN and GPM/T (gingivitis: periodontitis: missing teeth) indices. In this study, a survey of periodontal disease was performed among the adult population of Taipei City using the CPITN index. A total of 673 dentated persons (male 339, 50.4%; female 334, 49.6%) were examined and surveyed. Data were then divided into five age groups, 15-19, 20-29, 30-44, 45-64 and 64+. The prevalence of periodontal disease was analyzed using the percentage or sextant from all teeth. Treatment needs were determined by the worst periodontal score per sextant. It was found that about 91% of subjects suffered from various degrees of periodontal disease in at least one tooth. Gingivitis and periodontitis were 86.9% and 33%, respectively. Among them 48.9% had missing teeth. The prevalence of periodontitis and missing teeth increased with age. Most of them involved oral hygiene instruction and scaling. About 11.0% of the subjects needed periodontal surgery. Higher proportions of attachment loss with gingival recession caused by destructive periodontal disease were found in older persons who did not require periodontal surgery. {"title":"[Effects of subgingival antimicrobial irrigation in chronic periodontitis--clinical observation].","authors":"J M Lin, F Y Peng, S C Lin","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>One week prior to the establishment of baseline records, 144 sites with a probing pocket depth of 5-7 mm in 13 periodontitis patients were treated with plaque control and root planning. Sites with periodontitis involvement in each patient were divided, by split mouth design, into 4 quadrants. Each quadrant was irrigated subgingivally with either 0.05% metronidazole or 0.05% ornidazole, or 0.2% chorhexidine, or 0.9% normal saline (control) once every 2-3 days for 4 weeks. The plaque index, sulcus bleeding index, probable pocket depth and gingival index were assessed at the baseline and at 4 weeks, 8 weeks and 12 weeks following initial preparation. All 4 clinical parameters in the ornidazole treated group showed significant improvement as compared with baseline values, but the improvements were not maintained to the end of the study. Except for improvement in the sulcus bleeding index and the gingival index in the ornidazole group, which was superior to that of the metronidazole group at the 4th week, there were no significant differences in other parameters between the ornidazole, metronidazole and chlorhexidine groups at the irrigation sites. {"title":"[An in vitro study of the antimicrobial effectiveness of ultrasonic instrumentation].","authors":"C Chan, S Y Chao","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Successful endodontic therapy is based upon the classic triad of diagnosis, adequate canal preparation, and obturation. Preparation for a root canal has two major components--debridement and shaping. Debridement, or cleaning, consists of removal of necrotic intracanal dentin and organic debris, and reduction of the bacteria inside the root canal. According to the research, the endontic ultrasonic system plays a valuable and significant and synergistic role in aiding the bacteriocidal mechanisms based on the cavitation and acoustic streaming effect. The purpose of this study was to compare the antimicrobial effectiveness of two newly developed endodontic ultrasonic devices and the conventional hand-filing techniques. Eighty single root canal teeth with deep caries or apical lesions were selected and evenly divided into 4 groups. The root canals were prepared using conventional hand filing or one of two newly developed ultrasonic filing systems, ENAC and CAVI-ENDO. The contents of the root canals before and after debridement were cultured in a thioglycolate culture tube. The results showed that in the ultrasonically prepared specimens irrigated with a normal saline solution, the microbial culture numbers were significantly reduced and obviously different from those prepared by the hand instrumentation technique. {"title":"[The efficacy of sonic and ultrasonic appliances on root canal debridement].","authors":"C P Chen, C L Yeh, Y K Yang, C C Ma, S Nieh","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The purpose of this study was to compare the debridement efficacy of hand, sonic (MM-3000), and ultrasonic (Cavi-Endo) appliances, utilizing the same K-type files and a constant flow irrigation system, in the mesial root canals of extracted human mandibular 1st and 2nd molars. Only those roots with a canal curvature of between 10 and 30 degrees, by Schneider's method, were used in this study. The instrumentation time per mesial root canal for each treatment group was also evaluated. Following canal preparation and histological processing, the middle and apical cross-sections were examined to determine the percentage of canal and isthmus cleanliness, and the percentage of planed canal walls in the main canal area. Statistical analysis indicated no significant difference in canal and isthmus cleanliness at the middle and apical levels. All techniques achieved a high level of canal cleanliness. However, step-back hand instrumentation had the greatest percentage of planed canal walls in the main canal area at every portion of the root. {"title":"[Isolation and biotyping of Streptococcus mutans among preschool children in Kaohsiung city].","authors":"H S Chen, Y F Liou, S T Huang","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Strains of streptococcus mutans were isolated from the saliva of preschool children in Kaohsiung City. The unstimulated whole saliva samples were inoculated onto modified Gold's agar plates and incubated for 24-48 hours anaerobically at 37 degrees C. Strains of S. mutans were classified according to Shklair and Keene's biochemical methods and biotyped according to the methods of Farghaly et al. S. mutans could be detected in 102 (89.5%) of 114 preschool children. The most common biotype of S. mutans in single isolation was biotype I (72.9%). The percentages of the other biotypes are as follows: biotype II (5.2%), biotype III (7.3%), biotype IV (7.3%) and biotype V (7.3%). Our study revealed that the deft index was significantly higher in children who were biotype I carriers than in those who were nonbiotype I carriers. A case of aneurysmal bone cyst arising in the right mandible of a 15-year-old Chinese boy was presented with both the roentgenographic and microscopic findings. In view of the high recurrent rate and the large destructive mandibular lesion, segmental resection with immediate bone grafting for reconstruction and a conscientious follow up regimen was employed. The etiology and pathogenesis of this interesting lesion remained unclear but several theories were reviewed and the differential diagnosis of this lesion was also discussed. This unusual presentation of a calcifying odontogenic cyst is discussed and its significance for the clinician is reviewed. A calcifying odontogenic cyst may present some problems for the differential diagnosis both clinically and histopathologically. {"title":"A cross-sectional study on the skeletal development of the hand and wrist from preadolescence to early adulthood among Chinese in Taiwan.","authors":"H F Chang, K M Wu, K C Chen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In order to study the skeletal development of the hand-wrist among Chinese in Taiwan, a total of 1,679 hand-wrist radiographs were collected from the files in the Division of Orthodontics, School of Dentistry, National Taiwan University and the Orthodontic Clinic of the National Taiwan University Hospital. The ages of the subjects ranged from 6.3 years to 22.7 years. The nine commonly used ossification events, PP2 = ,MP3 = ,S, MP3cap, DP3u, MP3u, Riu, Rau and Rcu, were used in this study. It was found that the sequence of the nine skeletal maturation stages did not differ between males and females and was generally in agreement with similar studies among other ethnic groups. A statistically significant difference did exist between the males and females in the mean ages of onset of the skeletal maturation stages. All of the skeletal stages were more advanced in the females than in the males. The mean ages of the nine skeletal maturation stages obtained in this study could be used as a reference for daily pedodontic and orthodontic practice as well as for related studies.
