Colonic duplication presents diagnostic and therapeutic difficulties especially in neonates. We report on two patients, a child and an adult with tubular duplication. Treatment consisted of using the duplicated loop for a pull-through procedure in the first patient and resection of the duplicated loop in the second. To obtain reduction, an extensive surgical soft-tissue dissection is usually required. The accompanying damage to vessels, ligaments and articular cartilage increases the risk of symptomatic arthritis of the wrist. To avoid this complication as much as possible, a closed technique of treatment has been developed. The method consists of radiocarpal distraction with an external fixator followed by percutaneous reduction and immobilization with the same external fixator. The records were studied of 25 patients with acute intestinal obstruction due to massive adhesions. Since conservative measurements were unsuccessful, the patients were treated with internal intestinal splinting by means of a Miller-Abbott tube. These 25 patients underwent a total of 72 operations, 36 were performed for mechanical obstruction. Conservative treatment alone was effective during 25 admissions. The complaints of the patients lasted five years on an average. After lysis of adhesions the Miller-Abbott tube was introduced either via the nose, via a gastrostomy or via an enterostomy. The tube was left in situ for three weeks and then gradually withdrawn. There was no hospital mortality. There was one postoperative complication: a tube had to be removed under general anaesthesia. Long-term follow-up of the patients varied from 4.5 to 19 years with a mean of 11.3 years. One patient with recurrent intestinal obstruction due to adhesions, required surgical intervention after one year. A second patient with Peutz-Jeghers syndrome needed surgery because of an intestinal adenoma after six years. In six patients, the treatment consisted of closed reduction and immobilization in a below the elbow cast for 12 to 16 weeks. Five patients had a compromised healing of the scaphoid fracture. The frail vascularization of the scaphoid, extensive ligamentous injury and possible mid-carpal instability in case of perilunate fracture dislocation as well as a non-anatomical reduction of a scaphoid fracture are causative factors in ensuing scaphoid pseudarthrosis. If anatomical reduction of a perilunate fracture dislocation cannot be obtained, open reduction and internal fixation should be considered. Thirty-two patients had concomitant fractures of the acetabular rim and seven had also femoral head fractures. Treatment consisted of immediate reduction, followed by gentle traction during one week and progressive weight-bearing over six weeks. Thirteen patients needed internal fixation of the acetabular fracture. Fourty-six patients could be followed for a median of 6.5 years. Fifteen patients, all with associated fractures, had late complications: peri-articular ossifications (5), avascular necrosis of the femoral head (1), mild osteo-arthrosis (5) and pain of unknown origin (4). No complications were noted in patients with simple posterior dislocation of the hip. Simple traumatic posterior hip dislocation has an excellent prognosis. The Dutch hospitals are classified into three categories depending on the type of residency in general surgery. Overall, 80 per cent replied. Antibiotic prophylaxis in elective cholecystectomy is given in 76 per cent of the clinics (100/132), and single-dose prophylaxis is employed in 28 per cent of the clinics (28/100). In patients with acute cholecystitis, emergency surgery is the treatment of choice in 108 hospitals (82%). Differences in antibiotic prophylaxis between the three categories of hospitals include the omission of prophylaxis in elective cholecystectomy in 31 per cent of the C-clinics (vs. 10% in A-clinics and 16% in B-clinics), and the use of single-dose prophylaxis in 13 per cent of the B-clinics (vs. 30% of the A-clinics and 31% of the C-clinics). The initial aspecific symptoms mostly lead to late recognition of acute mesenteric ischaemia which leads to intestinal infarction. Intestinal infarction with signs of peritonitis has a very high mortality. Since 1982, a pro-active approach is followed in our department. After thorough exclusion of other causes, patients with serious acute abdominal complaints undergo an emergency angiography and/or emergency laparotomy. After confirmation of the diagnosis, primary vascular reconstructive surgery is performed if possible. We have treated eight patients with acute mesenteric embolism according to this policy. Per- and post-operatively, three patients died of extensive intestinal infarction. The five surviving patients were on a normal diet and had normal bowel functions at the time of discharge from the hospital. Histopathological examination of the resected iliac artery showed features of fibromuscular dysplasia. Dissection of the external iliac artery without involvement of the aorta is extremely rare with only two previously reported cases.
