Primary aortoduodenal fistulas are a rare complication of aortic aneurysms. Given a high degree of suspicion, this otherwise lethal disease can successfully be treated by surgery. The so-called herald bleed usually allows time for investigation and diagnosis. In emergency operations, a primary repair of the duodenum and replacement of the aortic aneurysm with a Dacron prosthesis is advised. Decompression of the duodenum, protection of the graft with an omentum flap and prolonged antibiotics can minimize the risk of postoperative complications. {"title":"Diagnostic problems of scaphoid fractures: the value of radionuclide bone scintigraphy.","authors":"E J van Beek, M M van Buul, A H Broekhuizen","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The diagnosis fracture of the scaphoid bone based on mere X-ray examination is unreliable. A correct diagnosis of scaphoid fractures is essential for the prevention of avascular necrosis of the proximal fragment and of pseudo-arthrosis. Thirty-three patients with a clinically suspected scaphoid fracture and a normal radiograph were examined. These patients had supplementary 99mTc-MDP scintigraphy. Eight of the 33 patients had normal scans, 12 patients had a scaphoid fracture and 17 had other fractures. We have retrospectively analysed the incidence of complications, risk factors for the occurrence of complications and patient satisfaction in 120 catheters. The total number of complications was 30, leading to the removal of 15 catheters. The majority of complications were infectious, occurring 28 times and leading to catheter removal in 13 patients. Other complications (i.e. catheter thrombosis) were rare. The only identifiable risk factor for the occurrence of complications was the male gender, associated with a higher rate of catheter infection. Patient satisfaction after catheter insertion, assessed by a questionnaire to which 24 of the 30 patients responded, was high. The diagnosis of this condition may be difficult and is often missed. Swelling, dislocation and a palpable lump are the main clinical findings. X-ray findings, the key to the diagnosis, are loss of parallelism, interruption of the so-called M-line and overlapping of articular surfaces. {"title":"Extended lymph-node dissection for gastric cancer: a challenge for better survival results.","authors":"J T Plukker, G H Kampschöer","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Most Japanese studies have better survival results for gastric cancer after surgery compared to the results reported by Western authors. Japanese surgeons trace their success to early detection and extended lymph-node dissection. Analysis of data from several Japanese and Western surgical departments could not substantiate factors other than the surgical procedure as an explanation for the better survival rates in advanced gastric cancer in Japan. Whether extended lymph-node dissection actually improves survival results or merely rectifies classification which influences stratified survival results, remains to be justified. A prospective randomized study, comparing the Japanese R2 with R1 resection, is initiated in The Netherlands to answer this question.
