The objective of this study is to determine the prevalence, length, and passage of the anterior loop of the mandibular canal, as well as the quantity of alveolar bone that is coronal to the canal, to help with implant placement in the anterior mandible. Cone-beam computed tomography (CBCT) scans of 124 patients with 248 hemi-sections were evaluated. Anterior loop prevalence was determined using reconstructed panoramic and cross-sectional views; length was measured as the distance between the most mesial aspect of the mental foramen to the most mesial aspect of the anterior loop on cross-sectional views. The bucco-lingual position of the anterior loop inside the mandible, and the apico-coronal dimensions of the alveolar bone above it, were measured on cross-sectional views to determine the passage of the anterior loop and the bone available coronally, respectively. The effects of gender, age, side, and dentate status on the prevalence and length of the anterior loop were analyzed statistically. Prevalence of the anterior loop at the patient and hemi-section levels was 25% and 24%, respectively, and its median length was 1.63 mm (0.52-3.92 mm). The anterior loop was apical to the mental foramen and mostly located within the buccal or middle one-third of the alveolar ridge, with an average height of coronal alveolar bone of 17.12 mm. Gender, age, side, and dentate status did not affect anterior loop prevalence and length. In conclusion, owing to great variation, a case-by-case CBCT evaluation of the anterior loop is necessary before placing implants in the anterior mandible.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"34 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"88963466","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The study conducted medical record reviews and clinical/radiographic assessments of 10 patients with severe maxillary atrophy who had undergone reconstruction with autogenous iliac crest grafts and rehabilitation with dental implants between 2008 and 2011. Patients were assessed for the survival of the implants, considering: implant diameter and length, smoking status, diagnosis of diabetes, type of loss and region of implant loss. In addition, a questionnaire with specific questions on the patients' satisfaction with the treatment was administered. Seventy-six implants were installed in the sampled patients and only one loss was observed (late loss in the anterior maxilla region) after an average follow-up of 7.9 years, which corresponds to a 98.60% survival rate. The installed implants were of the most frequently-used dimension (3.75x10mm).One sampled patient was diabetic and a second patient was both diabetic and a smoker. No loss of implants was observed in these two patients. All patients reported being completely satisfied with the treatment and would undergo the procedure again or refer it to a friend/relative. Six patients reported regular maintenance of the prosthesis and only 3 had changed the prosthesis prior to the time of questionnaire administration. The results of this limited study with a restricted sample population suggest that the reconstruction of the maxilla with an autogenous iliac crest graft provides both adequate implant survival and patient satisfaction.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"183 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83554126","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This versatile procedure has many indications including tooth root coverage, thickening of gingiva and improvement of the quality of the crestal gingiva. Several techniques have been described for graft harvest from the palate. Reported complications from these techniques include pain, inflammation, bleeding, flap necrosis and infection in the donor site. All patientswere rehabilitated through a full-arch restoration supported by four or six immediately loaded implants.Clinical and radiographic examinations were scheduled every 12 months to evaluate implant success rates and crestal bone levels. Significant differences in crestal bone levels and success rates between straight and tilted implants were investigated by means of independent statistical analysis and differences were regarded as significant if p < 0.05.70 straight and 50 tilted implants were placed to rehabilitate 14 mandibles and 12 maxillae in 20 patients. After a follow-up of 12 to 36 months, survival rate was 97.1% for straight implants and 96.0% for tilted implants; while success rate were 94.3% and 94.0%, respectively. Both success and survival rates were not significantly different (P>0.05). A 0.5 ± 0.4 mm changes in crestal bone level was recorded for straight implants, while that of tilted implants was 0.6 ± 0.4 mm (P>0.05).Straight and tilted implants seem to have similar behavior after immediate loading rehabilitations. After functional loading, straight and tilted implants do not differ significantly as far as their clinical outcome is concerned.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"52 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"82273824","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

These hollow spaces can act as a reservoir of bacteria which interferes with the health of peri-implant tissues. The aim of the present in vitro study was to compare the effect on bacterial leakage at the implant-abutment interface of applying different intermediate materials. A total of 75 implants (15 implants per group) were used. Implants were divided into 5 groups upon the material applied to the implant-abutment connection: 1) Atridox, 2) chlorhexidine, 3) Gapseal silicone, 4) saliva and 5) no material. All the implants were inoculated with 0.1 µl of Aggregatibacter actinomycetemcomitans then the related material was applied to each group. Abutments were connected to the implants and appropriate torque was applied as recommended by the manufacturers (Implantium, Dentium, Korea, Seoul) Any penetration of the bacteria was determined by evaluating the turbidity of the broths. Bacterial contamination was found in all samples with differences in time of penetration ; in groups 1, 2 and 3 the contamination was found within 7, 5 and 6 days respectively on average. In group 4 and 5 both within 4 days on average. The present study confirms the efficacy of Atridox applied to the implant-abutment interface in causing significant delay in bacterial penetration. It was also concluded that the presence or absence of saliva doesn't have any effect on the time of bacterial leakage.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"7 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"87851182","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The bonering technique followed a standardized protocol. The alveolar ridge was prepared using a congruent trephine, and depending on the defect size, an allograft bonering with an outer diameter of 6 - 7 mm was placed. The height of the bonering was trimmed with a diamond disc to the required length. The average height of vertical augmentation was 5.5 mm. Implants were inserted through the bonering into the native bone of alveolar ridge. After 6 months, dental implants were exposed, and dental prosthetics were placed. Of 81 implants placed with the bonering technique, two failed during a 12-months follow-up, corresponding to a success rate of 97.5%. One year after surgery, the allograft bonering exhibited an average vertical graft shrinkage rate of 8.6 %. In conclusion, the allograft bonering technique was associated with a favorable outcome and in cases with large vertical defects, both treatment time and donor-site morbidity could be reduced.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"9 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75531591","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Bioactive glasses have been shown to possess antibacterial effects against S. mutans and other microorganisms. This in vitrostudy was performed to investigate the influence of bioactive glass air-abrasion on S. mutans biofilm on sandblasted and acid etched titanium surfaces. Sandblasted and acid etched commercially pure titanium discs were used as substrates for bacteria (n=107). The discs were immersed in an S. mutans solution and incubated for 21 h to form S. mutans biofilm. Twenty colonized discs were subjected to air-abrasion with the Bioglass®45S5 (45S5 BAG), experimental zinc oxide containing bioactive glass (Zn4 BAG) and inert glass. After the abrasion, the discs were incubated for 5 h in an anaerobic chamber followed by an assessment of viable S. mutans cells. Surface morphology was evaluation using SEM (n=12). Thrombogenicity of the glass particle abraded discs (n=75) was evaluated spectrophotometrically using whole blood clotting measurement at predetermined time points. Air-abrasion with 45S5 and Zn4 bioactive glass eradicated S. mutans biofilm. Significantly less viable S. mutans cells were found on discs abraded with the 45S5 or Zn4 BAGs compared to the inert glass (p<0.001). No significant differences were found in thrombogenicity since blood clotting was achieved for all substrates in min 40. Air-abrasion with BAG particles is effective in the eradication of S. mutans biofilm from sandblasted and acid etched titanium surfaces. Zn4 and 45S5 BAGs had similar biofilm eradicating effects but the Zn4 BAG could be more tissue-friendly. In addition, the steady release of zinc ions from the Zn4 may enhance  the bone regeneration around the titanium implant and may thus have the potential to be used in the treatment of peri-implantitis. The use of either BAGs did not enhance the speed of blood coagulation.","PeriodicalId":77634,"journal":{"name":"The International journal of oral implantology : implantologist","volume":"33 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2019-12-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"81480544","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

A meta-regression was used to evaluate the effects of the \"fixed effects\" model on the implant survival, prosthesis success rates and marginal bone loss with follow-up time of 1 year and 3 years. Of the 11 studies included, The overall pooled 1-year implant survival rates were 98.14% for NDIs and 98.20% for RDIs. The overall pooled 3-year implant survival rates were 98.71% for NDIs and 98.84% for RDIs. The corresponding values for 1-year prosthesis success rates were 96.94% for NDIs and 99.25% for RDIs. The corresponding values for 3-year prosthesis success rates were 89.25% for NDIs and 96.55% for RDIs. The meta-regression showed that no significant difference between narrow diameter implants and regular diameter implants in implant survival rates, prosthesis success rates and marginal bone loss(MBL) in 1-year and 3-year follow-up (P > 0.05). This meta-analysis concluded that the diameter of the implant did not affect its survival, prosthesis success rates and MBL in 1 and 3 years. The use of a narrow diameter implants instead of bone augmentation procedures with regular diameter implants does not affect its survival, prosthesis success rates and marginal bone loss in a short-term and a middle-term. Thirty-five implants were installed in seven patients (mean age 65.8±6.7years). Implants were divided into two groups, according to platform design: G1 - MT, installed 2mm infra-osseous and G2 - EH, positioned according to Branemark protocol. Patients were evaluated at baseline (T0), 21 days (T1), 3 months (T2), 6 months (T3) and 12 months (T4) after installations. Bone loss and alveolar density were evaluated by standardized periapical radiographs and bacterial profile with checkerboard DNA-DNA hybridization. The values for optical alveolar density, peri-implant bone loss and bacterial profile of each group were evaluated according to the bacterial complexes and were compared by Wilcoxon test. P-values ≤ 0.05 were statistically significant. After 3 months, alveolar bone loss was significantly higher in the G2 (T2-T0 p=0.006, T3-T0 p=0.003, and T4-T0 p=0.005). No significant differences between G1 and G2 groups were observed for optical alveolar density. Microbiological analysis showed similar profiles between studied groups, however, there were significantly higher counts of Tannerella forsythia (p = 0.048), Campylobacter showae (p = 0.038) and Actinomyces naeslundii (p = 0.027) in G1 after 12 months.
