{"title":"[Bronchoscopy in the post-therapeutic monitoring of patients with bronchial carcinoma].","authors":"I Cucević, B Cucević, M Roglić, I Pongrac","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A study of 970 patients treated for bronchogenic carcinoma (operation, cytostatic therapy, irradiation) reveal that only 122 (12.5%) were evaluated by the bronchoscopy. Among 47 (38.5%) out of 122 patients recidivation was confirmed (cytology or histology). The efficacy of radiology and sputum cytology is poor. Only 22/47 (47%) had radiologic evidence of recidivation and 18/47 (38%) had positive sputum cytology. Radiology and sputum cytology reveal recidivation in 27/47 (57%) patients. {"title":"[Immunologic characterization of alpha-bungarotoxin binding proteins in human microcellular lung carcinoma].","authors":"N Huković","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Alpha Bungarotoxin, neurotoxin of long chain, is used very often as a tool for studying of muscular nicotinic acetylcholine receptors. It was found out that six human small-cell carcinoma cell lines tested had alpha Bungarotoxin binding sites. Using monoclonal and polyclonal antibodies alpha Bungarotoxin binding sites were found more similar to neuronal nicotinic receptors than to muscular nicotinic receptors. {"title":"[Approach to artificial respiration using differential ventilation in open thoracic surgery].","authors":"V Majerić-Kogler, B Maldini, N Hodoba","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The purpose of our study was to correct the vertical ventilation perfusion anomaly in lateral position using a differential ventilation with the two identical electronic synchronized \"Servo\" ventilators, during the thoracotomy. Our study included 44 patients. The differential ventilation has been investigated on the two clinically standardized simplified models. In model 1 distribution of the ventilation volume dependent-independent lung was 50%/50% with a selective PEEP in the dependent lung of 10 cm/H2O. In model 2 distribution of ventilation volume dependent-independent lung of 70%/30% with selective PEEP in the dependent lung of 5 cm H2O. {"title":"[Study of bronchial reactivity using dry, cold air and eucapnic hyperventilation].","authors":"N Mojsoski, F Pavicić","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The paper is concerned with diagnostic value provocation test with cold dry air and eucapnic hyperventilation for testing bronchial reactivity. Forty healthy subjects, 90 patients with bronchial asthma, 20 with chronic bronchitis, 2 with sarcoidosis, 14 with pollenosis, one with mucoviscidosis and 22 with dispnoa or long-lasting dry cough were tested. The total of 149 provocations have been carried out in patients and 40 in healthy subjects. Airways resistance after provocation with cold dry air and eucapnic hyperventilation in patients with bronchial asthma is increased over 100% in 13 (14%), over 200% in 12 (13%), over 300% in 25 (26%) and over 500% in 40 (47%) patients. Less intensive asthmatic attack was provoked in 14 (15%), moderate attack in 18 (20%) and severe attack in 58 (65%) patients with asthma. Provocation time lasted from 2 to 9.9 min. Inspired air temperature ranged from -23 degrees C to -26 degrees C. Airway resistance was measured before and after provocation by means of Bronchoscreen. Central airway resistance was increased in 2 patients, peripheral resistance in 78, and both resistances in 10 patients with bronchial asthma. {"title":"[Bronchial responsiveness in hops processing workers].","authors":"B Meznar, S Kajba","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In small group of workers (n = 17) who had been during their working process (storing and packing of hop) exposed to the inhalation of hop dust, the bronchial responsiveness with metacholine was studied according to cumulative method. {"title":"[Results of bronchodilation tests in workers exposed to occupation respiratory noxae].","authors":"V Popović, J Jovanović, R Todorović, M Arandelović, O Momcilović, D Dordević","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In order to detect and assess the spastic components in airway obstruction of workers exposed to inhalation of different nonorganic and organic dusts, 129 workers of metal industry and 135 workers of textile industry were examined. The workers of metal industry were exposed to the following respiratory noxa: toluol, paint aerosol, mineral dusts (SiO2 up to 10%), asbestos dust, metalic dust and several chemical noxa (acetone, CO, CO2, ZnO, FeO and petrol). In textile industry the dust of vegetal origin was detected with unfavourable micro climatic factors. In both groups the similar results were found with very high percent of spastic components. After bronhodilatatory testing the most significant differences were found in the following tests: Raw, FMF25-75, FEF75-85, components. Several significant spastic components were present in workers of metal industry which are due to the polluted working area. In relation to the mean values of tests, the greatest number of different results was found in FEF75-85 and Raw tests. {"title":"[The role of bronchoscopy with a flexible bronchoscope in the diagnosis of pulmonary tuberculosis].","authors":"V Gorjup","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Bronchoscopy with flexible bronchoscope has been proved as a useful method that has been applied also in the diagnostics of pulmonary tuberculosis during the recent years. In order to evaluate its role and use at the Institute for Diseases of the Chest and Tuberculosis Golnik documentation of 51 patients treated during 1985 and 1986 was surveyed. Prior to the examination from all patients the sputum on Mycobacterium tuberculosis was obtained at least three times and it was negative. Aspirations were obtained from 40 patients and the diagnosis was confirmed in 21 patients by recovery from tubercle bacilli specimens (52.5%). Transbronchial biopsy was performed in 34 patients and it was positive in 27 (79.4%), out of them in 7 patients the biopsy sample of the lungs was bacteriologically (38.9%) and in 23 patients histologically positive (67.6%). By bronchoscopy with flexible bronchoscope the final diagnosis was confirmed in 40 patients (78.4%). Bronchoscopy with flexible bronchoscope as well as transbronchial biopsy have proved to be a successful method in the diagnostics of active pulmonary tuberculosis. {"title":"[The effect of positive end-expiratory pressure on gas exchange and hemodynamics].","authors":"A Spec-Marn, L Tos, B Stangl","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The effects of positive end-expiratory pressure (PEEP) on arterial and mixed venous blood oxygenation as well as the effects on systemic and pulmonary circulation were studied in 15 patients with adult respiratory distress syndrome (ARDS) on mechanical ventilation. The measurements of arterial and mixed venous blood oxygen pressure (PaO2, PvO2), heart rate (HR), central venous pressure (CVP), pulmonary wedge pressure (WP), mean pulmonary arterial pressure (Ppa), mean systemic arterial pressure (Psa) and cardiac output (CO) were performed at 0, 5, 10 and 15 cm H2O PEEP. The positive effect of + 10 cm H2O on arterial oxygenation was found (PaO2 raised from 11.49 +/- 3.5 kPa to 13.3 +/- 3.3 kPa). Ppa raised too, from 23 +/- 7.6 mm Hg to 26 +/- 7 mm Hg. Psa and CO lowered at + 15 cm PEEP. Psa from 85 +/- 17 mm Hg to 73 +/- 17 mm Hg as well as CO from 8.22 +/- 1.1 l/min to 6.91 +/- 1.40 l/min. Authors conclude that PEEP can be accepted form of therapy in the treatment of refractory hypoxemia in patients with ARDS. Despite this beneficial effect, PEEP may adversely affect cardiovascular function.
