[Reconstruction of occlusal function with osseointegrated implant following mandibular resection]. [下颌切除术后骨整合种植体的咬合功能重建]。

Case 1: The patient was a 19-year-old man. Partial resection of the mandible was carried out under the diagnosis of odontgenic keratocyst of the left lateral incisor to the 1st premolar region. Iliac bone grafting was performed as secondary surgery. After confirmation of the fusion of the grafted bone, the occlusal function was restored by use of an osseointegrated implant. Case 2: The patient was a 36-year-old man. After segmental resection of the mandible under the diagnosis of ameloblastoma of the left 2nd premolar to the right 2nd premolar region, mandibular reconstruction was carried out with an A-O plate. Iliac bone grafting was performed as secondary surgery. After fusion of the grafted bone had been confirmed, the A-O plate was remounted and the fixture was immediately implanted. Five months later, vestibuloplasty by free skin grafting and attachment of the abutment were carried out. A superstructure was mounted. Case 3: The patient was a 22-year-old woman. The histopathologically confirmed odontogenic myxoma in the right mandible posteriorly from the second premolar was resected. Immediate reconstruction of the mandible was carried out by use of an iliac bone graft. After confirmation of the fusion of the grafted bone, the occlusal function was restored by use of an osseointegrated implant. Case 4: The patient was a 57-year-old woman with carcinoma of the right lingual margin and mouth floor. The tumor was resected with the mandibular margin. Immediate reconstruction of the tongue and mouth floor with a major pectoral muscle skin flap was carried out. Both the enzyme activity and BP content of the cells were transiently increased by dBcAMP treatment with both the maximal activity and content at 1 to 2 days after the treatment was started. The effect of dBcAMP was not detectable at concentrations lower than 10 microM and reached the plateau level at 500 microM in a dose-dependent manner. EC50 values of dBcAMP for increasing the enzyme activity and BP content were the same, approximately 200 microM. Sorbitol content was abnormally high in 21 cases out of 23 NIDD patients. Sorbitol content in the non-neuropathy group and neuropathy group was 47.3 +/- 11.9 and 59.6 +/- 23.6 nmol/gHb, respectively. In the non-cataract group and cataract group, it was 49.0 +/- 17.6 and 66.0 +/- 23.5 nmol/gHb, respectively. The contents in the Scott I group and Scott II + III group were 54.9 +/- 20.7 and 58.7 +/- 24.0 nmol/gHb, respectively. Sorbitol content in the non-nephropathy group and nephropathy group was 52.8 +/- 19.8 and 61.1 +/- 21.9 nmol/gHb, respectively. DADA increased OU at high concentrations, but not at low ones. However, CV became higher as the concentration became lower. DADA led to no remarkable toxicity when used alone. Analysis of junior high school and high school students].","authors":"K Koyama,&nbsp;T Yasui","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>In this study, we carried out epidemiological surveys on some of the factors that seem to be related to periodontal disease in adolescence, and tried to clarify the cause of periodontal disease in young people. Moreover, the direction needed for effective education of school students in the field of school dental health was discussed. The subjects of this study were 232 junior high school students and 142 high school students. Oral conditions evaluated in terms of the PMA index, the Community Periodontal Index of Treatment Needs (CPITN) and the salivary occult bleeding level for periodontal disease, the Simplified Oral Hygiene Index (OHI-S) for oral hygiene, and the DMFT index for caries experience. Psychological tests used in this study were the Yatabe-Guilford test (Y-G test), the Cornell Medical Index (CMI), and the Manifest Anxiety Scale (MAS). The dental questionnaire developed by Maeda was changed to a simpler form and used to obtain information on personal aspects or parameters such as the ability to recognize one's oral condition, their knowledge of proper dental health, consciousness about and interest in dental health, dental experience and health behavior. The condition of periodontal disease and the OHI-S value tended to improve as the school grade went up, especially among junior high school students. Regarding the personal aspects the subjects who had knowledge of proper dental health, consciousness about and interest in dental health, and health behavior tended to show a lower PMA index; and there were differences in these parameters between junior high school students and high school students. Among junior high school students the results suggested that consciousness about and interest in such factors as gingival bleeding, bad breath and sticky feeling in the mouth caused better dental health behavior. In the psychological aspects, the PMA index value tended to be lower in the low-anxiety group than in the high-anxiety group. From these findings, periodontal disease in adolescence appears to be affected by not only the oral hygiene condition but also personal parameters and psychological factors. Employing 20 healthy male subjects, we studied the changes in the rate x pressure product due to a body position change from the sitting position to the supine position in a dental chair. The following results were obtained: 1. The change from the sitting position caused a 10% decrease in the rate x pressure product, and this value did not return to the initial sitting position value even after 5 min in the supine position. 2. The heart rate tended to decrease due to the position change. 3. Immediately after the position change, the systolic blood pressure decreased significantly (p less than 0.05), but it tended to return to the initial value with the lapse of time. Interaction between growth factors (TGF-beta and EGF) and two hormones insulin and 1, 25 (OH)2D3, which have been noticed to accelerate the differentiation of the cells, were also studied, and the following results were obtained: 1) TGF-beta decreased alkaline phosphatase (ALPase) activity in a dose-dependent manner, and the inhibitory effect was not blocked by indomethacin, suggesting that the effect of TGF-beta on the cells may not be mediated by prostaglandins. Inhibitory effects of ALPase antagonists (L-phenylalanine, L-homoarginine, levamisole) on the activity were not affected by TGF-beta. TGF-beta showed no evident effect on the DNA synthesis (incorporation of [3H] thymidine) and collagen synthesis (incorporation of [2, 3-3H] proline into the collagenase-digestible protein) of the cells. 2) EGF stimulated the incorporation of [3H] thymidine and inhibited the ALPase activity. The inhibitory effect was not blocked by indomethacin, indicating that the EGF effect is not mediated by prostaglandins. Collagen synthesis was significantly inhibited by EGF. 3) Insulin showed a weak but significant inhibition of the DNA synthesis. Insulin increased the ALPase activity evidently, and accelerated the collagen synthesis significantly. 4) The vitamin 1, 25 (OH)2D3 significantly increased the ALPase activity though no significant changes were observed in the DNA synthesis and collagen synthesis. 5) PTH had no evident effect on the DNA synthesis and ALPase activity, but did tend to accelerate the collagen synthesis. 6) A study on the interaction between insulin and EGF or TGF-beta revealed that the acceleration of DNA synthesis induced by EGF was inhibited when the factor was combined with insulin, and the increase in ALPase activity elicited by insulin was inhibited by EGF and weakened by TGF-beta significantly when these factors were added simultaneously with the insulin. Or viewed another way, the inhibitory effect of EGF or TGF-beta on the ALPase activity was antagonized by insulin. The population of cells with flattened shape was increased by the treatment with RA+dBcAMP, or with RA+dBcAMP+NGF, while the treatment with NGF alone had almost no effect on the morphology. The NADPH-DPR activity was increased by the treatment of the cells with RA+dBcAMP+NGF and with RA+dBcAMP by about 2.2-fold and 1.8-fold, respectively, of that detected in the control (untreated) cells. Also in this case, NGF alone failed to increase the enzyme activity. On the other hand, no change in NADH-specific dihydropteridine reductase (NADH-DP) activity was detected by the treatment of the cells with either RA+dBcAMP, RA+dBcAMP+NGF, or NAF alone. However, three reaction steps are irreversible, i.e., those catalyzed by hexokinase, phosphofructokinase, and pyruvate kinase; for in each of these reactions there occurs a large negative free-energy change, and these are reactions thus bypassed by alternate enzyme-catalyzed reactions. Pyruvate kinase (EC 2.7.1.40, PK) plays an important role in controlling glycolysis and gluconeogenesis. To clarify the characteristics of glycolysis in dental pulp, we examined the enzymatic properties of pyruvate kinase from pig dental pulp and compared them with those of the enzyme from pig brain. 1) Pyruvate kinase from dental pulp and brain were purified by use of ammonium sulphate fractionation, phosphocellulose colum chromatography, and isoelectric focusing. The prepared enzymes showed a single protein band on SDS polyacrylamide gel electrophoresis. 2) The subunit molecular weight of dental pulp and brain enzymes was determined to be 63,000 and 59,000, respectively. 3) Substrate inhibition of dental pulp and brain enzymes by phosphoenolpyruvate was not observed, and the relationship between reaction velocity and substrate concentration at pH 7.2 was explained by the Michaelis-Menten equation. Fructose-1,6-diphosphate had no observable effect on either enzyme. 4) Effect of amino acids on dental pulp and brain enzyme activity were examined, and no significant relationship was observed between the side chain structure of amino acids and their potency in inhibiting dental pulp and brain enzyme activity. Glutamic and aspartic acids markedly inhibited dental pulp and brain enzymes at pH 7.2. 5) Oxalate showed inhibitory activity against dental pulp and brain enzymes, and the Ki value was determined to be 50 microM and 80 microM, respectively. Then the effects of oral administration of non-steroidal anti-inflammatory drugs on bone growth and bone wound healing were studied. Femur length and width, and bone mineral contents in the femur as judged by microdensitometry of the soft x-ray films of the cortical portion of the femur showed time-dependent linear increase between 3 and 6 weeks of age. Wound holes were made in the parietal bones of mice with a dental bar at 4 weeks of age, and uncalcified area in the holes was determined by image analysis of soft x-ray films. The area decreased their size with the advance in age, especially a marked decrease was observed 2 weeks after the operation. From the above results, 4 week-old male mice were employed and 8 kinds of non-steroidal anti-inflammatory drugs (indomethacin, 1 mg/kg; mefenamic acid, 10 mg/kg; flufenamic acid, 10 mg/kg; diclofenac sodium, 1 mg/kg; ibuprofen, 10 mg/kg; naproxen, 10 mg/kg; phenylbutazone, 10 mg/kg and aspirin, 50 mg/kg) and dexamethasone (1 mg/kg), a steroidal anti-inflammatory drugs were administered orally every day for 9 or 11 days except Sunday, and 15 days after the initial administration, the length and width of the femur were measured. The bone mineral contents were determined by microdensitometry of the soft x-ray film of the femur. Hydroxyproline contents of the femur were also determined. Wound holes were made in the parietal bones at 4 week-old and administered the drugs from one day before the operation for 9 days and the uncalcified area in the wound hole was determined 2 weeks after the operation with an image analyzer. The results obtained were as follows: 1. A significant inhibition of the growth in length of the femur was observed by dexamethasone, diclofenac sodium and naproxen administration for 9 and 11 days.
