These words by Charles Dickens well describe our world and the environment faced by school nurses today. The current economic downturn has had an impact on each of us in many ways. State and local governments are struggling to find ways to provide services on limited budgets. School districts have been forced to reduce salaries and cut staff. Many of the families that we serve have lost their income and their health insurance or have had it significantly reduced. School nurses are seeing increasing numbers of students with acute and chronic health conditions, as well as mental health needs. Many lack a medical home, and their needs remain unmet. Our families count on school nurses to fill the void. It seems that the increased needs of students and reduced funding for school health services have met on a collision course. Yet, with every challenge comesanopportunity— an opportunity to seek new and innovative solutions. The economic climate has forced us to take an in-depth look at our practice. It has forced us to examine our worth and define our value. Although school nurses have always been aware of the value of the health services they provide, now more than ever, it is time to demonstrate evidence of that value. This is an exciting time to be a school nurse! We are embarking on uncharted waters in the era of health care. Research has shown that although the United States spends far more on health care than any other nation, we live shorter lives and have a higher infant mortality rate than many other developed countries. Despite its vast medical resources, the United States ranks just 34th in the world in life expectancy. We fare little better in infant mortality, ranking 29th in the world, tied withSlovakiaandPoland(MacDorman&Mathews, 2009). However, little is known about the mental health status of these children, especially those in the welfare system. This study analyzed national data related to the incidence and severity of depression among children of Puerto Rican, Mexican, and other Latin and South American descent. The researchers hypothesized that caregivers who had resided in the United States more than 10 years would have higher rates of stress-related disorders, which would result in higher rates of depression among their children. The researchers conducted a secondary analysis of data from National Survey on Child and Adolescent Well-Being. Data from 385 children of Latino descent, aged 7 to 14 years, who had contact with the public child welfare system were analyzed. The average age of the children was 10.5 years, and 56% were girls. Most (78%) lived in homes with their caregivers; 40% reported speaking Spanish regularly in the home; and 47% were receiving child welfare services. Caregivers had an average of four biological children, 30% were married, and 16% had earned a high school degree or equivalent. The majority had incomes less than US$20,000. The Child Depression Inventory was administered at three intervals over 18 months. The researchers found that levels of child depression decreased over time (11% were clinically depressed at the onset, but only 4% were depressed by the third measurement after 18 months). Data was collected from ballerinas, current research publications, examination of anatomical foot variables, and the dissection of used ballerina pointe shoes. The researchers conducted either personal interviews or sent email questionnaires to 29 professional, preprofessional, and teacher ballerinas in the Boston area. For those who were interviewed, the researchers observed the subjects in different ballet positions, during standard ballet movements, and while putting on pointe shoes. These were documented with photographs. Data analysis was conducted using concept mapping related to areas identified as problems in the shoes, ergonomics, aesthetics, and convenience. Based on the analysis of data of perceived needs and structural problems, the researchers explored solutions, resulting in a prototype pointe shoe boot that included at least one modification to each section of the pointe shoe: the box, the platform, the shank, and the quarter. The innovations were designed to more evenly distribute weight on the toes (reducing discomfort, improving fit and biomechanics); provide more rigid side-to-side support (reducing the risk for hallux valgus); increase the life and comfort of the shoe (which could be as short as a few classes); and reduce the sound of the shoe when it touches the floor. The resulting prototype incorporated these considerations with a shoe boot design that molds to the foot. It was then informally tested with 12 ballerinas. Praeger","doi":"10.1177/1059840508322383","DOIUrl":"https://doi.org/10.1177/1059840508322383","url":null,"abstract":"Based on the premise that family relationships influence child development and parenting stress influences a child’s behavior, the researchers examined the impact of being diagnosed during preschool years with developmental delays 4 years after diagnosis. Developmental delay was defined as either a global developmental delay or developmental language impairment. An original group of 224 preschool children were contacted to be reassessed between the ages of 7 and 8 years. A total of 65 families participated in the follow-up study. Measures of the child’s overall health and well-being were determined using the Child Health Questionnaire Parent Form 50 looking at physical and psychosocial health. A 36-item Parental Stress Index Short Form was administered to parents to identify parental distress, dysfunctional interaction, and difficult child measures. Motor, cognitive, social, adaptive, and language outcomes were assessed with the full-scale Battelle Developmental Inventory. The Vineland Adaptive Behavior Scale assessed functional capability in communication, activities of daily living, and socialization. The findings of the study indicated that there were no significant differences in parental socioeconomic status from the original cohort group. Of the children who were assessed with the developmental and functional assessment tools, 98% demonstrated evidence of ongoing impairments in at least one area, and 83% were impaired in two or more developmental domains. Those children with a preschool diagnosis of developmental language impairment showed significantly better performance in adaptive behavior and developmental performance than children with global developmental delays. In terms of health-related quality of life, parental report indicated that the children in the study were significantly lower in the psychosocial area and higher in the role/social limitations area, physical functioning, and bodily pain/ discomfort scales of the physical health scales. Parental stress score analysis included only 53 parent responses. Of these, 42% had scores indicating clinically significant elevated stress levels. Parents of children with global developmental delay were significantly more likely to have elevated stress levels than parents of children with developmental language impairment. This difference was also significant for parents of children with moderate or severe delay at the preschool assessment.
