Like every other surgical speciality, Orthopaedic trauma surgery faced significant challenges during this period in terms of numbers as well as variation in trauma patients’ presentation. Aim The purpose of our study is to analyse the numbers and variations in orthopaedic trauma and its presentations during the pandemic in comparison with 2018 and 2019. Methods The data for this audit was collected from orthopaedic register of Our lady of Lourdes Hospital, Ireland. Patients operated under orthopaedic trauma team in three months period (March, April & May) in 2018, 2019 and 2020 were selected for analysis and comparison. Results In 2020, total number of surgeries were 32% less than the average of previous 2 years. (373 in 2020 vs 543 in 2019 and 551 in 2018). Similarly, the paediatric trauma surgeries dropped by 32 % in this period. Significantly lower proportion of sport related injuries, RTAs, and assaults presented in 2020. Higher proportion of trauma resulting from simple slip, falls from height and trampoline was observed in this period compared to previous two years. Slightly higher proportion of tibial pilon, calcaneum and peri-prosthetic fractures was noted as compared to previous years. The aim of this study was to evaluate whether the government advice relating to increased vigilance surrounding hand hygiene and use of personal protective equipment (PPE) reduced SSIs following elective arthroplasty. Method Data was obtained from Public Health England website relating to SSIs following primary hip and knee arthroplasty between April 2019 and March 2020 (pre-pandemic) performed at ESHT and compared to April 2020 to March 2021 (pandemic). Results A total of 454 patients underwent a total hip replacement (THR) during the pre-pandemic period with 12 patients developing an SSI (2.6%). Comparatively, during the pandemic period, 146 patients underwent a THR with 4 reporting an SSI (2.7%). A total of 449 patients underwent a total knee replacement (TKR) during the pre-pandemic period with 11 reporting an SSI (2.5%). In contrast, 9 of the 138 patients undergoing a TKR during the pandemic group developed an SSI (6.5%). Conclusions As the data shows, there was no significant difference observed between SSIs following THR performed at ESHT prior to and during the pandemic. Surprisingly, there was a 2.6-fold increase in SSI following TKR during the pandemic period compared with pre-pandemic. This is the first time these outcomes are assessed in urological patients on a large scale. Method All bladder, kidney, and prostate cancer patients who underwent elective cancer surgery between March 2020 and July 2020 in the international COVIDSurg-Cancer collaborative database were included in the study. The primary outcome was 30-day mortality. Secondary outcomes were respiratory complications within 30-days and the factors associated with COVID-19 infection. Results A total of 1,902 patients were included in the study. A total of 21 (0.1%) mortalities and 40 (0.2%) respiratory complications (acute respiratory distress syndrome or pneumonia) were observed within 30-days of operation. Mortality was more likely in patients aged 80 or above, ASA grade 3 or 4, ECOG grade 1 or above, undergoing major surgery, and amongst patients who had concurrent COVID-19 infection (OR 31.9, 95%CI 12.4–81.42, p<0.001; univariable logistic regression). Respiratory complications were more likely in patients aged over 70, from an area with high community risk, with a revised cardiac risk index of 1 or higher or with a concurrent COVID-19 infection (OR 40.6, 95%CI 11.41–144.45, p<0.001; multivariate). A total of 42 (0.2%) patients were diagnosed with COVID-19 during their inpatient stay; designated COVID-19 sites were not associated with increased COVID-19 infections. Conclusions Major urological cancer surgeries are safe to perform during the COVID-19 pandemic on well-selected patients with appropriate risk-stratification. We aimed to characterise our experience of oncological general surgery during the first 4-months of the pandemic and compare with the same period in 2019. Method A prospective cohort study was performed from 23/03/20 to 08/07/20. All elective oncological operations were included. Data on patient demographics, waiting times, inpatient characteristics and oncological outcomes were recorded. Statistical analysis was used to compare with retrospective data from 2019. Results 78 patients were included in total, 38 in 2019 and 40 in 2020. There were no differences in length of stay (2.5 vs 3.5 days, p = 0.355) or waiting list time (27.2 vs 24.2 days, p = 0.574). Oncological outcomes were comparable with no statistical difference in clear resection margin status (94.4 vs 84.6%, p = 0.168) or positive nodal status (24.1 vs 37.1%, p = 0.298). The percentage of staging CT scans requested externally was higher in 2020 (4 vs 32%, p = <0.05). There was no difference in time from urgent referral to first assessment (30.5 vs 26.4 days, p = 0.384) or time to operation (96.6 vs 85.7 days, p = 0.618). Conclusions Oncological surgery during Covid-19 can be performed safely with favourable oncological outcomes. Attempts were made to ensure the provision of treatment, continuity of care and follow-up of the patients. As a result, implementation of telephone consultations was started for most of the outpatient appointments. The aim of this study was to evaluate patient satisfaction and views on this alteration in service. Method The first 150 patients who had breast clinic appointments delivered by telephone consultation at Ipswich hospital during the pandemic in early 2020 were retrospectively identified and sent a postal survey. The satisfaction levels were assessed through a degree of agreement with statements (four items), yes/no answers (two questions), and space for comments. Results 142 survey responses were included (94.7% response rate). The majority of patients (n = 130) were satisfied with the care they received. Even though almost half of the patients stated that they would like to be seen face-to-face in the hospital, only 10% (n = 4) of them stated that they would not like a second phone consultation. Surprisingly,78% (n = 117) of patients stated they would be happy for further breast clinic appointments to be carried out in the same way. Conclusions Our study has shown that telephone consultations are a safe and rapid method of adaptation to the COVID-19 pandemic, achieving high satisfactory rate amongst patients. Our findings suggest that this tool has benefits in post pandemic healthcare delivery. The aim of the Best Practice Tariffs (BPT) is to promote hip fracture programmes that provide best practice in the care and secondary prevention of fragility fractures. Method Retrospective audit. Time frame: 11/03/2020 – 22/04/2020. Data retrieved from trauma management and radiology systems. Results Among 33 patients with mean age of 82, 61 % showed compliance to BPT criteria. 39% did not meet the criteria: 33% had delayed surgery >36 hours, 3% had delayed orthogeriatric assessment, and 3% not fit for surgery. 33 % delayed surgeries due to requirement for medical optimisation, and further investigation. Among COVID patients: 41.6 % mortality rate, of which 40 % were perioperative. There was a loss of BPT income (around £17k) during these unprecedented times. However, the provisions of NOF care did not fluctuate despite extreme pressures on our services. Conclusions Ensuring all members of the MDT have access to clinical management system. Implementation of early supported discharge programmes allowed reduction in length of acute hospital stay (from 14.2 in Jan 2020 to 8 in March 2020). Referrals for spinal surgery are increasing with varying effectiveness. Screening for neurosurgical red flags is critical to quickly identify the rare but serious causes of LBP, such as CES. The primary aim was to explore what effect COVID-19 had on the management pathway of these patients in primary care to investigate causes of LBP including ruling out CES. Method A service evaluation of all patients presenting to a large primary care provider in West Yorkshire with lower back pain who underwent MRI lumbar/sacral spine investigation between March 2020 and March 2021 was conducted. Results A total of 105 patients with matched MRI scans were included. Neurosurgical red flag screening was performed by virtual appointment only in 32 (30.5%) of patients. In 10 cases (9.5%), red flag screening was not documented. Radiological outcomes revealed three (2.9%) cases of CES. Thirty (28.6%) required onward referral to neurosurgery. The majority (n=56; 53%) had demonstrated radiological pathology amenable to conservative management. Conclusions During the pandemic, almost a third of patients in primary care did not receive a face-to-face examination to rule out red flags in lieu of virtual appointments and history alone. The majority of imaging revealed pathology that was amenable to conservative management. The safety of virtual consultations including telephone appointments to screen for neurosurgical pathology needs further investigation. Offering a day case procedure for simple mastectomy has the potential to improve outcomes in terms of patient safety, feasibility, and reduction in associated costs. We aimed to study the proportion of day case mastectomies over a 2-year period and assessed relative readmission and complication rates alongside patient reported outcome measures (PROMs). Following this, we wanted to analyse whether a day case procedure should be routinely offered. Method A retrospective audit was conducted on all patients undergoing a simple mastectomy. Data were collected from patient and pathology records. PROMs were obtained via telephone conversation using the validated BREAST-Q survey. Patient demographics, reason for mastectomy and readmission for complications were all assessed. Results Out of the 202 patients analysed, over 75% had an overnight stay. The most common complications for both groups of patients were readmission due to infection and haematoma evacuation, with similar rates across the two (p = 0.26, p = 0.69). Data from PROMs also suggested that patients had similar outcome and satisfaction levels (p = 0.27). Conclusions The result from this study shows that it is safe as well as acceptable to patients to offer simple mastectomies as a day case procedure. However, it was thought that the number of post-tonsillectomy complications had increased. We performed an audit to evaluate the rate of post-tonsillectomy complications in our trust and investigate possible causes. Method We measured the rates of patients who developed complications post-operatively during the period from June to November 2020 and compared it to the rates in the pre-COVID year and the national rate. Moreover, we scrutinized retrospectively the operative notes of each patient presenting with a post-tonsillectomy complication and identified risk factors. Results In the study period, we performed 129 tonsillectomies. Of these patients, 14 presented with complications; 11of which had bleeding, while 3 had post-operative pain. Two patients needed to return to the theatre to control the bleeding. During the same period in 2019, 28 patients had complications out of a total of 199 patients. The rate of complications in 2020 was 10% which compared favourably with the previous year (14%). The highest number of patients (9/14) was in a hospital which posed a new environment to our surgeons. Dissection by Bipolar diathermy was the most contribute factor for bleeding in most patients (11/14). Conclusions The disruption caused by the pandemic situation did not influence overall rates of complications.
