Cook","doi":"10.1097/00006254-199409000-00004","DOIUrl":"https://doi.org/10.1097/00006254-199409000-00004","url":null,"abstract":"The loss of a pregnancy in the first trimester is a common event and recent research has identified high levels of psychological distress amongst women who have miscarried. We believe this study is the first to examine the phenomenon from a longitudinal perspective using standardized measures. A sample of 65 women was rated for anxiety and depression using the Hospital Anxiety and Depression Scale at intervals of one, six and 12 weeks following early miscarriage. The results indicate that a large proportion of the sample had clinically important levels of anxiety (41 per cent) and depression (22 per cent) in the first week following miscarriage, which declined to 32 per cent and 6 per cent respectively by the 12th week. No association was observed between levels of psychological morbidity and a number of social and obstetric variables, the only exception being whether or not the pregnancy had been planned. These results are discussed with reference to the relevant literature. Several approaches were undertaken on the responses from subjects in three neurotically disturbed groups and a normal control group. Analyses indicated that simple cut-off points could discriminate effectively an 'adjusted-neurotic' dimension; simple cut-off points were relatively more difficult to develop for discrimination among the three neurotically disturbed groups. Discriminant analysis of the CCEI subscales indicated that approximately 74 per cent of all subjects could be classified correctly. The OBS and HYS subscales were not found to be effective discriminators among the four groups. {"title":"Psychotherapeutic approaches to the treatment of panic attacks, hypochondriasis and agoraphobia.","authors":"D B Diamond","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Various theoretical orientations suggest different treatment approaches to the agoraphobic syndrome. In this paper a self-psychology formulation is applied to the treatment of panic attacks, hypochondriasis and agoraphobia. When these symptoms are viewed as manifestations of self-fragmentation, their treatment can be conceptualized as a process of improving self-cohesiveness. The initial management of symptoms sets in motion a self-object transference which is the foundation of treatment. As therapy proceeds, the defences against affect which are associated with a primary defect in anxiety regulation can be understood in the light of childhood reconstruction and worked through in the transference. The resultant restructuring of defences promotes an increased capacity to tolerate affects without the threat of self-fragmentation. {"title":"The influence of alcohol on judgement of facial expression of emotion.","authors":"J A Borrill, B K Rosen, A B Summerfield","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Despite considerable research into the effects of alcohol on behaviour, there appears to be little information available about the influence of alcohol on interpersonal judgement. An experiment was carried out to test the hypothesis that alcohol would affect subjects' ability to judge facial expressions of emotion. Thirty male and 30 female normal social drinkers were allocated at random to high alcohol, low alcohol or placebo conditions and were shown photographs of faces displaying basic emotions. As predicted, subjects in the high alcohol condition made more errors and subjects in the low alcohol condition fewer errors than subjects in the placebo condition. Accuracy of judgement also varied significantly according to the sex of the subject and the category of emotion being judged. In particular, alcohol was associated with greater impairment of judgements of anger than of other emotions. Some authors have suggested that psychological factors may be a primary cause of infertility, others have suggested that the state of infertility itself can provoke psychological symptoms. The importance of psychological counselling for involuntarily childless couples has also been noted. This paper, which reviews studies that have attempted to investigate psychological aspects of infertility, is organized around three issues: psychological factors as a cause of infertility, the impact of infertility on psychological functioning, coping with infertility and the counselling needs of the infertile couple.
