{"title":"Inhibitors of platelet function in the prevention of arterial thrombosis.","authors":"L A Harker","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The use of platelet inhibiting drugs in the prevention of arterial thromboembolic occlusive disease demonstrates that only a few trials have been adequately designed to provide conclusive results. Dipyridamole is effective in preventing the thromboembolic complications of prosthetic heart valves and sulfinpyrazone reduces occlusive events in A-V silastic cannulae, while clofibrate is clearly ineffective in secondary prevention of myocardial infarction. No antiplatelet agent has yet been convincingly shown to exert any antithrombotic effect in venous thromboembolism, although the results with hydroxychloroquine deserve further study. Currently, only two antiplatelet drugs, dipyridamole and sulfinpyrazone are clearly proven clinically effective in reducing arterial thromboembolism. {"title":"The ultrastructure of the platelets in refractory anemia (\"preleukemia\") and myelomonocytic leukemia.","authors":"J E Maldonado","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We have conducted extensive morphologic studies of the platelets in 16 patients with preleukemia or myelomonocytic leukemia. Although the degree and frequency of the changes varied in the different cases, it was evident that the platelets in these two pathologic states often were structurally abnormal. The abnormalities include changes in size (mainly giant forms), shape (frequent presence of round cells), and quantitative (particularly decreases) as well as qualitative changes in the platelet granules. Quite remarkable has been the finding of giant granules of irregular contour and heterogeneous composition, perhaps the result of fusion of several single granules. {"title":"Chronic myelocytic leukemia: cytogenetic findings and their relations to pathogenesis and clinic.","authors":"D K Hossfeld","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The chromosome constitution of myeloid cells of 106 patients with chronic myelocytic leukemia (CML) was studied. Two thirds of the patients were in the chronic and one third in the blastic phase. Particular attention was paid to patients with a mixture of Ph1-positive and Ph1-negative metaphases, and those with additional chromosome anomalies. A statistical analysis of the additional anomalies observed during the chronic and the blastic phase waandom involvement of certain chromosome groups. Chromosomal findings in medullary and extramedullary tissues were compared; the results suggest an accumulation of cells with additional anomalies in extramedullary sites. Ph1-negative patients were shown to be older and to have a survival shorter than Ph1-positive patients. The medium survival time of patients in the blastic phase may be longer in those without additional chromosome anomalies.
