{"title":"[Karapandzic flap for reconstruction of extensive defects of the lower lip].","authors":"F Espitalier,&nbsp;B Dréno,&nbsp;O Malard","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objective: </strong>To discuss advantages and disadvantages of Karapandzic flap for reconstruction of extensive defects of the lower lip.</p><p><strong>Methods: </strong>Two clinical observations and a review of the literature are presented.</p><p><strong>Results: </strong>The Karapandzic flap requires only one operating time. The tissue used to reconstruct the lower lip is very close to the resected lip. The functional result is satisfactory, the lips being continent, allowing speech and feeding. The main advantage of this technique is the preservation of the labial sphincter, which is vascularized, mobile and sensitive. The aesthetic result is acceptable. This flap keeps the vermilion, but with rounded commissures that corrects spontaneously. The main disadvantage of this flap is the microstomia, requiring only rarely further surgery.</p><p><strong>Conclusion: </strong>The Karapandzic flap is a good option for reconstruction of extensive defects of the lower lip. It recreates the labial sphincter, while maintaining its vascularization and innervation. This is a retrospective study concerning 15 patients treated from 2010 to december 2012 for basal cell carcinomas of the nasal tip. The minimum follow up was 6 months. We describe the procedure and the indications. The best site to be proposed for this flap is beyond question the latero-medial part of the nasal tip. It can be adapted to skin defects as large as 15 mm. Obviously since it is a skin island flap it is important to respect the pedicle between it and the nasalis muscle. The closure of the donor site is done in the alar-facial sulcus with a VY suture and scarring on this level is very negligible. We analyzed the aesthetic result which is often very good. The surgery for nasal polyposis, adjuvant medical treatment, may allow olfactory improvement. A 18-year-old man admitted to emergency was easely intubated for 14 days for a non neck injury. Sixty-six days after intubation, he consulted for severe odynophagia with dyspnoea. Nasofibroscopic examination revealed a right arytenoid granuloma with oedema and inflammation, and bilateral arytenoid immobility. There was evidence on the CT scan of cricoarytenoid arthritis with fluid accumulation. Antibiotic treatment and two punctures for drainage allowed healing. There were cricoid and arytenoid calcifications. The patient recovered total and stable laryngeal mobility and function. This case is unusual, with features unlike those previously reported: intubation being the probable cause, the unusual clinical presentation with painless cricoid palpation, and the contrast between imaging findings during follow-up and the evolution under treatment. It is called LOV for \"Lifting of the Oval of Visage\". LOV is realised under local anaesthesia, lasts about 40 minutes. In this study, post-operative results are evaluated after one year.</p><p><strong>Material and methods: </strong>retrospective study with anonymous questionnaire measuring patient's satisfaction, operated between january 2011 and december 2011, by the same surgeon in the department.</p><p><strong>Results: </strong>43/45 questionnaires were analyzed, 40 females (93%) and 3 males (7%). Average age was 56 years, with a range of 51 years to 66 years. Our satisfaction ratings values after surgery were: very good: 32 (74.4%), good: 8 (18.6%), intermediate: 2 (4.6%), unsatisfied: 1 (2.3%), will not undergo LOV again: 1 (2.3%), would not recommend LOV to a friend: 2 (4.6%), recommend LOV to a friend: 37 (86%). The only complication (2.3%) was an unilateral hypoesthesia earlobe.</p><p><strong>Conclusion: </strong>LOV is simple and effective. It is a surgical alternative in the treatment of cervical area ptosis. Performed under local anesthesia, this short procedure responds to complaints of patients: safe rejuvenation and natural looking. Since 2008, LOV is performed in our department. Recruitment is most often \"word of mouth\", as confirmed by 86% of patients who would \"recommend it to a friend\". There are less than sixty cases described in the literature. Through both cases, we describe and discuss the clinical management of melanoma of the larynx.</p><p><strong>Clinical cases: </strong>Two men aged 80 and 61 years were supported for MPL. The tumor was revealed by persistent dysphonia for the first. For the second, it was a second melanoma of laryngeal location. In both cases it was performed a total laryngectomy extended to the first tracheal rings with bilateral lymphadenectomy associated with adjuvant radiotherapy.</p><p><strong>Discussion: </strong>Excision margins with oncological safety are necessary to except a curative surgery. Still controversial, the adjuvant radiotherapy seems to provide a benefit in terms of local control. The place of targeted therapies remains unclear. However, no prognostic factor is currently clearly identified for this disease.</p><p><strong>Conclusion: </strong>Rare tumor, the MPL is associated with a very poor prognosis. {"title":"The domes crossover: a new method of nasal tip modification.","authors":"Y Saban,&nbsp;F Perrone","doi":"","DOIUrl":"","url":null,"abstract":"<p><strong>Objectives: </strong>To discuss overprojected nasal tips, to overview treatment options, and to introduce a new method of nasal tip modification.</p><p><strong>Patients and methods: </strong>A retrospective study was conduced on 29 patients who underwent rhinoplasty where the domes crossover technique was employed. Patients were stratified into 3 groups: 18 patients presented with homogeneous overdevelopment of the LLC (nasal tip over projection type 1), 5 patients with predominant over development of the lobular portion of the LLC (nasal tip overprojection type 2), 6 patients with a true Pinocchio-type nose and a predominant overdevelopment of the columellar portion of the LLC (nasal tip overprojection type 3). Before and after photographs were compared to evaluate nasal tip characteristics. Analysis of results was performed by the use of 3 measurements: Goode's ratio, nasofacial angle and lobule-to-columella ratio.</p><p><strong>Results: </strong>Final nasal tip projection, rotation, volume and shape were satisfactory in all cases. No complication was observed during the follow-up period.</p><p><strong>Conclusion: </strong>This new method of nasal tip modification presents the advantage of preserving natural domes, being quick and allowing the achievement of desirable nasal tip projection, rotation, volume and shape. Studies have also shown that in these patients, the inability to produce an appropriate and spontaneous smile would be a key factor of depression. When facial palsy is considered complete and the nerve cannot be repaired, the patient can benefit from palliative surgery to regain a better quality of life in the aesthetic, functional, and psychological aspects. The hypoglossal-facial anastomosis (AHF), temporal myoplasty (MAT) and gracilis transposition (TG) are the major surgeries currently used for this purpose. The aim of our study is to assess quantitatively and qualitatively the effects of each of these surgeries on the lip mobility and production of smile. From this perspective, we proposed a protocol of an evaluation of facial motricity, of quality of life, and more particularly on the quality and the analysis of the smile. The results underline that there is no significant difference in the recovery of the facial motricity according to the surgery. Only the slower, deferred deadline of recovery at the patients AHF and TG who have to wait several months, it is for the same levels as that of the patients' MAT.
