Stimulatory therapy of this areas, especially by direct stimulation of the H improves the hypothalamic functions, the relationship between H and the RF and all the clinical status of parodontosis patients. In rabbits with experimental parodontosis have been found functional and histological alterations in cerebral cortex, and especially in hypothalamus, together with lesions in the hypothalamo-posthypophyso-neurosecretoric system, in the anterior pituitary (P) cells (for ACTH, TSH and FSH) as well as in zona fasciculares of the adrenal cortex (AC). This data, together with findings of other authors, prove that parodontosis is a diencephalopathy involving a whole system: CC-H-P-AC.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie. {"title":"[Anxiety, an emotional condition induced by stomatological procedures].","authors":"R Adamesteanu","doi":"","DOIUrl":"","url":null,"abstract":"

The great number of stomatological procedures applied to the general population will induce, by their nature, anxiety. This will precede and will accompany the stomatological experience, will induce a defense attitude and will render more difficult the therapeutical relationship between the stomatologist and the patient. Anxiety is also induced by the environment to healthy subjects as well, who have not had a stomatological experience and will induce similar reactions in cases where stomatological procedures are necessary. The study on the anxiety induced by stomatological procedures was carried out in 154 subjects, both sexes being represented, of median age, of which 37 were without stomatological lesions. It is proposed that before the start of a stomatological treatment efforts should be made in view to attenuate anxiety with the aid of simple psychological and psychiatric procedures.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie. The clinical picture at the onset is not specific. Later on in the course of the disease the immunological capacity of defense of the organism is affected and a series of symptoms including fever, infections with bacteria, lymph-node inflammation, encephalopathies, Kaposi sarcoma will develop. Apparently buccal infections with fungi are the earliest manifestations, and they are followed, by order of frequency, by herpetic lesions, \"hairy\" leucoplasia, tumours. Transmission through the saliva from the patient to the stomatologist of the HIV infection is theoretically possible due to scratches on the physicians' hands. In two cases the severity and the extension of the lesions resulted in a fatal outcome. In one of the cases the patient also had pericarditis, pleurisy and purulent meningitis, and in the second case the necrotizing mediastinitis extended to the pleural and pericardic cavities, and was associated to bilateral bronchopneumonia. In three cases the patients were cured following cervical mediastinotomy, efficient mediastinal aspiration of the anterior and of the posterior mediastinum, in association with antiinfectious and resuscitation therapy. Since the prognosis of these major complications is still severe the risk of diffusion of odontogenic suppurations should not be ignored or minimized since in all the cases iatrogenic factors have occurred over the evolution of the patients. Knowledge of stomatologists should be improved and they should be aware of the risks of odontogenic suppuration involving parapharyngeal and cranio-encephalic extension, or the risk of cervico-mediastinal diffusion.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie. {"title":"[Aspects of prevention of bucco-dento-maxillary affections in children from the urban environment].","authors":"P Firu, D Bogdanovici, A Ceachiris","doi":"","DOIUrl":"","url":null,"abstract":"

In the first part of the paper a detailed presentation is made of the methods for the prevention of dental decay, of gingivitis and of dento-maxillary anomalies. Data from the published literature, as well as the authors experience are the base for this part. In the second part a study is presented, carried out in 1,000 children from Bucharest, of which 250 were controls. The children were classified according to three categories of age and sex. The age groups were: 3-5 years, 7-9 years, and 12-14 years. In the experimental group the buccal cavity was maintained in a hygienic condition by brushing with fluorinated paste, dental decay was treated and topic applications were carried out with aminofluorine gels. Health education was also applied, with various methods. The children were followed clinically for a period of three years, and the results of the examination were recorded on standard observation files. The results showed a significant decrease in the frequency of dental decay in the experimental group as compared with the controls. Gingivitis decreased by 18-20% in the experimental group, but a less significant decrease was noted in the frequency of dento-maxillary anomalies. It is concluded that the application of a minimal program of hygienic measures with prevention-prophylactic aims may result in the significant reduction of bucco-dento-maxillary affections. However, it should be stressed that these programs should be carried out continuously and very consciously both by the physician and the child.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie. {"title":"[Periodontal diseases with Entamoeba gingivalis].","authors":"M Sefer, A I Boanchis, S H Chaouki, V Ganescu, P Constantin","doi":"","DOIUrl":"","url":null,"abstract":"

At the beginning of our century Entamoeba gingivalis was considered to be a pathogenic bacteria, capable to induce parodontal lesions. Later on it was also found in healthy persons, and the germ was less interesting from the medical view-point. In the present study the authors report their findings concerning E. gingivalis in 135 patients with various stomatological affections including: dental caries, parodontopathies, pulpitis, gangrene, ulcero-necrotic stomatitis etc. The study was started following the discovery of the amoeba in the gingival exsudate of a male aged 19 years with chronic superficial marginal parodontopathy, who, after a treatment with metronidazol, was cured. Entamoeba gingivalis belongs to the Rhizopoda class, together with E. dysenteriae, and E. coli, but, in contrast with these strains it does not have resistance forms (cysts). Oral amoeba were evidenced in 18 out of 78 patients with parodontal lesions (23.07%), in the gingival exsudate, the purulent secretion from parodontal pouches, in the dental tartar, the alveolar fluid following extraction etc. In 117 students from the Faculty of Stomatology, and in 57 patients with various other stomatological affections these germs were not found in any of the abovementioned products. Microscopic examination of fresh preparations, and of Giemsa-stained smears was the main method for the detection of the amoeba. The etiopathogenic role of E. gingivalis is re-examined in discussions regarding certain parodontopathies.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie. {"title":"[Models of automated mandibular kinematics in the Kennedy Class I partial edentate].","authors":"L Ene, T Trăistaru","doi":"","DOIUrl":"","url":null,"abstract":"

The biunivocal relationships which exist between the elements and the functions of the dento-maxillary apparatus are also found in the determination and the effects of automated mandibulary movements in patients with class I edentation. Kinesiographic mandibulary analysis correlated with clinical observations certain characteristics can be identified of which the most evident, resulting from the edentation, and at the same time the most important from the viewpoint of dysfunctional implications is the almost permanent presence of horizontal, mainly sagittal movements of the teeth and of the temporo-mandibulary articulation. Propulsion of the mandibula in this way is at the origin of many irritative foci: in the frontal teeth that have to transmit and to take up the forces which have unfavourable directions; the muscles, especially the external pterygoidal muscles that are overstressed; the temporo-mandibulary articulation where the unfunctional relationship between the mandibular condyl, the articular disk, and the articular eminence will acquire an almost permanent character. The consolidation of the propulsed (mesial) position of the mandibula by prosthetic treatment in this position cannot have but negative effects for the elements of the dento-maxillary apparatus.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie. An analysis is made of hypertrophic and hyperplastic changes from the histopathologic and the etiologic viewpoints, of the gingivo-parodontal tissues. A review is also presented of several complete classifications. Clinical and histopathologic aspects are further discussed, of the various forms of gingivo-parodontal hyperplasias. Results obtained in the treatment of 160 forms of hyperplasia found in 1500 patients that were investigated are also discussed. The study concludes on the importance of an early diagnosis of various forms of gingivo-parodontal hyperplasias. This is of particular importance for the treatment of those patients where the therapy is directed against the causative agent, but that should also include the application of antiinflammatory therapies.
","PeriodicalId":76435,"journal":{"name":"Revista de chirurgie, oncologie, radiologie, o. r. l., oftalmologie, stomatologie.
