Stimulation on the scalp excites corticospinal neurones in the motor cortex while stimulation over the spine excites spinal nerve roots. The difference between EMG response latencies after both stimulations represents conduction in the central motor pathways and is called central motor latency (CML). The aim of out experiments was to investigate certain methodological aspects of the technique in order to standardize the procedure. Recordings were done from slightly contracted abductor digiti minimi and tibialis anterior muscles after electrical stimulation on the scalp and from the relaxed and contracted abductor digiti minimi and biceps brachii muscles after magnetic brain stimulation. Stimulation over the spine (C7/T1 interspace in case of upper limb muscles stimulation and T12/L1 interspace in case of tibialis anterior stimulation) was always electrical. Using a rather weak non-commercial magnetic stimulator we were not able to activate lower limb muscles, neither we succeeded to evoke responses from the relaxed arm muscles in all subjects. Electrical scalp stimulation proved successful in all cases. Muscle response after cortical stimulation in contracting muscles and shorter latencies and provided more accurate estimate of conduction time in the central motor pathways than responses in the relaxed muscles. Latencies should be measured from several superimposed responses and not from averaged ones. The intensity of stimulation over the neck did not affect CML. We nevertheless suggest that the strongest stimulus intensities should not be used in order to avoid CML overestimation.</p>","PeriodicalId":76210,"journal":{"name":"Neurologija","volume":"38 4","pages":"271-83"},"PeriodicalIF":0.0,"publicationDate":"1989-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13842945","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

According to the analysis, if compared to the surgically treated group of patients, in those medically treated was registered a much higher rate of mortality in the period of hospital treatment and in the first six months following thereafter. The estimation of persistent neurological findings, performed 6 months after discharge from hospital, showed an improvement in 41% of non-surgically treated and 72% of surgically treated patients. The choice of muscle biopsy is important. Deltoid, biceps brachii and quadriceps femoris muscles are mostly used and specimens are usually obtained by an open biopsy. The interpretation of the muscle biopsy must be considered in relation to the clinical problem. Enzyme histochemical methods allow study of sub-cellular structures and are therefore particularly useful in diagnosis. They are standardized and a routine series should be used in each specimen. In routine diagnostic work electron microscopy is of relatively little value. It is first important to inspect the size, shape and fibre type distribution to get an impression of whether the biopsy is normal or abnormal. Then more characteristic features of either neuropathies or myopathies are looked for. Regarding prognosis it is difficult to draw conclusions from the muscle biopsy. Biochemical assays may be useful in establishing precise diagnosis, especially in glycogenoses and mitochondrial disorders.</p>","PeriodicalId":76210,"journal":{"name":"Neurologija","volume":"38 4","pages":"311-9"},"PeriodicalIF":0.0,"publicationDate":"1989-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13631709","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

For the present method, the optimal concentrations of IgG in CSF samples are about 0.025-0.030 g/l, corresponding to the applied amount of 125-150 mg. In our testing of this method, oligoclonal IgG bands in CSF specimens were clearly demonstrated in 52 (96%) of 54 patients with clinically established definite diagnosis of multiple sclerosis (MS), in 4 (40%) of 10 patients with infectious diseases of the CNS, and in 9 patients (25%) of 38 with other neurological diseases. Abnormal patterns were also demonstrated in the serum of patients with MS (43%). Intrathecally synthesized IgG was mathematically calculated in 43 (80%) out of 54 patients with MS. This method appears to be a useful alternative for the demonstration of oligoclonal IgG bands in the unconcentrated CSF, especially when questionable or negative results arise by routine electrophoretic technique for oligoclonal bands detection.</p>","PeriodicalId":76210,"journal":{"name":"Neurologija","volume":"38 1","pages":"11-21"},"PeriodicalIF":0.0,"publicationDate":"1989-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"13817969","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
