Cozza","doi":"10.1016/j.mor.2010.09.004","DOIUrl":"10.1016/j.mor.2010.09.004","url":null,"abstract":"<div><h3>Objectives</h3><p>The aim of the present study was to use published data to evaluate quantitavely the skeletal and dental effects of extraoral and intraoral molar-distalizing appliances in individuals with Class II malocclusion.</p></div><div><h3>Materials and methods</h3><p>We conducted a literature search that identified 37 prospective or retrospective clinical studies that met our predefined inclusion and exclusion criteria. The data reported in these publications (SNA°, SNB°, FMA°, LFH, SN-PP°) were analyzed to identify the skeletal changes (sagittal and vertical) produced by the molar distalization appliances.</p></div><div><h3>Results</h3><p>Use of extraoral distalizing devices (Kloehn headgear) produced changes in the sagittal plane that consisted in reduced maxillary protrusion (SNA° -1.88°). Instead this value tends to increase (albeit nonsignificantly) in subjects treated with intraoral devices (+0.14°).</p><p>Neither the extraoral nor intraoral distalizing appliances had any significant effects on the SNB° angle, which ranged from -0.55° to -0.35°, or on the inclination of the mandibular (FMA°) or palatal (SN-PP°) plane (which were +0.36° and +0.45°, respectively). Anteroinferior facial height (ANS-Me) tended to increase (mean +1.05<!--> <!-->mm).</p><p>In subjects treated with Kloehn headgear, significant distalization of the molars (3<!--> <!-->mm) was associated with secondary distalization of the maxillary premolars and upper incisors, whereas in subjects treated with intraoral appliances, significant loss of anchorage was observed at the levels of the first and second premolars (mean 1.5<!--> <!-->mm and 2.17<!--> <!-->mm, respectively) and the central incisors (mean 1.43<!--> <!-->mm).</p></div><div><h3>Conclusions</h3><p>Both the extra- and intraoral appliances were efficient in achieving a Class I molar relationship. Use of the Kloehn headgear allowed the maxillary molars to be distalized without altering the flaring of maxillary incisors. Noncompliant intramaxillary appliances (Pendulum, First Class, Distal-Jet And Jones-Jig) produced distalization of the molars that was inevitably associated with loss of anchorage, reflected by mesial movement of the incisor and premolars. The facial disharmony consisted in upper incisor protrusion and a profile characterized by a marked convexity, protrusion of the upper lip, and retrusion of the cutaneous chin. The general objectives were: (1) eliminate the arrest in mandibular growth; (2) exert an orthopedic effect on upper maxilla; (3) correct molar and canine relationships; and (4) improve the facial profile.</p></div><div><h3>Materials and methods</h3><p>Given the child's age, an excellent mandibular response could still be expected. The parents were informed that the boy's cooperation was essential for the success of the treatment. Treatment consisted in: (1) application to the upper arch of a standard edgewise multibracket appliance (0.021 x 0.028) based on the Tweed-Merrifield Directional Force System; (2) orthopedic treatment of the upper maxilla involving application of high extraoral traction (HpHg) (arch 0.018 x 0.025); (3) evaluation of results achieved with repeat radiography; (4) application of an appliance to the lower arch; (5) continuation of therapy to achieve correct dental relationships (arch 0.019 x 0,025). Use of high extraoral traction (HpHg), Class II elastic bands, and intermaxillary elastics; (6) removal of the appliance and application of an upper Hawley appliance and lower retainer 3/3.</p></div><div><h3>Results and conclusions</h3><p>The final results were very good in terms of esthetics (occlusal and skeletal). Baccetti","doi":"10.1016/j.mor.2010.11.004","DOIUrl":"https://doi.org/10.1016/j.mor.2010.11.004","url":null,"abstract":"<div><h3>Objective</h3><p>The aim of the present study was to evaluate the dentoskeletal effects produced by an intraoral appliance for the unilateral or bilateral distalization of the upper first molars.</p></div><div><h3>Materials and methods</h3><p>We analyzed the pre- and post-distalization cephalometric films of 17 patients (10 males and 7 females) with class II malocclusion treated with the First Class appliance. The mean age at the start of treatment was 13 years and 4 months. Bilateral class II molar relationships were corrected within 2.4 months on average.</p></div><div><h3>Results</h3><p>Seventy percent of the space created anterior to the first molars was the result of maxillary molar distalization; the remaining 30% was due to reciprocal loss of anchorage by the maxillary second premolars. The maxillary first molars showed significant distalization of 4.0<!--> <!-->mm, associated with a significant distal inclination of 4.6 degrees and a significant extrusion of 1.2<!--> <!-->mm. As for loss of anchorage, the second premolars exhibited significant mesial movement of 1.7<!--> <!-->mm and significant mesial inclination of 2.2 degrees. In the anterior region, significant mesial movement of the maxillary incisors of 1.3<!--> <!-->mm was associated with significant incisor proclination (2.6 degrees) and a significant increase in the overjet (1.2<!--> <!-->mm). The aim of this work is to evaluate both neuromuscular function and mandibular movements in 70 patients undergoing orthodontic surgical treatment.</p></div><div><h3>Material and methods</h3><p>During the orthodontic surgical treatment all patients were submitted to usual clinical and radiographic exams and to periodical electromyographic and electrokinesiographic evaluations.</p><p>The test group consisted of 70 patients (27 men and 43 women) at the end of growth.</p><p>Electromiography and kinesiography were performed at the time of diagnosis, at the beginning of orthodontic therapy, periodically during orthodontic treatment before surgery, the day before surgery, during the intermaxillary block, at the removal of the fixation, during orthodontic treatment after surgery, at the removal of orthodontic appliance and the during follow up.</p><p>The instruments used in this research were a Freely electromiograph and a K6-I EMG electromiograph and electrognatograph.</p><p>The anterior temporal and the masseter muscles were tested.</p><p>The control group consisted of adult patients with skeletal class I, with no temporomandibular disorders and no previous orthodontic or surgical-orthodontic treatments. Mandibular movements were assessed by a Myotronics K6-1 kinesiograph. The results were statistically analyzed by t test and ANOVA test.</p></div><div><h3>Results</h3><p>At the beginning of treatment the patients of the test group showed a malocclusion-compensating balance. During orthodontic treatment before surgery electromyographic and electrognathographic values started to worsen and continued to worsen until surgery. They improved during orthodontic treatment after surgery.</p><p>After removing orthodontic appliances, electromyographic values improved and reached optimal values. Recover of mandibular movements was also satisfying even if it took a longer time than muscular rehabilitation. The CAD/CAM device employed was a structured light-based scanner. CAD generates a 3D digital image that can be used to design the splint. During the CAM phase, the D-Bite (Digital Bite) was created from a block of polymethyl methacrylate (PMMA) with a thickness of about 20<!--> <!-->mm.</p></div><div><h3>Results</h3><p>The production of a digitally designed D-Bite (Digital Bite).
