{"title":"[The hardness and osseous structure of preserved ear ossicles].","authors":"K B Hüttenbrink","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The mechanical rigidity (breaking strength) of normal human ossicles, preserved with cialit and formalin, was studied in a tension compression testing machine. The bony structure of the ossicles before and after crushing was examined via high-grade magnifying radiography. With this new technique, minute bony details of the ossicles, which up to now could be demonstrated only histologically--consequently destroying the object--can be visualised without inflicting any changes to the ossicle. The different preservation methods had no effect on the structure or the mechanical strength of the ossicles. The fracture lines were distributed at random, with no preferential direction. In further experiments, defects in the ossicles' surfaces, which were cut with diamond burrs, were investigated radiographically and with a scanning electron microscope. The compressed bone dust seals up the opened marrow spaces. This reduces bone resorption significantly, as could be demonstrated with removed ossicular transplants similarly prepared, with had been implanted several years ago. The high grade radiographic magnification revealed that several ossicles which were removed during typical middle ear operations, were covered with metallic powder, originating from the intra-operative contact of the burr with the metallic suction tube. In most test situations, rotating chair motion and head fixation are carried out manually. As a result, specific and non-specific sources of error are involved in these procedures that hamper the reproducibility of test conditions. A system is described that provides quantifiable and reproducible, electronically controlled movements of the rotating chair. Ramp and rectangular stimulus profiles are achieved by infrared optical gate devices on the chair axis. Various processor-controlled sequences of angular displacement and duration can be programmed and monitoring of chair movement is available to the operator during testing, which is otherwise performed in complete darkness. Individual casts of the upper jaw connected with a telescope wall clamp facilitate optimal head fixation. Since great caution is required when using an electronically controlled chair for such tests, and the possibility of damage to the vertebral column is increased by head fixation, several devices have been incorporated into the equipment to fulfil safety requirements such as release mechanism in the head fixation system, besides other safety devices. This system enables cervically induced eye movements which can be related to amplitude, acceleration and velocity of trunk rotation. It causes hypersecretion, decreased nasal patency, sneezing and sometimes headache by local reflexes, which are beyond voluntary control. The synonymous name \"vasomotor rhinitis\" or \"vasomotor rhinopathy\" is no longer adequate with regard to our present state of knowledge of the autonomous innervation of the human nasal mucosa. Recent pharmacological investigations show the great importance of peptidergic neurons. In our own studies, the presence and the topical effects of the neuropeptide substance-P in human nasal mucosa are examined. A new concept of the autonomous innervation of the human nasal mucosa is presented. Apart from adrenergic and cholinergic neurons, it also includes peptidergic neurons (SP, CGRP, NKA, VIP, PHI, APP, GRP). According to this model, a hypothesis on the pathogenesis of HR is developed. A key position is occupied by the so-called \"axon reflex\" which is mediated by substance-P immunoreactive nerve fibers. It is released by a chemical, thermal or mechanical irritation. This axon reflex mediates pain, vasodilation and plasma extravasation (neurogenic oedema), hypersecretion such as smooth muscle contraction, and sneezing reflex. Capsaicin (8-methyl-N-vanillyl-6-nonenamid) leads to a selective degeneration of substance-P immunoreactive nerve fibres and desensitisation of its receptors after repeated topical or systemic application, thus blocking the axon reflex. The risk-free application of capsaicin was shown in self-experiments and in volunteers. An experimental morphologic study].","authors":"C Zöllner, J Strutz, G Bruggmoser, H Knüfermann, H E Schaefer","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Following the first positive therapeutic results of T1a vocal chord cancer with 125iodine seeds in man (brachytherapy), we were concerned with the effects of this low-energy photon source on healthy laryngeal tissue. In the larynx, changes in the area of the vocal chord and the cartilaginous laryngeal structures were investigated in particular. These experimental studies were performed on the larynx of the dog. Through direct laryngoscopy, two 125iodine seeds were implanted in the anterior third of each vocal chord in 8 dogs; the resulting prick canals were sealed with fibrin glue. During survival the correct position of the seeds was radiologically monitored. After survival periods of 1, 3, 6, and 12 months the larynges were fixed by perfusion, removed, imbedded in paraplast, and stained with H. E., Azan, and E. v. G. After this protracted irradiation with 125iodine seeds only slight and for the most part reversible pathological changes had occurred. One month after implantation, only a perivascular infection was found in the vocal chord. After three months an inhibition of the fibrin organization around the seeds was observed as well as a localized dyschylia with broadening of the gland ducts.
