Studies on different serum lipids in children with Type I diabetes have shown different results. Hyperglycaemia is the primary abnormality of diabetes. It is logical to consider elevated blood glucose as a possible risk factor in the development of macrovascular disease. Although associations between hyperglycaemia and diabetic complications have been repeatedly shown for microvascular disease, such an association is weaker for macrovascular disease. We report on the results of estimations of different serum lipids in 78 children with diabetes mellitus. These results are compared with those of assessment of the quality of glycaemic control. The results show an association between the quality of glycaemic control and the height of triglycerid concentrations. There were no associations between the quality of glycaemic control and LDL- and HDL-cholesterol. Cholesterol showed only a weak association to glycaemic control. In nearly all patients HDL-cholesterol was estimated to be in the upper normal range. The results are discussed in respect of possible risks for the development of macrovascular disease.</p>","PeriodicalId":76073,"journal":{"name":"Kinderarztliche Praxis","volume":"61 4-5","pages":"151-3"},"PeriodicalIF":0.0,"publicationDate":"1993-06-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19348423","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Functional encopresis is often connected with other emotional and behavioural problems. After excluding an organic incontinence the main aim of the diagnostic procedure is to search for stressors that may have caused the encopresis and now contribute to the stability of the disturbance. Usually indications of an early disturbed parent-child relation are found. Techniques of behavioural therapy are superior to psychoanalytical methods that do not surpass the rate of spontaneous remissions. The therapeutic approach by Wright and Walker is recommended, which comprises systematic rewards, glycerine suppositories and enemas. 78% of the such treated children showed a lasting remission. The connection of numerous other psychic disorders with encopresis is demonstrated by a case report.</p>","PeriodicalId":76073,"journal":{"name":"Kinderarztliche Praxis","volume":"61 3","pages":"104-7"},"PeriodicalIF":0.0,"publicationDate":"1993-05-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19313602","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

New concepts of treatment are important alternatives for the well known therapy with pyrimethamine and sulphadiazine. They cause less side effects and result in better compliance. Up to know prospective randomized trials are still missing although they might prove the value of medical treatment in ocular toxoplasmosis. Nevertheless it is generally accepted that ocular toxoplasmosis should be treated when central visual function is threatened. Treatment strategy is influenced by general symptoms, especially in children and immunocompromised patients. In this paper opportunities for treatment are discussed.
