Sixty-three adolescents, initially screened at age 10 using teacher and parent behavior rating scales, were assessed at age 13 by means of a semistructured psychiatric interview. In addition, their parents and teachers again completed behavior rating scales. The findings indicate a prevalence in the population of 54% who are clear of personality disturbance, 19% who display marked disturbance, and 27% who display some disturbance. Disturbance in personality functions was found to be associated with increased anxiety and depression, school difficulties and interpersonal withdrawal. Inappropriate behavior, suicide, aggression/hostility and self-abusive behavior were the predominant themes in the drawings and stories depicting “crazy” people. CHARITONOV","doi":"10.1016/S0002-7138(09)60289-2","DOIUrl":"10.1016/S0002-7138(09)60289-2","url":null,"abstract":"<div><p>The mean cardiointerval (CI) (M ± S.E.M.: 0.808 ± 0.019 sec) in 24 children and adolescents with Tourette's syndrome (TS) proved to be significantly higher than in agematched contrast groups with temporal lobe epilepsy (0.718 ± 0.022, N=20), schizophrenia (0.754 ± 0.028, N=15), residual organicity (0.690 ± 0.066, N=6) and myoclonic epilepsy (0.713 ± 0.012, N=7). The individual mean CI values in the TS group were higher than the respective normative age data in 20 of 24 cases. The mean deviation from the respective normative age data for all 24 patients was +10.9 ± 2.5% (p<0.001). The variability of CI values (0.076 ± 0.007, mean group standard deviation ± s.E.M.) was significantly higher in TS than in the contrast groups with temporal lobe epilepsy (0.044 ± 0.006, p<0.001), schizophrenia (0.044 ± 0.005, p<0.001), residual organicity (0.045 ± 0.016, p<0.10), and myoclonic epilepsy (0.051 ± 0.005, p<0.01). Slower and more variable heart rate in TS reflects enhanced vagal tonus in heart regulation. In contrast, increased eye-blinking rate in TS was confirmed, suggesting dopaminergic hyperactivity. Event related potentials (ERPs) were recorded from nine children (age range 10-13 years) diagnosed as having schizophrenia or schizotypal personality disorder, nine age-matched normal children, and nine younger normal children (age range 6-8 years). In all three groups, a positive ERP component in the 300-400-msec range was obtained. The study group and the comparison group, selected from a large nonclinical sample, were balanced for age, sex and socioeconomic status. Results suggest that there is a slight influence of temperament on achievement. Clinical and developmental implications of the findings are discussed. Three basic perspectives on problems and their solutions are (1) clinical, (2) forensic, and (3) systems. These perspectives are described in detail. Models of consultative practice that derive from these perspectives are also described, and an integrative model is proposed.
