Furthermore, because toxicities observed in our studies were identical to those reported in humans dosed with rIL-2, preclinical toxicity using animal models was fully predictive of the pathogenesis of rIL-2 in human. With continuous infusion, which maintained high blood concentrations of the product during the course of the study, rIL-2 constantly stimulated proliferation of lymphoblastoid cells in peripheral blood, subsequently causing vascular leakage, which consists of edema and extravasation of activated lymphoblastoid cells into perivascular areas. The migrating lymphoblastoid cells, identified as cytotoxic T lymphocytes and NK-LAK cells, caused perivascular and hepatocytic necrosis by means of enzymatic secretions from the cells in association with infiltration into parenchymal tissues. In addition to the stimulation of lymphoblastoid cells in peripheral blood, prolonged daily bolus administration of rIL-2 induced extravasation and infiltration of activated macrophages and B cells in tissues of all organs. Eosinophilia, fibrosis in the subcutis, or pseudolobular formations in the liver could be mediated by systemic release of many other cytokines from activated T cells, i.e., CTLs or LAK cells, B cells, and macrophages.</p>","PeriodicalId":75947,"journal":{"name":"International review of experimental pathology","volume":"34 Pt A ","pages":"37-55"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19437143","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

In bacterial meningitis, intrathecal synthesis of TNF-alpha and IL-1 is likely to contribute to CNS injury by recruitment and activation of inflammatory cells with subsequent release of toxic factors, such as reactive oxygen intermediates and excitatory amino acids (glutamate), leading to neuronal cell death with neurologic sequelae. In rats with experimental meningitis, pretreatment with TGF-beta inhibits cerebrovascular changes and brain edema formation in the early, TNF-alpha-independent phase. The data are sufficiently compelling that there is little doubt that this small peptide mediator can induce altered pathophysiology and tissue damage if injected in large quantities. Whereas injection of high-dose, exogenous rHu-TNF results in toxicity, and the production of very high levels in lethal septic shock is detrimental, the exact role of this molecule in inflammation is yet to be defined completely. At low, physiologic levels, TNF may be a necessary component required to orchestrate an effective immune response to a successful resolution. Several works have shown that inhibition of TNF with specific antibodies in models of bacterial infection can decrease survival (Havell, 1987; Echtenacher et al., 1990). Clearly, further work must be done to resolve the issue of the exact role of TNF in organ injury during septic shock.</p>","PeriodicalId":75947,"journal":{"name":"International review of experimental pathology","volume":"34 Pt B ","pages":"7-25"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"19442159","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

There was a resulting rapid release of mature neutrophils from the marrow storage pool into circulation and a distinct neutrophilia maintained for the dosing period in each study. R-metHuG-CSF corrected cyclic hemopoiesis in dogs, established sustained endogenous recovery after lethal bone marrow suppression, and elicited differention and maturation induction and enhanced functional activity of mature neutrophils. Reversible side effects due to an exaggerated pharmacological activity of the growth factor were observed at doses of > or = 115 micrograms/kg/day, with the exception of a rodent-specific osteopathy, which occurred in some animals after 3 months at > or = 5.7 micrograms/kg/day. The only irreversible adverse effect was seen in monkeys at doses of 1150 micrograms/kg/day (equivalent to 200 times the recommended human dose) when leukocyte counts exceeded 100 x 10(9)/liter. It consisted of hyperleukocytosis, leukostasis in terminal capillaries of the brain, followed by tissue hypoxia and intracerebral hemorrhage. Local tolerance of r-metHuG-CSF was good and no hypersensitivity reactions were observed. Antibody formation was minimal in monkeys but occasionally marked in dogs, which is not suprising due to the species difference in the amino acid sequence of the factor. The use of recombinant canine G-CSF, however, did not elicit formation of neutralizing antibodies in dogs. Moreover, relatively few untoward effects were seen in reproductive toxicology except when maternal toxicity or weight loss of dams occurred. In addition, negative results were obtained in all genotoxicity studies performed.</p>","PeriodicalId":75947,"journal":{"name":"International review of experimental pathology","volume":"34 Pt A ","pages":"173-88"},"PeriodicalIF":0.0,"publicationDate":"1993-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"18684616","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
