{"title":"[Clinical and biochemical follow up of Refsum's disease (author's transl)].","authors":"G S Barolin, E Hodkewitsch, E Höfinger, H Scholz, H Bernheimer, B Molzer","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Within a family refered here, two sisters are presenting heredopathia atactica polyneuritiformis (Morbus Refsum) with the complete clinical, electromyographical and serological findings (increased phytanic acid level, accumulation of mono- and diphytanyl, triglycerides). In addition, some symptoms of Refsum's disease are clinically apparent in the eight year old daughter of one of the patients, but significant principal characteristics are absent (dissociation of the spinal fluid, retinopathia pigmentosa, increased phytanic acid level). The phytanic acid level in the serum of ten clinically normal blood relations (three of which are also obligatory heterozygotes) is normal. In both patients with manifest illness we found not only the usual spinal fluid changes with highly increased protein levels (gammaglobulin, IgA- and IgG-fraction) but also a remarkably large number of macrophages with vacuoles. Within almost two years of keeping to a strict diet with low phytanic acid and phytol content, both patients showed a distinct clinical improvement in parallel with a decrease of the phytanic acid level. Even a temporary increase of the phytanic acid level in one of the patients did not cause clinical relapse. {"title":"[Highly located transverse lesion of the cord with quadriplegia caused by giant cell arteritis (author's transl)].","authors":"W D Engelke, D Dörstelmann","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>This is a report of a 64 years old patient who experienced a transverse lesion of the spinal cord with quadriplegia caused by giant cell arteritis. It is stressed that giant cell arteritis not only affects the temporal arteries but on principle any other artery of the body. The giant cell arteritis generally is accompanied with similar humoral findigns as a malignant neoplasm. Therefor in a few cases with high spinal cord lesion suspected to be caused by a malignant tumour the giant cell arteritis may be the real cause, if a spinal mass is excluded. In order to prevent irreversible paralytic defects a cortisone therapy has to be started with as early as possible. {"title":"[Cognitive and learning theory aspects of anxiety (author's transl)].","authors":"C Klicpera, F Strian","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The learning model of anxiety by Mowrer and Miller has significantly influenced the development of theories in clinical psychology and psychosomatic medicine as well as the development of behavioral therapies. This concept, however, is insufficient in explaining factors effective in psychophysiological conditioning experiments, behavioral therapies, and in animal experiments of anxiety conditioning. This study discusses new learning theoretical concepts intending to explain the development and persistence of anxiety states. Therewith the various effects of cognitive, psychobiological, and situational factors on each other are of special significance. {"title":"[Neurological symptoms in a-beta-lipoproteinemia (author's transl)].","authors":"W Tackmann, S Herdemerten","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Abetalipoproteinaemia (Bassen-Kornzweig syndrome), an autosomal recessive inherited disease, up to now has been described in 53 instances in the world literature. Neurological symptoms were reported in 34 of them. Ataxia, loss of proprioceptive sensation, and areflexia characterize this disorder, resembling Friedreich's ataxia. Other signs, which may be inconstantly found, are weakness, diminution of cutaneous sensation, and in the later course an atypical retinitis pigmentosa. Laboratory examinations show acanthocytosis, lowered concentrations of serum cholesterol, triglycerides and phospholipids and as the pathognomonic feature of this disease absence of beta-lipoproteins. Reduced serum concentrations of fat soluble vitamin are secondary effects of this metabolic disorder. In a few cases there are connections to familial hypobeta-lipoproteinaemia, which is autosomal dominantly inherited. However, our studies concerning N = 6384 in-patients with affective psychosis demonstrate that they do not differ from the normal population in respect to their month of birth.
