Renal manifestations are mainly angiomyolipomas, renal cysts and cancer. We report three female patients, mean aged of 31 years (range: 22-37), with a family history of TSC in one case. Cutaneous and nervous symptoms were found in all patients. Clinical complaints were mainly abdominal pain (3 cases), hematuria (2 cases), right flank palpable mass (1 case). Angiomyolipomas (AML) were diagnosed by ultrasonography in all patients, associated to renal cysts in one case. A patient underwent total right nephrectomy without any event during the follow-up. In a second patient, a selective arterial embolization of AML was indicated. Total left nephrectomy was performed due to the persistence of hematuria and the increased flank mass. Pathological examination of the kidney revealed a clear cell carcinoma. The third patient with small AML, associated with renal cysts, required careful monitoring. Renal manifestations in TSC are frequent and serious, they are the second leading cause of death after nervous lesions. {"title":"[Epidemiology and maternal thrombosis].","authors":"Jean-Luc Bosson","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The monthly incidence of deep vein thrombosis during pregnancy varies from 0.1 to 0.8 per 1000 pregnancies, depending on the study. These figures are undoubtedly an underestimation because they were determined from clinical events with no estimation of asymptomatic forms which, in general, increase the prevalence about 3-fold. Although the absolute figures are reliable, the consequences in terms of maternal mortality and post-phlebitis sequelae warrant the careful attention paid to this condition. We report here two cases with favorable outcome. Clinical features are stereotypic: tonsillis, cervical pain revealing deep vein thrombosis, and pulmonary septic metastasis. The most frequent causal germ on blood cultures is Fusobacterium necrophorum but other anaerobial bacteries can be found. Cervical Doppler-ultrasonography, and thoracic tomodensitometry are useful. {"title":"[From back pain to life-discontent. A holistic view of psychopathological contributions to pain].","authors":"Jean-Paul Mialet","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Back pain is a common and disabling condition; it afflicts young adults, and often pursues a chronic course--representing a major public health problem. The physical component of such pain is often overestimated. This paper describes five cases demonstrating how somatic factors can interact with psychological and psychiatric factors--i.e. the 'psychoaffective context'--to determine the degree of perceived pain. In a schema, we summarize the three ways in which the psychoaffective context may contribute to pain: i) physiologically, as a result of increased arousal, leading to muscle tension and perceptual hypervigilance, ii) emotionally, as a result of anxiety leading to stress and feeling of threat, and iii) cognitively, as a cognitive bias caused by increased attention to signals from the target region, and by an increased tendency to interpret these signals as aversive. Personality factors, autobiographical memory, and the place devoted to the body within the narcissistic equilibrium contribute to the degree of anxiety and cognitive bias. Moreover, anxiety and the cognitive bias are also exacerbated by the depressive syndrome that often accompany such pain.
