Cardiac transplantation is still the primary treatment for patients who are in end stage heart failure. The development of artificial hearts (total and partial) was implemented by the discrepancy between the scarcity of available donors and the importance of waiting lists for cardiac transplantation. The technical progresses achieved since the first works of Kolff in 1957 now allow these patients to have their circulation restored and go home to wait for their transplantation. Kouassi","doi":"10.1016/j.anchir.2006.04.007","DOIUrl":"10.1016/j.anchir.2006.04.007","url":null,"abstract":"<div><h3>Aim of the study</h3><p>to report our experience in the management of acute intestinal intussusceptions in adults.</p></div><div><h3>Patients and methods</h3><p>Retrospective study of data of patients older than 15 years who were operated on for acute intestinal intussusceptions from January 1997 to December 2001.</p></div><div><h3>Results</h3><p>Twelve of the patients were males and eight females with an average age of 41 years (range: 16–71). The clinical and radiological findings were suggestive of bowel obstruction (<em>N</em> <!-->=<!--> <!-->14), peritonitis (<em>N</em> <!-->=<!--> <!-->5) and appendicular abscess (<em>N</em> <!-->=<!--> <!-->1). Correct preoperative diagnosis of acute intestinal intussusceptions was established in 6 cases. Type of intussusception was jejunojejunal (<em>N</em> <!-->=<!--> <!-->1), ileo-ileal (<em>N</em> <!-->=<!--> <!-->8), ileocolic (<em>N</em> <!-->=<!--> <!-->1), ileocecocolic (<em>N</em> <!-->=<!--> <!-->7) and colocolic (<em>N</em> <!-->=<!--> <!-->3). Necrosis was found in the intussusceptum in 10 cases and a tumor on the lead point in 14 cases (5 benign lesions and 9 malignant ones). For intussusception involving the colon, all patients underwent en bloc resection with immediate anastomosis, while intussusception located on the small bowel were treated by surgical reduction (<em>N</em> <!-->=<!--> <!-->1), en bloc resection (<em>N</em> <!-->=<!--> <!-->8) with immediate (<em>N</em> <!-->=<!--> <!-->7) or delayed (<em>N</em> <!-->=<!--> <!-->1) anastomosis. The mortality rate was 15%.</p></div><div><h3>Conclusion</h3><p>In our experience, intussusceptions in adults is not an uncommon clinical entity but correct diagnosis is often established peroperatively. Delpero","doi":"10.1016/j.anchir.2006.03.017","DOIUrl":"10.1016/j.anchir.2006.03.017","url":null,"abstract":"<div><h3>Purpose</h3><p>The aim of this study is to show that the removal of the rectum is not an obstacle to implement an optimal surgery in advanced epithelial cancer of the ovary.</p></div><div><h3>Material and methods</h3><p>Retrospective study on a population of 44 women with advanced epithelial cancer of the ovary. The surgery was realized between January 95 and July 03, and all surgeries requiered a posterior exenteration. This treatment was completed by chemotherapy for 36 of them.</p></div><div><h3>Results</h3><p>The median survival of this population is 36,6 months. 6/44 patients (13,6%) had post-operative complications. The completion of chemotherapy started after an average of 5,2 weeks after surgery. All the assessable patients (43/44) have an anal satisfactory continence.</p></div><div><h3>Conclusion</h3><p>The posterior exenteration, when it's necessary, for advanced epithelial cancer of the ovary must not be an obstacle to obtaint an optimal surgery. Anal continence is respected and there are no more complications. Two such cases are described in this article. One is of a 44-year-old man presenting a high grade gastric stromal tumor associated with an adenocarcinoma of the large bowel. The other is of a 68-year-old man presenting an adenocarcinoma of the head of the pancreas associated with a low grade gastric stromal tumor. The hypothesis that the association is due to a simple coincidence particularly in areas with high rates of digestive cancer is proposed. In primary amyloïdosis (light chain amyloïdosis), localized gastric involvement is a rare finding which can mimick malignancy. We report the case of a 56-year-old man, admitted with upper digestive outlet obstruction. Linitis plastica with lymph node involvement was suspected by gastroscopy, barium meal and endoscopic ultrasonography but was not confirmed by gastric biopsies. The patient was treated with total gastrectomy with lymph node dissection. Pathological examination demonstrated gastric and lymph nodes amyloidosis and no malignant tumor was found.
