Before surgery, positron emission tomography scanning was performed: there were no para-aortic nodal metastasis. The histologic examination confirmed the radiological staging. Computed tomoangiography helpfully suggests diagnosis of a mesenteric hematoma and refuts an organic cause as vascular anomalies, by an equivalent method to conventional angiography, noninvasely. In our case report, patient's deterioration justified an emergency surgery corroborating medical imager findings. Post-traumatic adrenal haematoma have been observed in 2% of patients with abdominal injury and can be life threatening in cases of bilateral haemorrhage with risk of acute adrenocortical insufficiency. Diagnosis remains difficult due to absence of clinical and biological specific parameters. One case of retroperitoneal preaortic paraganglioma localised between the celiac trunk and the superior mesenteric artery is reported. The management of paraganglioma involves endocrinologists, geneticists and surgeons but the only potentially curative treatment remains surgical resection. Pathology reports can not always discriminate between benign or malignant tumors. Hereditary in paraganglioma occurs in approximately 25% of cases. Genetic investigation is therefore mandatory in all patients with PG. Authors report a case in a 56 year-old man. The pathogeny is still unknown. Symptomatology has no specificity and the diagnosis is often late, in an advanced stage of the sickness. Recurrent or non recurrent fistula-in-ano requires multiple biopsies for pathology analysis in order to screen a related cancer. Tumor dimension, lymph node involvement, histologic grade and treatment modality are independent prognosis factors for survival. Jardé","doi":"10.1016/j.anchir.2006.06.006","DOIUrl":"10.1016/j.anchir.2006.06.006","url":null,"abstract":"<div><h3>Objective</h3><p>To know the future of appeals against a public establishment of health.</p></div><div><h3>Material and methods</h3><p>We studied 16 files of dispute having ended in a definitive payment over three years in the service of orthopaedics of the Amiens hospital (2001-2003).</p></div><div><h3>Results</h3><p>The majority of the plaintiffs are people (63%), and the average age is of 45,6 years (24 year/68 years). In 14 cases on 16, an expertise was done (7 on the initiative of the hospital and 8 on the initiative of administrative court). The opinion of the expert was followed in every case, except one. In the final, 5 patients (31%) received a compensation (3 with the administrative court and 2 after a love rule). Eleven patients (69%) were not indemnified (7 after demand with the administrative court 4 after demand of love rule). On the whole, 10 case was treated in the administrative court (62%) and 6 by the legal department of the hospital. Among cases settled by the service of dispute of the hospital, 2 ended in a love rule and 4 were classified. Fifty cases have been described in the literature. We report two new cases. The first is of a 12-year-old child with left multiple ureteric valves associated to bilateral vesico-ureteric reflux. The patient was treated by uretero-ileoplasty with unfavourable evolution to renal failure. The second case is that of a proximal ureteric valve discovered in a 30-year-old woman who had postpartum acute pyelonephritis. Treatment consisted in ureteric resection–anastomosis with favourable outcome.
