{"title":"[Suicide attempts in old age - a follow-up study (author's transl)].","authors":"C Wächtler","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>48 patients above the age of 55 years were seen in a follow-up study 8 1/2 months after having been admitted to an emergency ward following a suicide attempt. Somewhat more frequent than in young persons the suicide attempts were caused by depressive or schizophrenic psychosis. Nevertheless, with the elderly the most frequent diagnosis was depressive reaction and neurosis. Physical illness was overrepresented in our group, but only in some cases fear of being ill was justified by the real physical status. Such cognitive dissonances were also found to concern the subjective valuation of the social situation. Some of the patients were satisfied with their living conditions though they were actually isolated. On the other hand patients with a high rate of social contacts sometimes complained of loneliness. A real social isolation of old people having attempted suicide seemed to be more rare than expected. Finally we found that the elderly chose different ways to seeking help from social and medical services in comparison to younger people. The personal tie to the doctor was of great importance for this group. - During the time of investigation only one patient had attempted suicide once more. In both groups there is an equivalent increase of the cardiac index by 20%, the initial value being lower in groups B. The increase lasts for more than 2 Min. During 5 Min the pulmonary pressure is increased by 25%. By introducing a HDL-cholesteroltest in the multiple screening we have the chance to valuate the diagnostical validity of the test value of triglycerides, total cholesterol and HDL-Cholesterol and of the calculated value of LDL-cholesterol and ratio HDL/LDL-cholesterol. 2. We studied the incidence of lipoid findings of 210 patients by graphical statistics. 3. The test values of triglycerides, total cholesterol and HDL-cholesterol manifest a mixed collection with a graph having different maximums whereby statistically a \"normal\" population can be differentiated from a \"pathological\" population. 4. The calculated value LDL-cholesterol is normal distributed, the ratio HDL/LDL-cholesterol is logarithmic normal distributed. (author's transl)].","authors":"J Mössner,&nbsp;H J Pusch,&nbsp;W Koch","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>We compared the excretory pancreatic function of a healthy young population (n = 18, mean age 31.8 years) with that of a healthy older people (n = 18, mean age 58.7 years) using a modified secretin-pancreozymin test which comprised short infusion of secretin, injection of pancreozymin, and repetition of this procedure immediately after the end of the collection period following the first stimulation. Secretion rate and bicarbonate following secretin, lipase, amylase and secretin rate following pancreozymin were significantly lower in the old than in the young after first and second stimulation. Trypsin was lower in the old than in the young following repeat stimulation. Increasing amounts of money are invested in basic and applied research in aging although priority allocated to gerontology is still low when compared to other areas. The University gerontological establishment (Chairs in Gerontology and Geriatrics, Institutes, Centers etc.), has mushroomed in the USA, Europe and the developed areas of the world and its costs, though not exactly known, are growing. However, it is the gerontological practice which is the most costly of all as \"graying budgets\" of stages and the growing share of aging in GNP of the Western World are showing. Conflicting tendencies are discernible between the growing demands for further development of services and rising costs of care. The question to what extent will societies be able to expand or even to maintain the present investments in gerontological practice is a yet unanswered. A survey carried out on 296 patients primarily suffering from heart circulation disease in the preclinical and clinical stage has shown that in case of elderly patients one generally must reckon with a latent heart and circulation disease. The risk of complications arising as result of the physiotherapy with respect to the heart and circulation system can be reduced by a thorough diagnosis. The relative intensity of the exercise employed should be on the low side. The study was based on the reduced reinforcement hypothesis which predicts a low rate of social relationships (i.e. the loss of positive reinforcement) for those individuals who are prone to depression. This hypothesis was both tested and confirmed among the residents in homes for the aged in Berlin. The senior citizens who reported a low rate of social activities had higher depression scores than those reporting a higher rate of such social involvement. These results were confirmed for various indicators of social participation. A further variable influencing the rate of depression was identified as the self-rating of personal health. The polypi of 170 patients were located at a rate of 63,5% in the rectum and sigma area and of 36,5% in the proximal colon sections. Out of 45 carcinomas 80% could be localized in the rectum and sigma area and 20% in the further parts of the colon. Patients, if only sigmoidoscopically examined, showed polypi at a rate of 4,7% and carcinomas of 1,3%. For patients coloscopically examined polypi were found at a rate of 23,5% and carcinomas of 8% in the rectum and sigma area. 66% of all polypous changes and 82% of carcinomas were found with patients beyond 60 years of age. In 17 cases colitis ulcerosa, in 3 cases Morbus Crohn and in 3 cases of lipom could be established. The relatively high rate of pathological abnormities found in the upper colon section proves the diagnostic value of a total coloscopic examination.
