[Dental treatment and the prognosis for handicapped children]. [残疾儿童的牙科治疗与预后]。

The subjects were 77 handicapped children who received dental treatment at the pedodontic clinic of the Aichi-Gakuin University Dental Hospital and periodic examinations after the completion of dental treatment. The average age of the subjects was 7.4 (2.11 to 16.7 years old). (1) At the first visit, the mean number of untreated decayed teeth was 7.1 for the group of children who received dental treatment without general anesthesia, and 12.0 for the group of children who received dental treatment with general anesthesia. And the rate of untreated decayed teeth was 40.1% of the deciduous and 21.0% of the permanent teeth for the group of children who had received dental treatment without general anesthesia, and 65.5% of the deciduous and 34.2% of the permanent teeth for the group of children who had received dental treatment with general anesthesia. (2) For the group of children who received dental treatment without general anesthesia, restoration was performed on 62.5% of the deciduous and 87.5% of the permanent teeth; the remaining 37.5% of the deciduous and 12.5% of the permanent teeth were extracted. Endodontic treatment was performed on 20.6% of the deciduous and 8.9% of the permanent teeth. (3) For the group of children who received dental treatment with general anesthesia, restoration was performed on 57.0% of the deciduous and 95.1% of the permanent teeth; the remaining 42.9% of the deciduous and 4.9% of the permanent teeth were extracted. Endodontic treatment was performed on 25.3% of the deciduous teeth and 12.2% of the permanent teeth. (4) In the case of restorative treatment, the highest incidence was in metal crowns on the deciduous teeth, and amalgam fillings in the permanent teeth for the groups of children who received dental treatment with or without general anesthesia. In the case of endodontic treatment, the highest incidence was in pulp amputation for the groups of children who received dental treatment with or without general anesthesia. {"title":"[Pharmacological study on blood pressure in rats with bone disorders].","authors":"T Shamoto","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>To evaluate the relationship between the elevation of blood pressure and altered bone metabolism, the changes of systolic blood pressure in six experimental models for bone disorders were investigated. Rats used were either parathyroidectomized, ovariectomized, fed with a calcium-deficient diet, fed with a vitamin D-deficient diet, treated with HEBP (1-Hydroxyethylidene-1, 1-bisphosphonate) or treated with streptozotocin. Hypertension developed in 5-week-old male rats fed with a calcium-deficient diet for 2 weeks, which evoked hypocalcemia and nutritional hyperparathyroidism. The blood pressure returned to normal when fed with a normal calcium diet. In parathyroidectomized rats receiving a normal calcium diet, the blood pressure did not rise, though the plasma calcium level decreased to an extent similar to the rats fed with the calcium-deficient diet. These findings seem to indicate that hyperparathyroidism, but not hypocalcemia, was involved in the elevation of blood pressure in rats fed with a calcium-deficient diet. Hypertension was not observed in rats fed with a vitamin D-deficient diet or treated with streptozotocin. These rats showed not only an increase in parathyroid hormone (PTH) but also a decrease in 1,25 (OH)2 D3. These results may suggest that the presence of 1,25 (OH)2D3 as well as the enhanced parathyroid function is necessary for the development of hypertension. The elevated blood pressure was reduced by a calcium antagonist, nifedipine, or by calcium supplementation, but not by an inhibitor of angiotensin-converting enzyme, captopril, or by calcitonin. This may indicate that hypertension due to nutritional hyperparathyroidism responds to the calcium antagonist nifedipine and to calcium supplementation, but does not depend on renin or salt. Furthermore, an acute hypotensive effect by human PTH (1-34) was not observed in the hypertension of calcium-deficient rats, suggesting the difference between acute and chronic effects of PTH. The materials used in the present study consisted of plaster casts for 397 people, ages 4-20, collected on Rarotonga, Mangaia and Pukapuka in the Cook Islands. The dental characters observed were 15 traits on the teeth crown. The samples have been divided into five population series on the basis of their native islands: Pukapuka, N-S group (northern-southern island crossbreedings), Rarotonga, Mangaia, and S group (southern islands except Rarotonga and Mangaia). For comparison with other ethnic populations, the samples excluding the Pukapuka are combined into one group to represent full-blood southern-Cook Island Polynesians and named \"Southern Cook\" here. There were no significant sex differences in the incidences of dental traits in the data used in this study, so they were combined for the statistics. A cluster analysis was carried out to compare inter-population affinities on the basis of Smith's Mean Measure of Divergence (MMD) using the frequencies of shovelling, Carabelli's cusp, protostylid, cusp 6, cusp 7, and deflecting wrinkle. The results are briefly summarized as follows: 1. The differences between the Pukapukans and other four groups in the Cook Islands were observed in the case of 11 characters. Within the samples of the four island groups making up Southern Cook, there were no definitive population differences in dental morphology. The cluster analysis based on Smith's MMD values emerged as two main clusters in the Cook Islands: one cluster consisted only of the Pukapukans, and the other consisted of the rest. The latter was divided into two subclusters. 2. The frequencies of shovelling in the Southern Cook (23%) were quite similar at the medium level (S + S.S), to those in other Polynesian groups as well as in the Micronesian groups. The Pukapukans, however, gave a fairly low percentage (10%). 3. The two populations from the Southern Cook and Pukapuka were compared with other Oceanic populations. The incidences of double shovelling were similar to those of the Guamese, but lower than those of the Hawaiians. The Carabelli's cusp, protostylid and cusp 6 showed higher frequencies than those in the Hawaiians and Guamese. On the other hand, the interstitial tubercle showed a lower percentage than those in the Hawaiians and Guamese. The first of these was a 51-year-old man who complained of swelling in the right cheek and the mandibular region. The other was a 27-year-old man who chiefly complained of swelling, pain and trisms in the right mandible. Both patients were treated with antibiotics and the osteosclerotic tissue was removed. The antibiotics were injected into the superficial temporal artery using a one-shot technique. Radiographs of the patients taken one to two years after the treatment revealed normal conditions with no signs of recurrence. In this study, ossification of the distal phalanx of the first digit was used as an indicator of bone maturity. We found significant correlation between this ossification of the phalanx and the cranio-facial development. Stage E2 was reached, on the average, at the age of 6 years 4 months in girls and 8 years 11 months in boys, stage E4 at 12 years 9 months in girls and 14 years 11 months in boys. Girls were ahead of boys, the difference being about two years at both E2 and E4 stages. The degree of the cranio-facial development at stage E2 compared with adults was 83.5% in girls and 83.8% in boys; at stage 4 it was 94.8% in girls and 94.5% in boys. At stages E2 and E4, the cranial base was the most developed, the maxilla was next, and then the mandible. This tendency was found in both boys and girls. A4-Go was the least developed at both E2 and E4 stages. Mandibular movements with or without the intraoral articulating paper (thickness: 35 microns) were detected as the lower incisal point displacement and were recorded three-dimensionally with the Selspot system. The date displayed on the X-Y chart recorder on the selected plane were analyzed quantitatively. 1. The paths of the mandible differed between the outward (from the intercuspal position) and the return (backward to the intercuspal position) movements. In lateral excursions the return path tended to be antero-inferior to the outward path, whereas in protrusive excursions the former tended to be dextro-inferior to the latter. 2. In lateral excursions the return path tended to be less stable than the outward path. In protrusive excursions no significant difference was found between the two paths. 3. In lateral excursions, 21-34% of the movements performed using the articulating paper deviated from the normal paths made without using the paper, and in the case of protrusive excursions, deviation was found in 21-50%. At or near the intercuspal position, the return path showed a greater deviation than the outward path. Near the edge-to-edge position, however, the relationship between these two path was reversed. 4. In this study, two types of alloy specimens, namely Ag-Pd alloy and Ni-Cr alloy, were prepared by casting. These specimens were given sandblast treatment by 37 microns or 250 microns Al2O3 particle under the following conditions: 3 kg/cm2 for 30 sec, 5 kg/cm2 for 10 sec, 30 sec and 45 sec, and 7 kg/cm2 for 30 sec. Under each condition of sandblast treatment, changes in roughness and wettability of the alloy surface were measured. After this treatment, the alloy surface was Silicoated, and then a light-cured resin was bonded to it. Specimens were stored in air for one week and thermal cycled from 4 to 60 degrees C for 10(4) cycles. The bond strength was measured by compressive shear stress. It was shown that the sandblast treatment caused the surface area to increase in the range of 1.3-1.9 times. Also, the sandblast treatment made the alloy-water contact angle smaller and wettability greater. Maximum bond strengths for Ag-Pd alloy and Ni-Cr alloy, when sandblast treated and stored in the air for one week, were 213.7 kg/cm2 and 198.7 kg/cm2 respectively. On the other hand, bond strengths for Ag-Pd alloy and Ni-Cr alloy after thermal cycling were 129.5 kg/cm2 and 159.9 kg/cm2 respectively. These values of bond strength were 2-3 times greater than those of for specimens not given the sandblast treatment. [Etiological study of relationship between impacted permanent teeth and malocclusion]. 阻生恒牙与错牙合关系的病因学研究

The materials used were dental X-rays, occlusal dental X-rays, panoramic radiograms and cephalograms and dental casts of 194 cases (51 males and 143 females) which revealed impacted permanent teeth among 3979 cases (1215 males and 2764 females) from 1964 to 1985 in the Department of Orthodontics, School of Dentistry, Aichi-Gakuin University. The results obtained were as follows: 1. The incidence of impacted permanent teeth in these samples was 4.9% of the entire number of orthodontic patients. There was no significant difference between sexes. 2. The incidence according to the number of impacted permanent teeth was as follows: one impacted tooth, 74.8%; two impacted teeth; 21.1%, three impacted teeth; 3.6%, four impacted teeth; 0.5%. The incidence of two symmetrically impacted teeth was 61.5%. 3. As to the type of impacted teeth, the frequency of maxillary central incisors was the largest (33.7%), followed by 3 (25.0%), 5 (22.2%), 5 (11.9%), 2 (4.4%), 3 (1.2%), 4 (0.8%), 4 (0.4%), and 7 (0.4%). But no impacted teeth were found in the regions of 1, 2, 6, 6, 7 and 7. 4. As to impacted teeth and the type of malocclusion, the spaced arch in the anterior region had impacted teeth most frequently followed by crowding, open bite and lateral cross bite. Malocclusions with the smallest incidence were cross bite and upper protrusion. The spaced arch had a higher incidence of impacted teeth than bimaxillary protrusion. In the posterior region (impacted molar teeth), the lateral cross bite, spaced arch, and upper protrusion had impacted molars most frequently, crowding was second and cross bite had the least. 5. In the direction of long axis, the impacted tooth with a horizontal direction was found most frequently (35.2%), followed in the order of frequency by inverse direction (32.7%), labial or mesial tipping (22.0%), lingual or distal tipping (5.7%), and right-angled (4.4%). Moreover, the inverse maxillary central incisor and mesially tipped maxillary canine were found most frequently. The frequency of maxillary central incisors with crooked roots was 64.7%, which was higher than that of maxillary canines (19.0%). 6. Lack of space is the most frequently found cause of impacted teeth, and in the anterior region this was the main cause of canine impacted teeth. X buccolingual d.). The determination of molar size order was estimated by the three methods of mean values, size sequences and reductive indices (M2/M1, M3/M1). The results obtained are summarized as follows: 1. The molar size sequence of the seven species. The pattern of sequence in the molars of the Cercopithecidae was divided into six types: (I) M1 less than M2 less than M3 or (M1M2) less than M3, (II) M1 less than (M2M3), (III) M1 less than M3 less than M2, (IV) (M1M2M3), (V) (M3M1) less than M2, and (VI) M3 less than M1 less than M2 or M3 less than (M1M2). 2. The upper molars. i) The most frequent pattern of mean values found was type II for all the species of the Macaca and Colobus. The two species of Cercopithecus showed predominantly the characteristics of types III, IV and V. ii) The molar size sequence individual data revealed that in the mesiodistal crown diameter types I and II were distributed frequently in the Macaca, types IV and V in the Colobus, and type VI in the Cercopithecus. The reduction of the third molar, therefore, seemed to be more pronounced in the Cercopithecus when compared with the other two genus, Macaca and Colobus, because type VI is the type with more size reduction in the third molar. In the case of the buccolingual crown diameters, the most common size sequences were found in types II and III for the Macaca and Colobus. In the Cercopithecus, however, type V exceeded other types in frequence. iii) The results of the reductive indices showed that the second molars in the Colobus were greatly reduced compared to the first, followed by those of the Cercopithecus and Macaca. In the third molar reduction, the Cercopithecus had a relatively small third molar compared to the first, following the Colobus and Macaca. 3.
