Baseline data and 5-year mortality experience.","authors":"A Reunanen,&nbsp;A Aromaa,&nbsp;K Pyörälä,&nbsp;S Punsar,&nbsp;J Maatela,&nbsp;P Knekt","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The Social Insurance Institution's Coronary Heart Disease Study is a prospective population study designed to investigate the prevalence, risk factors and incidence of coronary heart disease (CHD) in middle-aged Finnish men and women. The study population consisted of 5 738 men and 5 224 women, aged 30-59 years at entry, drawn from 12 cohorts from south-western, western, central and eastern Finland. The cohorts consisted of whole or random samples of rural or semiurban dwellers or employees of a factory. The participation rate was 90 per cent. The prevalence of symptoms was determined by the Rose questionnaire and abnormalities on resting ECG were coded according to the Minnesota code. Blood pressure, smoking habits, serum cholesterol, triglycerides, postload plasma glucose and obesity were the risk factors analysed at the baseline examination. The mortality of examinees has been followed continuously. This report deals with the main findings at the baseline examination and the mortality follow-up experience in 5 years. The prevalence of typical angina pectoris was 4.4 per cent in men and 5.4 per cent in women. Unequivocal ECG signs of past myocardial infarction were observed in 1.0 per cent of men and 0.3 per cent of women. Other ECG findings suggesting CHD were observed in 9.2 per cent of men and 11.1 per cent of women. The 5-year mortality was 4.3 per cent in men and 0.9 per cent in women. Men with typical chest pain symptoms had a seven-fold risk to die from CHD, compared to men without symptoms. Men with ECG abnormalities compatible with an old infarction had a 19.5-fold and men with other ECG findings suggesting CHD a 7.1-fold risk to die from CHD compared to men without resting ECG abnormalities. Men with ECG findings as the only indicator of CHD had worse survival than men with symptoms as the only indicator of CHD. The value of symptoms and ECG findings as predictors of CHD mortality in women was very low.</p>","PeriodicalId":75385,"journal":{"name":"Acta medica Scandinavica. A review.","authors":"J Ostman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The use of beta-blockers in diabetes mellitus has largely been restricted because of the reported adverse effects. Clinical investigations aimed at elucidating the possible reactions associated with the use of beta-blockers have disclosed no evidence of masking or signs or insulin-induced hypoglycaemia or potentiation of the insulin effect. Prolonged hypoglycaemia may develop, however, as a result of physical effort. There is no proof that during insulin-induced hypoglycaemia the concentrations of counter-regulatory hormones are depressed, but that of glycerol, a gluconeogenic precursor, is slightly diminished. Intensification of the hypertensive reaction during hypoglycaemia is less likely to occur during treatment with beta-selective blockers. In insulin-dependent diabetics receiving beta 1-blockers there is no evidence of any change - either deterioration or improvement - in metabolic control. In one small controlled trial there was no sign of impairment of the peripheral arterial circulation over a short period of administration of a non-selective beta-blocker. In general, for patients suffering from insulin-dependent diabetes, cardioselective agents are preferable. Since cardioselectivity is a dose-dependent property, reasonable caution should also be observed when using this type of drug in diabetes.</p>","PeriodicalId":75385,"journal":{"name":"Acta medica Scandinavica. Half were treated with hydrochlorothiazide and triamterene and half were given placebo and a second capsule was given and if necessary up to 2 g of methyldopa/day. The measurements in a sample of 157 patients suggested 80-86% compliance rate. A significant blood pressure difference of 25/10 mm Hg was obtained between the groups and maintained during five years of follow-up. The increase in serum creatinine found in the actively treated group was related to the decrease in sitting systolic blood pressure. Changes in serum uric acid correlated with changes in serum creatinine both in the placebo and in the actively treated group. Fasting blood glucose changed significantly in the active treatment group. The balance between this decreased risk on the basis of the blood pressure reduction and the increase produced by the rise in blood glucose and the other treatment effects remains to be determined. A mini review.","authors":"J Ostman","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Altogether, histologic findings in animals and in man with diabetes mellitus, together with clinical findings in man with this disease and with biochemical abnormalities, suggest that the main cause of microangiopathy in persons with diabetes mellitus lies in metabolic disorders. The possibility that genetic and environmental factors are influential cannot, however, be ruled out. In a small although steadily increasing number of diabetics early microangiopathic and neuropathic abnormalities have proved to be reversible. This improvement includes functional abnormalities of the retina, glomeruli and nerves. Early morphologic changes of the retina also, seem to be preventable. On the other hand, more severe morphologic changes such as proliferative retinopathy, and severe diabetic neuropathy have proved resistant to intensified insulin treatment. It remains to establish whether continuous optimal control by insulin treatment generally can prevent early although clinical microangiopathy. The development of instruments and other aids for improved metabolic control, such as transplantation of insulin-producing tissue, would seem to be a logical step in this direction.</p>","PeriodicalId":75385,"journal":{"name":"Acta medica Scandinavica.
