Material and methods: We treated 3 patients with choledocho-enteric fistula using originally designed biliary catheter endoprosthesis of 14-16 Fr, made of the distal end of drainage catheter, which enabled bile derivation with simultaneous treatment of fistulous channel. In pre- and postprocedural evaluation we used ultrasonography and computed tomography. Results: In 2 of 3 cases we treated the fistula successfully while preserving bile derivation. The average survival time was 7,3 months (5-10). Conclusion: The use of this therapeutic procedure is reserved for the terminal phase of malignant disease. It is a paliative procedure and implies previous experience in the use of all possible percutanous biliary drainage.","PeriodicalId":75377,"journal":{"name":"Acta chirurgica Iugoslavica","volume":"62 1","pages":"41-45"},"PeriodicalIF":0.0,"publicationDate":"2015-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68393154","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Comparing to other cyclophotocoagulation it is more safe and selective. Aim of the study: To evaluate the efficacy and safety of transscleral diode laser cyclophotocoagulation in treatment of refractory glaucoma. Material: A retrospective cohort study included 35 patients (36 eyes) with refractory glaucoma treated in Centre for Sight \"Oftalmika\", form March 2008 to February 2012. All patients were treated by transscleral diode laser cyclophotocoagulation. The mean age was 68.8 ± 16.8 years. Most prevalent type was the neovascular glaucoma (23 ili 65.7%). The treatment consisted of 16-20 applications of 1.5-2.0 W energy applied for 1.5-2.0 seconds. Results: The average IOP reduction was 33.4mmHg or 63,1%. The best treatment result was achieved in the neovascular glaucoma, where IOP reduction 6 months after the procedure was 71,2 %. The poorest results were obtained in congenital glaucoma (28,5%). Prior ciclodestructive procedures and/or filtration glaucoma procedures did not influence laser treatment results. In only 5,7% patients repeated procedures were indicated. Overall complication rates was 22,8%, and the most prevalent complications were hyphema and reversible keratopathy (8,6% eyes). Postoperative hypothonia had one patient. Number of antiglaucoma medications, 6 months after the treatment, was reduced by 1,1 (45,7%). Medication reduction was most prevalent in the case of seconday glaucoma 1,4 (52,6%), and the least prevalent in congenital glaucoma 0,5 (14,3%). Only one patient had occasional pain after the procedure. Conclusion: Transscleral diode laser cyclophotoco-agulation is safe and effective procedure in the treatment of refractory glaucoma. The best treatment result was achieved in the neovascular glaucoma, and the poorest in congenital glaucoma. Effect of the procedure on pain relif is unequivocal.","PeriodicalId":75377,"journal":{"name":"Acta chirurgica Iugoslavica","volume":"62 1","pages":"19-23"},"PeriodicalIF":0.0,"publicationDate":"2015-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68393536","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This type of trauma can range in its severity and some injuries can require immediate or delayed urinary reconstruction. In addition to direct trauma to the lower urinary system (bladder, prostate and urethra), pelvic trauma can disrupt nerves to the bladder and penis and lead to urinary problems like leakage and erectile dysfunction. Bladder rupture is rare and is often associated with other serious injuries and a high mortality rate. The bladder can also be torn or burst during a pelvic fracture. If these tears are large or urine spills into the abdomen around the intestine, they have to be fixed surgically.","PeriodicalId":75377,"journal":{"name":"Acta chirurgica Iugoslavica","volume":"62 1","pages":"9-12"},"PeriodicalIF":0.0,"publicationDate":"2015-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68393567","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Swallowed foreign body is more common in children, persons deprived of their liberty and psychiatric patients. It frequently passes asymptomatic by elimination from the digestive tract. The patient J.P. 75 years old from Bar, hospitalized at the department because of metallic foreign bodie - sewing needle in the abdominal cavity. X-ray native abdominal, MDCT of the abdomen and EGDS in consultation with gastroenterologist from Clinical Center in Podgorica, is made outpatient. After treatment the patient was operated on 19th November, 2013 by the classic laparotomy and after the expected recovery, without complications discharged. Foreign body in the omentum can arrive from the digestive tube migration and penetration through the abdominal wall. In some organs of the abdominal cavity, such as the liver can come through bloodstream. Perforation of the GI tract occurs in less than 1% of cases, mainly through the digestive tube runs without problems in about a week. The migration of a sewing needle through the stomach can cause a state of emergency abdominal or can go through asymptomatic. Treatment is traditionally classical laparotomy, while today there are more works that favor the laparoscopic method. The aim of this study is to precisely identify the characteristics of the fractures, that will be taken into account in the individualization of treatment of intracapsular fractures of the femur. Materials and Methods: We analyzed, in a retrospective study, 148 patients with intraarticular fractures of the femoral neck from the registry of the Orthopaedic Department in Pozarevac, in the period from 2009 to 2014. Fractures were classified by the modified Garden’s classification. Garden type III fractures were divided into two sub-types. Garden type IIIa included fractures in which the distance between the fragments of the medial cortex was less than ½ the diameter of the femoral neck. Garden type III b included fractures in which the distance of the medial cortex of the fracture fragments was larger than ½ the diameter of the femoral neck. Patients with the Garden type I fracture (6 cases), Garden type II ( 22 cases), Garden type IIIa (35 cases) and Garden type IV (5 cases) were treated by closed reduction and internal fixation. Patients with Garden type IIIb (28 cases) and Garden type IV (52 cases) were treated by primary total hip replacement. Results : All fractures in patients with the fracture Garden type I (6 cases) and Garden type II (22 cases) have healed and avascular necrosis did not occur. In the group of patients with the Garden type III b fracture, 3 patients with non-union were treated with total hip arthroplasty. In the same group in two patients avascular necrosis occurred. In the group of patients with the Garden type IV fracture who were treated by closed reduction and inter- nal fixation (5 cases), two patients with non-union were treated with total hip arthroplasty. In patients with Garden sub-type III b, in the time of monitoring we found 4 dislocations after primary total hip arthroplasty. One patient underwent acetabular revision in the same group. In the group of patients with Garden type IV fracture, we found 7 dislocations after total hip arthroplasty and two patients underwent acetabular revision surgery. Conclusion: Our results indicate that fractures Garden type I, II and Garden type III a can be successfully treated with internal fixation. Fractures Garden type III b and IV should be treated by primary hip arthroplasty, because of internal fixation of these fractures lead to unsatisfactory results.","PeriodicalId":75377,"journal":{"name":"Acta chirurgica Iugoslavica","volume":"62 1","pages":"65-68"},"PeriodicalIF":0.0,"publicationDate":"2015-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68393306","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

AIM: The aim of this study was to investigate the effects of mistletoe extract on platelet as well as monocyte functions, as an important factors in immunomodulation of cancers metastatic potencial and angiogenesis in tumors. METHODS: The effect of different concentrations of mistletoe extract on agonist-induced platelet activation markers and their aggregation with leukocytes was examined in the blood of healthy subjects (n=6) using flow cytometry. Effects on LPS -induced activation markers was determined in the blood of healthy subjects as well as on THP-1 cell line using an ELISA essays and flow cytometry. RESULTS: Mistletoe extract significantly inhibited agonist induced P selectin expression and platelet-monocytes aggregation. Additionally, mistletoe extract exerts anti-tumor effect through the stimulation of TNF-a production in LPS induced monocytes activation. CONCLUSION: Obtained data demonstrate that mistletoe extract was effective in modulating platelet and monocyte functions, as a part of pleiotropic anticancer effect. Benignant and malignant lesions include various tumors, most frequently lymphomas. The histological diagnosis obtained by biopsy is the most important determinant for the further treatment. Introduction of a known biopsy method - the Chamberlain procedure in our institution has been commenced as one of the methods for obtaining tissue samples of the anterior mediastinal masses. An anterior mini thoracotomy in the 2nd or 3rd intercostal space gives an excellent approach to the tumor. Three children were operated by this method. The volume of three obtained tissue samples was sufficient for the pathologist and gave a 100% accurate diagnosis. The Chamberlain mini anterior thoracotomy is a good biopsy operative method. It can be used in hospitals which do not have endoscopic thoracic equipment or trained endoscopic surgeons, it is easy to perform and gives comfort to the anaesthesiologist.","PeriodicalId":75377,"journal":{"name":"Acta chirurgica Iugoslavica","volume":"62 1","pages":"15-17"},"PeriodicalIF":0.0,"publicationDate":"2015-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68393523","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
