CD-V mode Mitkovic type external fixator had been used in both cases for correction of these deformities. The method is based on hemicallotasis - bone callus distraction after a partial transversal bone cut (hemocorticotomy). The first patient had varus-recurvatum posttraumatic deformity of the ankle joint. Operation time was 65 min and hospital stay was 9 days. Superficial pin-track infection was resolved by the pin removal and by daily wound toilette. The second patient had varus-antecurvatum malunion deformity of the femoral diaphysis. Operation time was 50 min and hospital stay was 6 days. Mild knee contracture had been successfully treated by physical therapy. Angular deformity correction had been performed by distraction (for the first patient) and compression (for the second patient) in the fixator frame. Hemicallotasis of femur or tibia using CD-V mode Mitkovic type external fixator is a good alternative in the treatment of posttraumatic lower limb angular deformities, providing minimal surgical trauma and maximal use of the bone osteogenetic capacity. Gradual compression or distraction in the fixator frame allows to perform accurate angular correction in both coronal and sagital plane in one act. [Projekat Ministarstva nauke Republike Srbije, br. Method: We conducted concomitant study in Japan and Europe , in 31 Asian patients (AP) with AAA and 130 control subjects of the same population, and in 30 European patients with AAA and 126 control subjects (EP). We observed various demographic and anthropologic parameters. Aortography was performed in all patients at the same type of CT-scanners using the same examination protocol and postprocessing. For data analysis, various statistical models were used. Results: There are statistically significant differences in multiple epidemiologic and morphologic findings in AAA patients, as well as in comparison with controls in both groups. This is most distinct in terms of anthropologic characteristics and number of risk-factors, and in terms of morphologic findings, in the length of neck of the aneurysm, transverse diameter of abdominal aorta (AA) and common iliac artery (c.i.a), and volumes of c.i.a. We obtained paradoxical results considering diabetes mellitus, which presented itself as a protective factor for AAA. Conclusion: Demographic-epidemiologic analysis accompanied with morphologic measurements using modern imaging modalities enables obtaining new information on pathology of AAA in different races. There are differences in number of risk-factors, and in terms of morphologic findings, in the length neck, angle of the aneurysm, transverse diameters of abdominal aorta (AA) and in the length of common iliac artery (c.i.a), and volume of c.i.a. Recent reports showed that CM is not associated with increased morbidity, but those are lacking the data about risks of intensive care unit (ICU) treatment. Aim: The authors evaluated the factors affecting the duration of ICU treatment in patients after CM. Material and Methods: The study included 57 women who underwent CM and were postoperatively admitted and treated in ICU. Correlation analysis was used to estimate the effect of various parameters on the duration of ICU treatment. Results: There was a highly significant correlation between duration of ICU treatment and number of postoperative transfusions (p=0.001), duration of surgery (p=0.007), intraoprative hemorrhage (p=0.008) and myoma diameter (p=0.009). Duration of ICU treatment was also correlated with gestational age, hypertensive syndrome in pregnancy, preoperative hematocrit and hemoglobin values, number of intraoperative transfusions, postoperative hemorrhage and repeated myomectomy. Conclusions: In our report, longer ICU tretment was required in cases of perioperative hemorrhage, prolonged surgeries and those requiring perioperative transfusion. Patients who have had previous myomectomy, with lower preoperative hemoglobine and hematocrit vaues and bigger myomas are at risk of prolonged ICU treatment.","PeriodicalId":75377,"journal":{"name":"Acta chirurgica Iugoslavica","volume":"62 1","pages":"71-76"},"PeriodicalIF":0.0,"publicationDate":"2015-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"68393712","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Different treatment methods of these injuries are described in the literature. Authors of this paper are presenting own experience and results in the method of „one-stage“ external fixation with or without limited internal fixation of the fracture. Material and methods: All the patients had emergent surgical treatment in 4 to 8 hours after the hospitalisation. Surgical protocol included wound irrigation, debridement, fracture reduction and external fixation in type A fractures. Type B and type C fractures were treated by „bridging“ external fixation with limited internal fixation. In the cases where wound had not been possible to close primarily additional surgical procedures (soft tissue defects coverage) were performed. Results: Final functional outcome was excelent in 5(31,25%) patients, good in 6 (37,5%) patients, moderate in 3(18,75%) patients and bad in 2 (12,5%) patients (according Karlsson Score). Final anatomical and functional results were in correlation with the type of primary injury. One case of septic pseudoarthrosis and three cases of late ankle arthrosis were found as a major complication. Conclusion High-energy open distal tibial pilon fractures are still considered as a challange and significant surgical problem. The method of external fixation with or without limited internal fixation, as „one-stage“ method, was approved in clinical practice to give excelent and good final functional and anatomical results. Though final functional results were in correlation with degree of the injury, described method gives good outcome results and further clinical application will surely approve its clinical significance. Main age of the patients was 62,3 years. We used Cierny-Mader classification for the clinical and pathoanatomical assessment. For the assessment of the bone defect we used Engh classification. Complete union we had in 22(81,4%) patients. Mean time for healing was 5,7 months, range (3-15). Mean residual limb shortness was 4,7 cm and mean follow-up was 21 months. We also had a five nonunion (18,6%) complications: three with septic intrarticular nonunion, two had intolerance to the Ilizarov apparatus, so we removed earlier.
