Johns, Alka Ahuja","doi":"10.1002/rfc2.71","DOIUrl":"https://doi.org/10.1002/rfc2.71","url":null,"abstract":"To assess patient and clinician satisfaction and identify any differences between the two with the use of video consulting (VC) in the fields of obstetrics and gynaecology (O&G) and fertility in Wales.A retrospective electronic survey study.All public hospitals in Wales that used virtual methods for delivery of fertility and/or O&G appointments during and after the COVID‐19 pandemic between August 2020 and March 2022.Patients awaiting an appointment on the National Health Service, who have attended virtual fertility or O&G appointments in Wales, and fertility and O&G clinicians who have conducted appointments virtually.Analysis of patient and clinician responses to a VC satisfaction survey delivered after their consultation taking place between August 2020 and March 2022.Patient and clinician satisfaction rates with the use of virtual consultation in fertility and O&G appointments.In satisfaction data collected from 420 patients and 161 clinicians, 83% of patients and 63% of clinicians reported their VC quality to be ‘very good’ or ‘excellent’. Difficulties when using VC were experienced by a minority of patients (1%–9%) and clinicians (1%–8%) and most patients (52v82%) and clinicians (34%–67%) found several aspects of VC to be ‘very beneficial’. Fertility patients rated their VC experience more positively than O&G patients.Most patients and clinicians were satisfied with their VC experience. Patients were more satisfied with the use of VC than clinicians. Hasselbacher","doi":"10.1002/rfc2.66","DOIUrl":"https://doi.org/10.1002/rfc2.66","url":null,"abstract":"To understand patient preferences around early abortion methods and care‐seeking during the coronavirus disease 2019 pandemic in a state that expanded insurance coverage for abortion care.We conducted phone interviews between July 2021 and February 2022 with 50 Illinois residents (aged 18–45) who had recently sought abortion at 18 clinics throughout Illinois at or before 11 weeks of pregnancy. We used a semistructured interview guide to explore preferences, motivations and decision‐making around the abortion method. We coded transcripts and used code summaries to conduct a thematic content analysis.Half of the participants chose medication abortion, and half chose procedural abortion. Some participants relied upon their past abortion experiences to choose a method. Participants' reasons for choosing medication abortion included home setting with support persons, the noninvasive nature, desiring a more ‘natural’ experience or negative perceptions of procedural abortions. Participants choosing procedural abortions valued increased certainty of completion, the option of sedation and the defined timeline of a clinic visit with fewer physical side effects. Some participants without insurance coverage were motivated to select a method based on cost. Around half of the participants expressed interest in considering a telehealth abortion.Patients cited complex and personal preferences influencing their method selection; when cost barriers were reduced, preferences centred physical or emotional experiences, setting, effectiveness and timing. Observational, population‐based studies in the United States, Canada, United Kingdom, France, Germany, Spain, and Italy, published in English between 2012 and 2022 were considered for inclusion. Estimates by country were reported and stratified by gestational age, birth plurality, and race/ethnicity, data permitting.Ten publications and nine grey literature reports were included. Epidemiologic estimates of PTL were reported for the United Kingdom and France: PTL was diagnosed in 2.2% of pregnancies and preceded 50% of PTBs. PTB rates were reported for the United States, Canada, United Kingdom, France, and Spain. Among live births in these countries, annual PTB incidence ranged from 5.9% (Spain, 2020) to 10.2% (United States, 2019). Most countries reported the PTB incidence by gestational age; reports by birth plurality or race/ethnicity were scarce. PTB rates for Germany or Italy were not identified.While PTBs were well‐reported overall and by gestational age, how rates varied by plurality, race/ethnicity and etiology is unclear. Epidemiologic estimates for PTL, a leading cause of PTB, were rarely reported in the literature. Peak intrauterine pressure was measured during a Valsalva maneuver in the first and second stages of labor, both during a contraction and at rest and compared between obese and nonobese patients. Pushing pressure, defined as the mean of the highest three out of five peak pressures attained minus the intrauterine resting pressure were also compared for the two groups.A total of 115 patients were enrolled: 50 (43.5%) obese and 65 (56.5%) nonobese. We found pushing pressures were significantly less in obese patients both with risk difference (RD) = 13.71 (95% confidence interval [CI]: 5.53, 21.90) and without RD = 12.19 (95% CI: 4.46, 19.91) a uterine contraction compared to the nonobese patients in the second stage of labor. This effect persisted after controlling for confounding variables.Obese patients with BMI ≥ 35 kg/m2 generated lower intrauterine pressures during pushing than compared to nonobese (BMI < 35 kg/m2). Physicians and participants provided feedback on their insertion experience.Successful placement occurred in 48 of 50 (96.0%) enroled participants. Investigators rated insertion device loading “easy” in 49 (98.0%) and placement “easy” in 44 (88.0%) cases. Receiving adequate prenatal care is linked to a greater likelihood of achieving desired permanent contraception, and access to such care may be critical for women with Medicaid insurance given that the federally mandated Medicaid sterilization consent form must be signed at least 30 days before the procedure. We examined whether adequacy of prenatal care mediates the relationship between neighbourhood socioeconomic position and postpartum permanent contraception fulfilment, and examined moderation of relationships by insurance type. Methods This secondary analysis of a retrospective cohort study examined 3012 Medicaid or privately insured individuals whose contraceptive plan at postpartum discharge was permanent contraception. Path analysis estimated relationships between neighbourhood socioeconomic position (economic hardship and inequality, financial strength and educational attainment) and permanent contraception fulfilment by hospital discharge, directly and indirectly through adequacy of prenatal care. Multigroup testing examined moderation by insurance type. Results After adjusting for age, parity, weeks of gestation at delivery, mode of delivery, race, ethnicity, marital status and body mass index, having adequate prenatal care predicted achieving desired sterilization at discharge ( β = 0.065, 95% confidence interval [CI]: 0.011, 0.117). Living in neighbourhoods with less economic hardship (indirect effect −0.007, 95% CI: −0.015, −0.001), less financial strength (indirect effect −0.016, 95% CI: −0.030, −0.002) and greater educational attainment (indirect effect 0.012, 95% CI: 0.002, 0.023) predicted adequate prenatal care, in turn predicting achievement of permanent contraception by discharge. Insurance status conditioned some of these relationships. Conclusion Contact with the healthcare system via prenatal care may be a mechanism by which neighbourhood socioeconomic disadvantage affects permanent contraception fulfilment, particularly for patients with Medicaid. Methods This was a retrospective observational study. The study population consisted of women who gave birth to term infants (&gt;37 weeks of gestation). Participants were given the Dietary Screener Questionnaire (DSQ) after birth and asked to recall their dietary intake in the last month of pregnancy. Participants' estimated dietary intakes were then compared to the 2020–2025 DGA which includes specific recommendations for pregnant women. Results Out of 51 women who completed the DSQ, none consumed the recommended amounts of all surveyed dietary factors. Specifically, only one woman (2%) met the recommended intake of fruits, 11 women (22%) met the recommended intake of calcium, 25 women (49%) exceeded the recommended upper limit for added sugar intake, and none of the women (0%) met the intake of vegetables, whole grains, dairy and fiber. Conclusion Women in our study did not adhere to the pregnancy‐specific DGA recommendations in the last month of pregnancy. This relationship has not been rigorously evaluated in Tanzania. We performed secondary analyses of data collected during the perinatal study, a double‐blind trial that investigated the effect of vitamin supplements on perinatal outcomes in Tanzania between 2001 and 2004. In this analysis, we used log‐binomial models to investigate the adequacy of vitamin B12 and folate intake among 7633 mothers and the association of vitamin B12 and folate intake during pregnancy with low birth weight, preterm birth (PTB), small for gestational age and foetal death. Sixty‐seven percent of women had vitamin B12 intake below the recommended dietary allowances (RDAs) of 2.6 mcg for pregnant women, and 98% of women had folate intake below the RDA of 600 mcg for pregnant women. Compared with women in the lowest tertile of vitamin B12 intake women in the highest tertile were 26% less likely to have PTB (relative risk [RR] = 0.74, 95% confidence interval [CI]: 0.65, 0.84) and 36% less likely to have severe PTB (RR = 0.64, 95% CI: 0.50, 0.83). Compared with women in the lowest tertile of folate intake women in the highest tertile were 21% less likely to have PTB (RR = 0.79, 95% CI: 0.69, 0.89) and 21% less likely to have severe PTB (RR = 0.79, 95% CI: 0.69, 0.89). The objective of this report is to describe a case of a woman who presented with acute watery diarrhoea during menstruation for the last 3 years. Outcome A case report has been compiled of a patient who visited the emergency room of the hospital on May 20, 2023, complaining of severe watery diarrhoea. An increased frequency of Gastrointestinal symptoms during menses has also been reported in women, but specifically acute watery diarrhoea symptoms during menses for the last 3 years is quite an unusual case. This study explores the possibility of a relationship between hormonal changes with increased prostaglandin levels and menstrual diarrhoea. Besides norethisterone acetate was utilized by the patient due to irregular menstruation, which might have an influence on this gastrointestinal problem but more studies are needed to determine its effect. Stunted growth and suboptimal weight gain remain major public health concerns during the most critical stages of growth and early childhood development. This study aims to assess the prevalence of stunting among children under 5 years of age and identify its associated factors in remote district of Nepal. Methods We conducted a survey with 487 mothers of children under 5 years of age in a remote area of Nepal. Smartphones were installed with KoBocollect software to collect the data. Trained health researchers collected the data including the anthropometric measurement (height, weight and mid‐upper arm circumference, MUAC) for assessment of the nutritional status. We followed World Health Organization guidelines for the anthropometric measurement and calculated the Z ‐score using Emergency Nutrition (ENA) software. Regarding the questions related to dietary diversity, we used a validated questionnaire: household dietary diversity score, child dietary diversity score and women dietary diversity score. We analysed all the data from the survey using SPSS version 24. We performed descriptive and multivariable analyses to find the nutritional status and factors associated with stunting. Result We found a high prevalence of stunting, that is, 56.7% among children under 5 years of age. Sociodemographic variables like age, sex of the child and ethnicity were significantly associated with the prevalence of stunting. Male children (60.7%) were more likely to be stunted compared to females (51.8%). We found that children from the Brahmin/Chhetri ethnic group were more likely to be stunted compared to children from Dalit and Janajati. Mothers who delivered their youngest child by skilled health personnel were less likely to have their children to be stunted. Similarly, children aged 24–59 months were 2.5 times more likely to be stunted than those aged 6–23 months. Conclusion Stunting among children under 5 years of age is very high in the remote district of Nepal. Sex, age of the child, ethnicity and delivery by a health professional were associated with the prevalence of stunting. There is a need for awareness and public health intervention to reduce the prevalence of stunting and improve the nutritional status of children under 5 years of age in remote districts of Nepal.
