Here we present a case of a 17-year-old female patient, who presented with symptoms of pansinusitis with other symptoms gradually following. Hypercalcemia and renal affection are rarely initial presenting features, and in the absence of pulmonary symptoms, the diagnosis of sarcoidosis in those patients could be a diagnostic challenge. A case-patient with sarcoidosis presented with elevated serum calcium and creatinine levels. Renal biopsy showed nephrocalcinosis with chronic fibrosing interstitial nephritis. The extensive mediastinal, abdominal, axillar, and neck lymphadenopathy was presented on the computer tomography scan of the patient's chest and abdomen. The neck lymph node surgical biopsy showed confluent, non-caseating, epithelioid granulomas consistent with sarcoidosis. The serum angiotensin-converting enzyme level was elevated. Treatment with oral prednisolone was started, and improvement of renal function and normalization of serum calcium was noted. Sarcoidosis should be considered in the differential diagnoses in patients with renal impairment and non-parathyroid hormone (non-PTH) dependent hypercalcemia.</p>","PeriodicalId":74492,"journal":{"name":"Prilozi (Makedonska akademija na naukite i umetnostite. It is a phenomenon as old as humanity itself, but its complexity continues to confuse and fascinate us alike. Whether it is the tension that boils in a heated argument or the explosive violence of warfare, aggression permeates our lives in subtle and overt ways. This paper analyses forms of aggression, some theories that explain aggression in humans, especially in youth, but the most emphasis is placed on neurobiological changes in brain structures associated with aggressiveness. Special attention has been paid to parenting as a factor in the occurrence/overcoming of aggression in children. The aim of this article is to summarize the main known methods for measuring the emotions/feelings published in Medline basis. {"title":"Health-Related Habits and Nutrition of Pregnant Women in Serbia.","authors":"Jovana Plešinac, Vera Plešinac, Ivana Babović, Miloš Radičević, Snežana Plešinac","doi":"10.2478/prilozi-2025-0003","DOIUrl":"https://doi.org/10.2478/prilozi-2025-0003","url":null,"abstract":"<p><p><b>Goals</b>: The aim of this paper was to do a demographic analysis of the population of pregnant women in Serbia and to summarize data regarding: their health related habits during pandemic years, pregnancy course, and the impact the COVID-19 pandemic on their mental health. <b>Methods</b>: The study was prospective observational non-randomized study in the public health. A group of 1,019 patients were included in this prospective cross-sectional observational study. The patients were assessed using a questionnaire designed by the International Federation of Gynecology and Obstetrics which was implemented cross-country in primary healthcare institutions during regular pregnancy visits. Data was collected during the first pandemic year in 2020. The IMB SPPS 27 program was used for descriptive statistical analysis of the collected data. <b>Results</b>: Out of the respondents 10.6% had elevated blood pressure. The mean fasting level of glucose was within the suggested limit. Almost all pregnant women were not on any special diet, and the most common special nutritional regime was a diabetic diet. Most respondents were non-smokers and most of them practiced some kind of recreation. Almost all pregnant women adhered to protective measures during the pandemic and more than half of them felt increased stress due to the situation. <b>Conclusion</b>: Our research suggests that despite the increased psychological pressure and restrictive measures which took place during the first pandemic year the Serbian population of pregnant women managed to follow majority of the health recommendations, including the protective measures from the COVID-19 virus.</p>","PeriodicalId":74492,"journal":{"name":"Prilozi (Makedonska akademija na naukite i umetnostite. APS can occur alone or in association with other diseases, more commonly systemic lupus erythematous. In patients with both underlying diseases episodes of arthritis, skin changes in the form of livedo reticularis, thrombocytopenia and leucopenia were more common. Cardiac manifestations have also been reported. The aim of this paper is to examine the treatment of attrition in everyday dental clinical practice with the help of soft inserts.</p><p><strong>Material and method: </strong>In this study, 30 patients were included, divided into two groups of 15 patients, the study and the control group. The examined patients had first degree of attrition diagnosed according to the Bardsley index, after which the condition was noted in four intervals: the initial situation, after 2, 4 and 6 months from the first visit. The research data were processed in Statistica for Windows 7.0 and SPSS version 20, and the same were presented graphically.</p><p><strong>Results: </strong>In the study group with I degree of attrition, for p>0.05, no statistically significant difference was determined in the range of the Bardsley index between the four measurement times (Friedman Test: N=15; Chi-Square=7,200; df =3; p=0.0658). In the control group, for p<0.05, a significant difference was determined in the range of the Bardsley index between the four measured times (Friedman Test: N=15; Chi-Square=15.180; df=3; p=0.0017) with a significantly high value after 6 months.</p><p><strong>Discussion: </strong>From the results obtained when comparing the study group in patients with I degree of attrition and the control group, a negative change from the parafunction bruxism was observed in 20% of the test group and 47% of the control group. The results of our research indicated that in patients with the first degree of attrition from the study group, soft dental inserts give significantly better results than untreated patients in the control group. This correlates with the research of Khayat N. et al.</p><p><strong>Conclusions: </strong>From the results of the subjects in the group with I degree of attrition, we can conclude that the soft inserts has great efficiency as a means of management of this parafunction and therefore its application in daily therapy is recommended.</p>","PeriodicalId":74492,"journal":{"name":"Prilozi (Makedonska akademija na naukite i umetnostite. Symptoms include inspiratory stridor, barking cough, and hoarseness due to inflamed vocal cords. Diagnosis is based on symptoms difficulty breathing, stridor, chest in-drawing voice changes and head nodding. Severity ranges from mild to severe cases. This study aimed to explore variations in croup and factors contributing to its recurrence in children. This retrospective longitudinal study analyzed health records from the Pediatric Practice \"Vitalino\" in Veles, North Macedonia, from 2016 to 2022. It included children aged 0-5 years who had croup at least once. The sample was substantial and representative, allowing for a thorough examination of potential correlations and trends in croup occurrence. Among 707 studied children, 254 had croup, with a prevalence of 35.9% over seven years or 5.1% per year. Females had lower odds of recurrent croup, while males had higher odds, but the differences were not statistically significant. Atopic dermatitis and bronchiolitis exposure were significantly associated with croup in both genders. The study provides insights into the prevalence, distribution of croup and associated conditions in young children. The significant associations of atopic dermatitis and bronchiolitis suggest a link between these conditions and recurrent croup. Considering these strong associations, we can conclude that bronchiolitis and croup should be included in the trajectory of the atopic march.</p>","PeriodicalId":74492,"journal":{"name":"Prilozi (Makedonska akademija na naukite i umetnostite. This position paper of Scientific societies of endocrinologists, nephrologists and cardiologists has been consensually brought at a couple of mutual meetings, aiming to synthesize current knowledge on screening, diagnosis and staging of DKD, emphasizing the need for an early detection and intervention in order to prevent progression to end-stage renal disease (ESRD). The role of glycemic control, blood pressure management, lipid management and the use of reno and cardioprotective agents, including angiotensin-converting enzyme inhibitors, sodium-glucose co-transporter 2 inhibitors and non-steroidal mineralocorticisteroid receptor antagonist has been entirely considered. Furthermore, we highlight the importance of a multidisciplinary approach in the care of patients with DKD, integrating lifestyle modifications and patient education into the clinical practice. This paper advocates for the implementation of standardized screening protocols and the development of personalized treatment strategies to optimize patient outcomes. {"title":"Implementing an Antimicrobial Stewardship Program and Complete Screening in an Intensive Care Unit in Relation to the Overall Clinical Outcome of Patients.","authors":"Jelena Micik, Aleksandar Dimovski, Zoran Sterjev, Ljubica Shuturkova, Aleksandra Grozdanova","doi":"10.2478/prilozi-2024-0019","DOIUrl":"https://doi.org/10.2478/prilozi-2024-0019","url":null,"abstract":"<p><strong>Background: </strong>The high prevalence of resistant microorganisms indicates a multidisciplinary approach, which will ensure efficiency and balance between resistance therapy of choice and the implementation of an antimicrobial stewardship (AMS) program in intensive unit care (ICU).</p><p><strong>Objectives: </strong>The objectives are based on the assessment of AMS in a cardiac surgical intensive care unit (CICU), by determination with the reduction of: inotropic support, length of antibiotic treatment, and need for renal replacement therapy (RRT).</p><p><strong>Material and methods: </strong>The research is a retrospective, group comparative, analytical cross-sectional study, in the period from 2020-2023, within the Cardiosurgery department, at Acibadem Sistina. 1277 patients participated in the research, divided into two groups: group 1 (2020-2021) and group 2 (2022-2023). Primary endpoints investigated: inotropic support, need for RRT and length of antibiotic treatment. Secondary endpoints investigated: optimization of antibiotic therapeutic regimen and clinical outcome assessment of patients (survival).</p><p><strong>Results: </strong>The inotropic support during 24, 48 and 72 h was statistically significantly lower in group 2. A statistically significant shorter length of time for antibiotic treatment was determined in patients in group 2 (p=0.000), as well as a lower value of the need for RRT. A statistically significant difference in time to event (fatal outcome) was determined between the two groups (p=0.000). A significant difference (p=0.000) was determined in the prescription and optimization of the therapeutic regimen.</p><p><strong>Conclusion: </strong>Integrating AMS, initial comprehensive microbiological screening and application of biomarkers in the CICU, established appropriately, will result in improved overall clinical outcome for patients.</p>","PeriodicalId":74492,"journal":{"name":"Prilozi (Makedonska akademija na naukite i umetnostite.
