The aim of this paper is to present a broad overview of the present situation of education, practice and regulation of Chinese medicine in Brazil. This is a descriptive study based on the revision of present literature and legal documents, and the exchange of information with experienced people related to Chinese medicine practice. It was conducted in order to find out and describe the most reliable and updated data. Based on the present study it is possible to demonstrate that the practice of acupuncture is very heterogeneous with no specific official regulation, so it can be practiced by anyone with proper education, based on the Federal Constitution of Brazil. While it has taken thousands of lives, there are no vaccines, antibodies nor anti-viral drugs at present. Confronted by massive difficulties, researchers have still gained great success in the prevention and treatment of COVID-19 via vigorously using the integrated therapy of traditional Chinese medicine (TCM) and western-style medicine. To describe and discuss the cognitions of SARS-CoV-2, pathogenesis of COVID-19, mainly around ACE2, and application TCM in the prevention and treatment of COVID-19 to provide clinical references of this disease. An overview of pathogenesis of SARS and COVID-19 and TCM may be used in the treatment of SARS and COVID-19 is presented. Narrative overview of the literature synthesizing the findings of literature retrieved from searches of computerized databases. Characteristics of TCM are the organic conception of the human body and discriminate and dialectical demonstration. It is worth noting the application of TCM in the prevention and treatment of COVID-19 to provide clinical references of this disease. This review focuses on the elaborate analysis of Valeriana jatamansi Jones regarding botanical characteristics, phytochemistry and pharmacology. Valepotriates and flavonoids are the major bioactive compounds mainly isolated from the roots/rhizomes. Both the major chemical constituents and different extracts of Valeriana jatamansi Jones are shown to possess the neuroprotective, antidepressant, anti-tumor, gastrointestinal, cytotoxic, anti-virus, antioxidant, sedative and other activities. This review could be helpful to provide a strong bolster for a promising natural resource of drugs for the treatment of various diseases and a long term development in exploring highlighted area. Crude plant of Valeriana jatamansi Jones contains various compounds and a wide range of pharmacological activities. However, further studies are warranted to illuminate the mechanisms responsible for these activities at the molecular levels. Chuthaputti","doi":"10.21037/LCM-20-36","DOIUrl":"https://doi.org/10.21037/LCM-20-36","url":null,"abstract":": Even though traditional Chinese medicine (TCM) have been in Thailand for several hundred years, the role of TCM in the public health service system has just started about 25 years ago after the establishment of the Collaborating Center of Thai-Chinese Medicine in the Ministry of Public Health in 1995, and the Department for Development of Thai Traditional and Alternative Medicine in 2002. Initially, TCM practitioners in Thailand learned and gained experience in TCM from their ancestors or from universities abroad, before the start of the five-year Bachelor’s degree program in Thailand in 2004 and the development of the standard criteria for the accreditation of TCM educational institutions thereafter. Currently, there are a total of 9 universities with accredited TCM faculties and colleges producing about 200 graduates each year. In addition, there is also a three-month acupuncture and moxibustion training course for medical doctors. Under the Practice of the Art of Healing Act (No. 4) B.E. 2556 [2013], the Profession Commission in the Branch of Traditional Chinese Medicine, among various duties and responsibilities, is in charge of organizing annual TCM knowledge test and the issuance of TCM practitioner license. Regarding the regulation of Chinese medicinal products, their production, import, sale and advertisement were regulated under the Drug Act, B.E. 2510 [1967] until 2019 when the Herbal Product Act, B.E. 2562 [2019] has come into force and TCM products become a type of herbal products under this new Act. Presently, acupuncture is the only TCM treatment modality officially available in some public health service facilities, while a wide range of TCM therapies are available in TCM clinics. The number of patients who come to receive TCM treatment in public hospitals and private clinics has continuously increased each year. This article systematically reviews the available literature about TCM on fu xie from pre-Qin period to now and summarized information about fu xie in terms of its categories and insidious regions, pathogenic features, etiologies and clinical manifestations, and therapeutic methods. Treatment strategies of fu xie may include protecting yin-fluid, unblocking moyuan, opening onto the pleurodiaphragmatic interspace, and adopting the combination of extermination with appeasement. Notably, the combination of extermination with appeasement developed by the author (Tiansong Zhang) involves how to infer the hiding place of a pathogen and how to eliminate the pathogen with appropriate treatment. Modern advances in the doctrine of fu xie reflect the academic thought of TCM, which originates from clinical and discursive thinking and is highly enlightening. The fu xie theory has evolved with the advances in the doctrine of febrile disease and it has high theoretical and practical significance in immune-related diseases, involving infections, autoimmunity, allergic reactions, immunodeficiency, tumors, skin diseases, renal diseases, heart diseases, and spleen/stomach diseases. Vitamin D (or calciferol) is the pivotal regulator of calcium in humans and low levels are associated with bone health in children and adults. Recent research though has spread in several other fields including common urological conditions. Early reports indicate that vitamin D deficiency might be associated with increased risk of prostate cancer and hyperplasia, stone disease, semen impairment and urinary tract infections. Methods: A non-systematic search in PubMed/Medline, Google Scholar, Web of Science, and Embase was performed using the terms “vitamin D” and “benign prostatic hyperplasia/enlargement”, “prostate cancer”, “prostate malignancy”, “urolithiasis”, “nephrolithiasis”, “male infertility”, “urinary tract infections”. The search duration period was set between 2011 and 2021. Exclusion criteria were non-English and retracted studies. Key Contents and Findings: In prostate cancer, vitamin D deficiency is associated with adverse outcomes, but its usage needs to be clarified. In hyperplasia, correlations are not certain. In contrast, in urolithiasis, the achievement of a normocalciemic status should be considered the primary goal in terms of prevention and reduction of recurrence. Moreover, vitamin D seems to regulate host immunity. In children, pregnant and women of reproductive age, screening for deficiency might be of benefit, whereas supplementation may be used as a secondary tool in the management of urinary tract infections. In infertile men, treating deficiency might be a cost-effective approach instead of a multi-panel empirical antioxidant treatment but its role needs further evaluation. Conclusions: Although highest level of evidence still lacking to support vitamin D administration as per guidelines practice, last decade research looks promising in terms of its role and its therapeutic potential. Background: Precision treatment is the core idea of Western medicine. According to the difference in diseases, physiology, physiques, and genes, doctors will apply different treatment protocols to achieve the best results. Pattern classification is a major methodological means of dialective treatment in Traditional Chinese medicine (TCM) and has a relationship with precision treatment according to the methods applied in the treatment. However, how to explain the material foundation for Traditional Chinese medicine remains scant. Methods: From the perspective of Traditional Chinese medicine, the treatment method given to patients with even a same disease is different, thereby achieving the best results. This article started from the cold pattern, hot pattern, deficiency pattern, blood stasis pattern, with modern agencies and drug metabolism, to explain the material foundation of pattern classification and treatment of drugs, and the consistency of the precision treatment concept both in traditional Chinese medicine and modern medicine.
