{"title":"Decoding cancer and herbal renaissance.","authors":"Lan Ko","doi":"10.21037/lcm-22-12","DOIUrl":"https://doi.org/10.21037/lcm-22-12","url":null,"abstract":"<p><p>The original notion in quest of cancer targets to end cancer still stands, yet the secret of common human cancer was concealed by a chicken-egg paradox. Solid tumors initiate in the tumor microenvironment from rare stem cells, which express a mutant target protein as their specific marker. For decades, the stem cell and target protein cannot paradoxically be found one without first finding the other. With combined evidence from genetics, pathology, stem cell biology, clinical oncology, and herbal medicine in particular, this paradox is resolved. Historical successful anticancer herbs, together with clinical oncology drugs, paved the way to decode cancer. In solid tumors, the liable stem cells are pericyte stem cells on blood vessels in the tumor microenvironment inducing angiogenesis. One identified target protein in pericytes is a DNA repair factor and transcriptional regulator named GT198 (gene symbol <i>PSMC3IP</i>, alias name Hop2). Since GT198 is found as a direct drug target of many chemotherapy drugs and clinically successful anticancer herbs, more herbal medicines worldwide can now be screened against this target. In the near future, safer and more effective natural herbal medicines could systematically treat common solid tumors. This review discusses a unified theory of cancer and diseases in which pericyte stem cells are fundamental to both. It also reveals a new approach to identifying multi-functional herbs. Various components in Camellia nitidissima Chi have been identified and some of its health-beneficial functions have been demonstrated. The review aims to supply an overview of the main chemical components and health-beneficial functions of Camellia nitidissima Chi. Methods: The references in this review are mainly collected from published books and the scientific literature databases including Web of science, PubMed, and China National Knowledge Infrastructure, with a timeframe from January 1986 to March 2022, containing English and Chinese references. Key Content and Findings: Chemical analysis reveals that Camellia nitidissima Chi contains a variety of chemical constituents, including phenolic compounds, saponins, polysaccharides, and other substances. Phenolic compounds, particularly flavonoids, are the most well-studied components in Camellia nitidissima Chi. Many of them are bioactive and contribute to the health-beneficial functions of Camellia nitidissima Chi. Plenty of studies confirm that Camellia nitidissima Chi have effects on antioxidant, anti-cancer, anti-hyperglycemia, anti-hyperlipidemia, anti-allergy, and anti-depression. Part of the underlying mechanisms are unveiled. Conclusions: Based on current research progress, Camellia nitidissima Chi has the potential to be applied in dietary supplements and even medications. Natural products from botanicals could be beneficial in the prevention or treatment of a variety of cancers. Saffron (the extract of <i>Crocus sativus</i>) includes its major constituent, crocin, have been used as a folk medicine for a long time, and they have shown obvious cancer chemoprevention potential. The objectives of this review are to present the progress of research on the effects of saffron and crocin in cancer management and the underlying mechanisms of action.</p><p><strong>Methods: </strong>We searched publications in the English language, published between January 1, 1980 and September 30, 2022, of saffron and crocin on cancer through several search engines, i.e., PubMed, SciFinder and Web of Science.</p><p><strong>Key content and findings: </strong>In this article, we first summarize the phytochemical studies of this botanical. Then, we present the anticancer effects of saffron and crocin on different human cancer cells. Saffron and crocin showed obvious antiproliferative effects on human cancer cell lines, including colorectal cancer, breast cancer, lung cancer, prostate cancer, cervical cancer, leukemia, glioblastoma and rhabdomyosarcoma. Finally, the anticancer-related mechanisms of action are summarized, including cell cycle arrest at G0/G1 or G2/M phases, induction of caspase-dependent apoptosis, signaling pathway-linked tumor metabolism regulation, and adaptive immunity regulated inflammation of host immune responses.</p><p><strong>Conclusions: </strong>Previous studies related to saffron and crocin in cancer management have been reviewed and reported results have been analyzed. Clinical data suggest that saffron and crocin have beneficial effects to improve quality of life in cancer patients.
