{"title":"Cecal and ilial intubation rates in colonoscopy: Comparative study.","authors":"Osamah Tahir Muslim, Hasan Osamah Al-Obaidi","doi":"10.47750/jptcp.2021.847","DOIUrl":"https://doi.org/10.47750/jptcp.2021.847","url":null,"abstract":"<p><strong>Background: </strong>Colonoscopy is the procedure of choice for both the diagnosis and treatment of large intestine and distal ilium in patients complaining of bowel symptoms, anemia resulting from malabsorption, radiographic colon abnormalities, screening for colorectal carcinoma, after polypectomy and cancer resection surveillance, ulcerative colitis surveillance, and those with suspicion of neoplastic masses. Inspection of the whole colonic and distal portion of terminal ilial mucosa is usually feasible during colonoscopy. Quality examination of the large bowel includes intubation of the complete colon and mucosal visualization. The investigators demonstrate that terminal ilium intubation is possible in endoscopy practice and yields additional clinical details. Furthermore, it may be used as an indicator of colonoscopy completion.</p><p><strong>Objectives: </strong>This study estimated the rate of cecal and ilial intubation by a single well-trained endoscopist and compared it with the results of a heterogeneous group of endoscopists.</p><p><strong>Patients and methods: </strong>This retrospective comparative study estimates the rate of cecal and ilial intubation in a private endoscopy center in which all the endoscopic procedures were conducted by a single consultant gastroenterologist, and compared it with the rates of a governmental center with by five colonoscopy endoscopists (general surgeons, general physicians, trained endoscopists, and gastroenterologists). The study population included 442 patients (245 males [55.42%] and 197 females [44.58%], ranging from 14 to 85 years of age.</p><p><strong>Results: </strong>Overall cecal and ilial intubation rates were 88% and 47.5%, respectively. The adjusted rates for cecal and ilial intubations were 94.2% and 50.8%, respectively, after considering cases of anatomic colonic obstruction and when the clinical indications do not justify total colonic intubation. These figures were superior in comparison to the results of a multi-operator study in which the cecal- and the ilial intubation rates were 51.81% and 30.69%, respectively.</p><p><strong>Conclusion: </strong>Cecal and ilial intubation are important quality indicators for colonoscopy, and in this study, they were found to be superior in qualified gastroenterologists than in general surgeons and physicians. Although many theories have been implicated for keloidogenesis, the precise pathophysiological cause is still masked. Different treatment strategies have been tried in their management, but there is no satisfactory option for treating hypertrophic scar currently; moreover the standard steroid therapy is associated with numerous local side effects, and there is a need for researches in new treatment options. The aim of this study is to evaluate the role of topical isoxsuprine in experimentally induced hypertrophic scar in rabbits. In the current experimental study, 40 healthy male albino rabbits between 12 and 14 months of age were studied. These rabbits were categorized into five groups: healthy animal group (<i>n</i> = 8), hypertrophic scar without treatment (n = 8), hypertrophic scar treated with triamcinolone acetonide gel (<i>n</i> = 8), and hypertrophic scar treated with isoxsuprine gel (<i>n</i> = 8). Histological assessment of skin biopsy, including the conventional hematoxylin and eosin stain, and immunohistochemistry for transforming the growth factor beta 1 level (TGF-β1) and collagen 3 alpha1 (COLIIIαI) in skin tissue was done. The immunohistochemical score of TGF-β and collagen III was highest in group 2 (hypertrophic scar without treatment), followed by group 3 (hypertrophic scar treated with triamcinolone acetonide gel) and group 4 (hypertrophic scar treated with isoxsuprine gel) - no significant difference between them since <i>p</i> > 0.05, and then by group 1 (healthy control group). Regarding histopathological scores of inflammation, the scar height, and scar index, the scores were highest in was highest in group 2 (hypertrophic scar without treatment), followed by group 3 (hypertrophic scar treated with triamcinolone acetonide gel) and group 4 (hypertrophic scar treated with isoxsuprine gel) - no significant difference between them since <i>p</i> > 0.05, with the exception of index of scar, and then by group 1 (healthy control group). All participants had completed a three-part questionnaire related to geriatric dentistry: Part I (knowledge), Part II (cognitive evaluation), and Part III (awareness and attitude of psychosocial and health problems). Comparisons were made between the groups, and the data were analyzed using SPSS software.</p><p><strong>Results: </strong>The responses on the Part I were not statistically significant among the groups (P > 0.05). The knowledge mean scores comparison showed an evident significant relationship among the groups (P < 0.05). Overall the Group II participants achieved the highest scores for all the Parts (all P > 0.05).</p><p><strong>Conclusions: </strong>The students belong to Group II, and the interns achieved higher scores than the Group I students. Reports indicate that psoriasis is affecting about 2% of the world population and life quality in a way that limits the patient activity and thereby productivity in a community. Therefore, determining appropriate therapeutics for psoriasis would make a difference in the lives of the patients and their communities. To address this, scientists have been working on different biological agents, such as interleukins and their antagonists, to regulate the common inflammatory process in psoriasis. For the current research project, serum was collected from 26 psoriatic patients and another 26 health controls, and the serums were examined for the level of IL-30 to determine differences among the tested groups and to investigate a possible correlation between IL-30 and psoriasis area severity index (PASI). Current clinical and endoscopic indices only measure disease activity at a specific time point. The recently published Lémann index (LI) is the first tool that aims to measure cumulative structural bowel damage and thus evaluates long-term disability.</p><p><strong>Aim of study: </strong>To measure the LI at initial presentation of CD in a sample of Iraqi patients attending the gastroenterology and hepatology teaching hospital (Baghdad).</p><p><strong>Patients and methods: </strong>This is a descriptive single-center study conducted in gastroenterology and hepatology teaching hospital and enrolled 30 patients with CD diagnosed from 2013 to 2015. Apart from limiting access to health care services, the attitudes, beliefs, comfort level of the treating doctors towards the geriatric patients can motivate or demotivate them to access dental care.</p><p><strong>Aim: </strong>To explore the Saudi Arabian undergraduate students perception of geriatric patients and identify potential barriers that prevent the utilization of their dental appointment.</p><p><strong>Methods: </strong>A close-ended questionnaire with one question and eight reasons was administered to the fifth year clinical students. The students were requested to specify their agreement with each question on a 5-point Likert scale. Among the barriers presented, each reason's approval was expressed as the percentage of the total number of responses. In addition, the gender comparison of mean scores was made, and an independent sample t-test was used to analyze the statements agreed by the students. All analyses were performed using Statistical Package of Social Sciences (SPSS) version 21.0 (IBM, USA) with the probability of statistical significance at 0.05 level.</p><p><strong>Results: </strong>Fifty-one students recorded their perceptions on the questionnaire administered during their clinical posting in the fifth year of the geriatric dental education program. It was concluded that students believed that geriatric patients give overwhelming importance to other problems with minor importance to oral health care. In addition, gender comparison was more evident as the percentage expressed was more in females.</p><p><strong>Conclusions: </strong>There is a need for more clinical exposure of geriatric patients during their clinical postings. Unfortunately, it is unlikely that this pandemic will disappear soon. No health system, with its present resources and workflow, is capable enough to deal with a full-blown wave of this pandemic. Acquisition of specific new skills may be fundamental in delivering appropriate health care for our patients. The gold standard for diagnosis of the COVID-19 infection is real-time reverse transcription polymerase chain reaction. Radiological investigations (chest X-ray or high-resolution computerized tomography [CT]) can be helpful both for diagnosis and management, but they have many limitations. Ultrasound has been suggested as a reliable and accurate tool for assessing the lungs in COVID-19 patients. Lung ultrasound (LUS) can show specific signs of interstitial pneumonia, which is characteristic of COVID-19 pulmonary infection. In addition, nonradiologist specialists with experience in ultrasound can be trained on LUS with a relatively rapid learning curve. In pregnancy, LUS can be particularly useful due to the avoidance of exposure to ionizing radiation. Near-future forecasts were also performed. Moreover, the causalities between the Stringency Index's indicators and total cases in E7 countries in COVID-19 period were examined. Countries were grouped as \"stationary,\" \"transition,\" and \"exponential\" based on the data and model fits. The proposed models produced good fits to the COVID-19 data of E7 countries and it was possible to predict the number of cases in the near future. {"title":"New-generation antiepileptic drugs during pregnancy and the risk of attention-deficit hyperactivity disorder: A scoping review.","authors":"Christine Vaccaro, Ahmad Shakeri, Emily Czaplinski, Sherif Eltonsy","doi":"10.15586/jptcp.v27i4.722","DOIUrl":"10.15586/jptcp.v27i4.722","url":null,"abstract":"<p><p>The use of maternal antiepileptic drug (AED) during pregnancy is associated with an increased risk of cognitive adverse effects among the offspring. As new-generation AEDs continue to enter the market, evidence on their safety during pregnancy is limited yet necessary. To date, there are no published reviews summarizing the evidence of new-generation AED exposure in utero and the development of attention deficit-hyperactivity disorder (ADHD) in the offspring. The objective of this scoping review is to summarize the available evidence on the risk of ADHD after maternal exposure to new-generation AEDs during pregnancy. We searched EMBASE and MEDLINE for articles published from January 1988 to April 2020. New-generation AEDs were considered if marketed after 1988. ADHD was defined as attention-deficit hyperactivity disorder, hyperkinetic disorder, hyperkinesis, or conduct disorder. Of the total articles screened (n = 805), eight publications were finally included (seven cohort studies and one systematic review). Across the studies, the sample size of pregnant women exposed to AEDs ranged from 1 to 1383. Monotherapy was examined in six studies (mostly lamotrigine), while only two studies examined polytherapy. The included studies reported a range of adjusted relative risks, from 0.84 [0.59-1.19] to 1.63 [0.41-6.06]. Lamotrigine monotherapy holds the largest body of evidence, concluding that no significant risk of ADHD exists among the offspring. However, the available evidence is considered scarce and has several methodological limitations. Disentangling the effect of AEDs from epilepsy itself and examining polytherapies are challenges that merit additional investigations. {"title":"The Role of Vegan Diets in Lipotoxicity-induced Beta-cell Dysfunction in Type-2-Diabetes: A Narrative Review.","authors":"M. Chronic insulin resistance and a progressive decline in beta-cell function are discussed as the root causes of type-2-diabetes. Both are associated with obesity and prolonged exposure to pathologically elevated concentrations of circulating free fatty acid (FFA) levels in the blood. Fatty acid derivatives may interfere with beta-cell function and ultimately lead to their death through lipoapoptosis. The harmful effects of chronically elevated FFA levels on glucose homeostasis and non-adipose tissues are generally referred to as lipotoxicity. Pancreatic beta-cells appear to be particularly vulnerable, and both dietary fat quantity and quality may impact their function. Diets high in saturated fat are especially harmful to beta-cells while (poly-)unsaturated fatty acids (PUFA) are associated with beta-cell protective effects. Therefore, this narrative review suggests that a dietary modification toward a low-fat vegan diet might help to prevent or reduce lipotoxicity-induced beta-cell dysfunction. By cutting the oversupply of saturated fat and reducing total calorie intake, and by improving both body weight and glycemic control, low-fat vegan diets may reduce the likelihood of lipotoxic events to occur. In light of the accumulating evidence that lipotoxic events are tightly coupled to excess glucose levels, improved glycemic parameters appear to be of utmost importance. These mechanisms are likely to contribute complementarily to improved beta-cell function in individuals with type-2-diabetes who choose a low-fat vegan diet.
