In this work, we have a case report of a patient with Carotid Artery Stenosis (CAS) (Stenosis ACC et ACI, left ACI stenosis over 80%,right ACI stenosis 70%, Atherosclerosis ACC et ACI, Insuff. Verthebrobasi-laris) solve Carotid Endarterectomy (CEA) under local anesthesia. Abstract Motivation and objectives: The diffusion of ultrasound machines with better resolutions and accessibility allowed its use in region-al blocks, making the blocking of the interfascial planes a common procedure. Forero et al. 2016 describes for the first time the Erector Spinae Plane Block, this type of blockade has been shown effective in thoracic and abdominal surgeries. The objective of this report is to describe the use of a catheter for continuous perioperative anes- thesia. Case report: Patient with multiple drug allergies, with previous mas- tectomy, submitted to breast reconstruction, with continuous erector spinae plane block guided by ultrasound. These occur from an accident, homicide or suicide. So, this study was conducted to analyse the clinical profile, type & level of injury, airway management, choice of anaesthesia technique in such injuries. Material and methods: This retrospective study included 19 cases of penetrating neck injury who were brought to emergency department and had received treatment. The demographic data, site, type, level & depth of injury, type of anesthesia, central venous cannulation, airway management, induction agent used, and any complications were recorded and analyzed. Results: Age range varied from 15 years to 50 years. There were 14(73.68%) males & 5(26.32%) female patients. Majority of penetrating neck injuries were homicidal 11(57.89%) followed by accidental 4(21.05%) and suicidal 4(21.05%). 15(79.05%) patients had zone II injuries. In 8(42.1%) cases of deep penetrating injury of neck, an emergency tracheostomy was done. Conclusion: We conclude that the majority of penetrating neck injuries was seen in younger age group with male predominance. {"title":"Analysis of an Anesthesia and Intensive Care Ventilators during Suctioning through a Fiber-optic Bronchoscope","authors":"P. Methods: In a lung model, a 5.9 mm OD FOB was inserted into an endotracheal tube through a tight-fitting connector. Pressure and flow sensors were placed at the airways opening (Pao) and distal to the bronchoscope at the trachea (Ptr). This model was tested under pressure and volume controlled mandatory ventilation and during Continuous Positive Airway Pressure (CPAP) delivered by both, anesthesia and intensive care ventilators. End-expiratory pressure was set at 10 cm H2O. Measurements were performed during suctioning with a mean negative flow of -12 ± 0.2 L/min at the tip of the FOB. Results: The anesthesia ventilator failed to keep end-expiratory pressure above zero at the trachea during FOB suctioning using both, continuous mandatory ventilation and CPAP. When using an intensive care ventilator, Ptr decreased below zero during FOB suctioning during volume controlled ventilation but remained slightly above zero under pressure controlled ventilation. When this ventilator delivered 10 cm H2O of CPAP, Pao remained constant while Ptr decreased by 2.5 cm H2O during suctioning. Conclusion: During FOB suctioning, targeted Ptr could neither be maintained anesthesia nor intensive care ventilators used in mandatory modes of ventilation.
