The aim of this review is to stress the huge interest of extracellular vesicles circulating into blood and other body fluids, as potential candidates for early diagnosis of gastric cancer. After briefly recalling the previous blood-based liquid biopsies as a non-invasive help to GC diagnosis and the main properties of extracellular vesicles, the review will be focused on the recent progressive emergence of exosomes as important partners in GC liquid biopsy. Faced with the always increasing number of exosomal biomarker candidates, the necessity of two independent searches is suggested. The biologists need to solve some pending biological problems about extracellular vesicles, and the eukaryotic microorganism Dictyostelium discoideum might be an appealing both \"in vitro and in vivo\" model for that purpose. Objective: To analyze prospectively a series of 300 cases of hemorrhoidal disease managed with transanal hemorrhoidal dearterialization guided by Doppler ultrasound (THD) 1.2. Material and Methods: During the months of May 2011 to June 2019 a total of 300 patients were included with an average age of 43 years, (3%) were diagnosed with hemorrhoidal disease grade II, grade III (15%), grade IV (10%) and mixed hemorrhoidal disease (72%), predominantly male (57%), which were treated with THD; surgical time, intensity of postoperative pain, recurrence, recovery time and time of return to work were analyzed. 1.3. Results: The average age was 43 years, with a predominance of 57% male, the predominant symptoms before surgery were 100% bleeding, 79% prolapse and 60% pruritus, 3% had Grade II hemorrhoids, 15% grade III, 10% Grade IV and 72% mixed hemorrhoids, the average surgical time of the surgery was 15.9min (range 15-20 min), 1 day hospital stay (100%), 2.3% presented urinary retention, in The VAS scale at day 1 patients presented a value of 4 (range 2-7), at day 7 a value of 2 and at day 30 a value of 0 in all patients, the recovery time on average was 10 days and return to work in 14 days (range 10-20 days), recurrence of 3%, 10 patients It is presented evidence of new external flaps. 1.4. However, due to limited funds and liver donors, liver transplantation is seldom performed in Asia, especially in China. For patients who have experienced oesophageal and gastric varices bleeding and waiting for a liver transplant, transjugular intrahepatic portosystemic shunt (TIPS) has proven to be an ideal treatment procedure. However, TIPS may lead to severe complications. At present, TIPS infections remain a rare complication, and the definition remains controversial. Specific pathogenesis has not been elucidated. In the literature, no pathogenic bacterium has been confirmed as a major pathogen of TIPS infection, and no infections have been attributed to the placement of TIPS. Whether or not such infections are all associated with stent implantation of TIPS remains a question that needs to be answered. Previously classified as Langerhans cell histiocytosis and non-Langerhans cell histiocytosis, and recently into 5 categories (L, C, R, M and H), it can be localized to an organ or have a systemic manifestation, affecting most commonly the bone, skin and lymph nodes. Gastrointestinal involvement is extremely rare especially for the non-Langerhans group. We report a rare case of a non-Langerhans cell histiocytosis affecting the rectum, manifesting as a large, nodular, ulcerated, continuous lesion found during a colonoscopy, in an elderly man with initial presentation of rectorrhagia and abdominal pain. Aim of the study: To determine the prevalence of Helicobacter pylori [H. pylori] infection in Type 2 diabetic Patients. Methods: A cross sectional study recruited three hundred patient two hundred out of them were type 2 diabetics from Benghazi diabetic center and one hundred healthy individuals or non-diabetic patients which served as control group was taken from Benghazi medical center and Elhiaa clinic . Their ages ranged between 25 to 70 years, from the period of 2015 -2019. All members of the study were investigated for serum H.pylori -IgG by Elisa, blood glucose levels and HbA1c and Parasitic tests were also done for all patients. Socio demographic characteristics were taken during the interview. Results: The results revealed that the overall prevalence of infection with Helicobacter pylori among Type2 diabetic patients were 144 [72.0%] with highly significant difference [p=0.000] compared to 49% of the non-diabetic group. Our results show that there was no statistical difference in H. pylori positivity according to gender smoking history and education level in diabetic and non-diabetic while presence of parasitic infestation and gastrointestinal symptoms in H. pylori positive patients were statistically different between diabetic and non-diabetic group being higher in diabetics. H. pylori was statistically more prevalent in diabetic with higher fasting plasma glucose and HbA1c. Recent studies suggest that incidence of covid-19 among this population is generally higher due to the fact of chronic immunosuppressant. However, there is a scarcity of literature describing clinical presentation, and outcomes in Liver Transplant (LT) patients with Covid-19. We aimed to describe the clinical characteristics in LT recipients diagnosed with Covid-19 who had undergone liver transplantation at the liver and kidney transplant center of Al Rassoul hospital in Lebanon. Methods: A retrospective case series study conducted among patients who previously had a liver transplantation at the kidney and liver transplant center of Al Rassoul hospital in Lebanon and diagnosed with Covid-19, before the era of vaccination. Demographic information, past medical history, clinical manifestations, severity of the disease and patient’s outcome were recorded. Results: During the study period, a total of 27 LT recipients were enrolled among whom 6 (22.2%) patients were tested positive for Covid-19. The majority of the infected patients were female (66.6%). Out of these patients, 83.3% had mild Covid-19. The most frequent reported symptoms were fever (66.7%). 50% of patients presented with GI symptoms. The rate of hospitalization was 50%, and no patient death was recorded. All infected patient was receiving tacrolimus as immunosuppressive therapy throughout the course of infection. {"title":"Palliative Cholecysto-Jejunostomy for Extra-Hepatic Billiary Obstruction Under Local Anaesthesia: A Rewarding Experience","authors":"A. In its severe form, it may be life threatening due to complications such as, hepatic encephalopathy, Hepato-renal syndrome, fulminant cholangitis and jaundice related coagulopathy. Obstructive Jaundice is the most common presentation following extra-hepatic billiary obstruction and carcinoma of the head of the pancreas and choledocholithiasis are the most common malignant and benign causes. Most patients present with advanced disease where palliation is the only viable option. However, such patients may not be suitable for surgery under General Anaesthesia due to advanced age, poor nutritional state with hypolbuminaemia, extra-abdominal metastasis producing respiratory compromise or a high risk for hepatic encephalopathy. Objectives: We aim to assess the safety and effectiveness of a palliative Cholecysto-Jejunostomy done under local Anaesthesia. Methods: This is a prospective cross-sectional study of total of 54 patients, both males and females, aged 16 years and above, who presented at the General Surgery units of State Specialist Hospital Damaturu and General Hospital Potiskum, both in Yobe State Nigeria; with Obstructive Jaundice. All had clinical and Radiological evidence of extra-hepatic Billiary obstruction and were managed surgically with a palliative Cholecysto-Jejunostomy under local Anaesthesia involving infiltrative cutaneous field block and USS guided Oblique Subcostal Transversus Abdominis Plain Block (OSTAPB). It is a 6-year study done from January 2015-December 2020. Informed consent was obtained from all patients according to Helsinki guidelines and Ethical clearance was granted by the leadership of both hospitals. All data were analysed with SPSS 20.0 software for correlation of outcomes. Results: The study population were 54 patients, 59.3% males and 40.7% females. The mean age is 50.5 years and an age range of 16-75 years. Malignant lesion is the most common cause of extra-hepatic billiary obstruction, with exocrine pancreatic cancer responsible for 48.1% of all causes. Chronic pancreatitis (16.7%) and impacted common bile duct (CBD) stones at 14.8%, are the two most common benign causes. All the patients had an abdominal USS for diagnosis or non-contrast enhanced abdominal CT Scan if there is ambiguity. A total of 77.8% had palliative Cholecysto-Jejunostomy done under local Anaesthesia involving cutaneous field block with Lignocaine with Adrenaline at a standardised dose of 4mg/kg body weight and USS guided Subcostal TAP Block. The remaining had a palliative Cholecystostomy or no surgery done. All abandoned cases were because of an involved or fibrosed Gallbladder. The most common post-operative complications are Surgical Site Infection (SSI) at 24.1%, excessive Primary Haemorrhage and reactionary haemorrhage, at 9.3% each. The total fatality rate was 7.4%. Our objective was to identify the main gut microbiota genera and characterize these patients’ functional mutations after using antibiotics.METHODS: The 16S rRNA sequencing method was used to compare the gut microbiota of healthy controls (HCs) with CCP using and not using antibiotics.RESULTS: All CCP demonstrated a significantly reduced alpha diversity of the gut microbiota (P<0.01). The gut microbiota's alpha diversity and the abundance of genera’s beta diversity did not show statistical differences between the non-antibiotics and antibiotics groups. There were 15 altered genera with common abundance in the non-antibiotics and antibiotics groups compared to the HC group. The area under the curve (AUC) of the top three probiotics, i.e., Faecalibacterium, Eubacterium, and Subdoligranulum, was 0.91. Among the 13 genera altered in the non-antibiotics group, the top three genera were not appropriate as biomarkers for cases receiving antibiotics. Compared to these 13 genera, the differences between the genera and the proportion of gram-positive bacteria in the 17 genera altered only in the antibiotics group were not statistically significant. The Kyoto Encyclopedia of Genes and Genomes (KEGG) analysis indicated that the antibiotics caused alterations in the abundance of certain genera.
