{"title":"Detection and analysis of an Achromobacter xylosoxidans outbreak in a urodynamics unit.","authors":"Simón Juárez Zapata, Carla Benjumea Moreno, Charo Porrón, Carles Alonso-Tarrés","doi":"10.1016/j.eimce.2025.02.006","DOIUrl":"https://doi.org/10.1016/j.eimce.2025.02.006","url":null,"abstract":"<p><strong>Introduction: </strong>Achromobacter xylosoxidans is a gramnegative bacillus resistant to multiple antibiotics, present both in the environment and in hospitals. This study describes an outbreak of colonizations and infections by A. xylosoxidans in the urodynamics unit of the Puigvert Foundation.</p><p><strong>Methods: </strong>On November 11, 2022, a patient developed a fever, and A. xylosoxidans was detected in their urine and blood. The case was linked to a recent urodynamic study. As a result, all A. xylosoxidans cases since 2018 were reviewed, and inspections were conducted in the unit, along with the collection of 24 environmental samples.</p><p><strong>Results: </strong>The review identified 21 patients with A. xylosoxidans infections after urodynamic procedures since April 2022. Environmental microbiological controls revealed that pressure transducers were the likely source of infection. Corrective measures included the temporary closure of the unit, thorough cleaning with hypochlorite, use of single-use urinary catheters, daily replacement of equipment lines and pressure transducers, as well as other improvements in disinfection, handling, and workflows. A multidisciplinary team was formed to implement and supervise these actions.</p><p><strong>Conclusions: </strong>The measures resulted in the elimination of the outbreak and the safe resumption of activities in the unit. They have excellent immune modulatory and adjuvant properties. The aim of this study was to improve the diagnosis of latent tuberculosis infection (LTBI) by adding GNPs in tests based on interferon-gamma (IFN-γ) measurement.</div></div><div><h3>Methods</h3><div>GNPs were coated with <em>Mycobacterium tuberculosis</em> (Mtb) recombinant proteins including TB10.4, CFP-10, ESAT-6, and TB7.7. Then, they exposed the whole blood of subjects with active tuberculosis (aTB), LTBI, and healthy controls (HC). Subsequently, IFN-γ produced in GNP tubes (QFT-NG) was measured and compared with IFN-γ produced in tubes without GNPs [QFT-A: (CFP-10, ESAT-6, TB7.7, TB10.4), QFT-B: (CFP-10, ESAT-6, TB7.7)].</div></div><div><h3>Results</h3><div>The results showed that the IFN-γ production in the GNPs tubes was significantly higher than in tubes without GNPs in aTB, LTBI, and HC subjects. In aTB patients, the mean difference (MD) between the QFT-NG and QFT-A tubes was 0.44, with a <em>P</em>-value of 0.04, and a 95% CI: 0.07–0.88. Furthermore, in LTBI individuals, the QFT-NG and QFT-A tubes exhibited an MD<!--> <!-->=<!--> <!-->0.21, <em>P</em>-value<!--> <!-->=<!--> <!-->0.03, 95% CI: 0.15–0.4. This review synthesized data on the prevalence of this bacterial infection in South American countries. A comprehensive literature search relevant articles (published between 2010 and 2022) was conducted in four databases. Full texts were selected based on PECOTS eligibility and JBI methodological quality. The pooled bacterial infection rate was calculated using a random-effects meta-analysis model. Subgroup and meta-regression analyses were used to investigate statistical heterogeneity. Five studies (covering 717 individuals from Peru and Ecuador) were eligible for meta-analysis. The percentage of seropositive IgG antibodies against <em>B. bacilliformis</em> was 28.21% (95% CI: 6.29–33.39) among healthy Ecuadorian children. In Peru, the pooled bacterial detection rate in symptomatic individuals was 15.60% (95% CI: 4.24–31.98), using molecular tests. Carrion's disease is endemic in the Andean valleys, but a gradual reduction in infection rates among acute febrile patients in Peru has been observed in recent years. Laboratory diagnosis of this infection continues to be neglected in other South American countries.
