{"title":"In Vitro Fertilization Cycle Outcomes for a Patient Undergoing Oocyte Retrieval following Ovarian Torsion.","authors":"Roisin M Mortimer,&nbsp;Catherine Gordon,&nbsp;Serene Srouji","doi":"10.4293/CRSLS.2022.00092","DOIUrl":"https://doi.org/10.4293/CRSLS.2022.00092","url":null,"abstract":"<p><strong>Introduction: </strong>To report a case of ovarian torsion following ovarian hyperstimulation with subsequent detorsion and oocyte retrieval.</p><p><strong>Case description: </strong>The patient was diagnosed with torsion following acute onset abdominal pain following her leuprolide acetate trigger injection. The patient underwent diagnostic laparoscopy which confirmed right ovarian torsion. Following detorsion, the patient underwent oocyte retrieval as planned with 72 total oocytes and 70 mature oocytes retrieved. Thirty-six mature oocytes were cryopreserved; 34 were inseminated with conventional in vitro fertilization, of which 27 (79.4%) were fertilized. Sixteen blastocyst stage embryos were cryopreserved.</p><p><strong>Discussion: </strong>Ovarian torsion during ovarian hyperstimulation is a rare event, but consideration should be given to detorsion first, followed by oocyte retrieval. These hernias can be asymptomatic or have various clinical presentations. One of the clinical scenarios is incarceration of bowel through the diaphragmatic defect. A bowel obstruction from an incarcerated diaphragmatic hernia is a rare occurrence. {"title":"Recurrent Paravaginal Abscess: An Unusual Presentation of a Distal Ectopic Ureteral Remnant.","authors":"Daniel Y Lovell, Amanda L Merriman, Kristi C Benjamin, Gerald B Taylor","doi":"10.4293/CRSLS.2022.00044","DOIUrl":"10.4293/CRSLS.2022.00044","url":null,"abstract":"<p><strong>Introduction: </strong>Obstructed hemivagina ipsilateral renal agenesis (OHVIRA) is a rare syndrome with limited data on both treatment and postoperative sequelae, specifically of ectopic ureters to the vagina.</p><p><strong>Case description: </strong>This case study describes a 22-year-old patient with OHVIRA syndrome presenting with pelvic pain and drainage from a chronic vaginal abscess secondary to a remnant distal ectopic ureter after nephrectomy and proximal ureterectomy. Imaging was significant for a right paravaginal abscess. Previously, the patient was treated with conservative therapy and on presentation her evaluation confirmed a right paravaginal abscess. She subsequently underwent a robot-assisted incision and drainage of the paravaginal abscess and excision of the remnant distal ectopic ureter. She did well postoperatively without recurrence.</p><p><strong>Discussion: </strong>We discuss the successful surgical technique used to identify and excise a paravaginal abscess. We also highlight the unique anatomy of a patient with OHVIRA syndrome. Heterotopic pancreas is a rare congenital anomaly where pancreatic tissue is located outside of the pancreas. These lesions may be encountered incidentally during surgery, which raise unexpected management questions.</p><p><strong>Case 1: </strong>A single pathology confirmed ectopic pancreas lesion encountered in the jejunem during laparoscopic Roux-en Y gastric bypass.</p><p><strong>Case 2: </strong>Two pathology confirmed heterotopic pancreas lesions encountered in the jejunem during laparoscopic Roux-en Y gastric bypass.</p><p><strong>Discussion: </strong>Heterotopic pancreas lesions are generally benign and encountered incidentally during intra-abdominal surgery. Surgeons must decide whether to resect the incidentally found mass. Case Description: A 61 -year-old female with a body mass index of 27 with vault prolapse underwent laparoscopic sacrocolpopexy. The patient was discharged on postoperative day two. She presented again in the emergency department on the fifth postoperative day with complaints of frequent vomiting episodes with intermittent and colicky pain in the abdomen. Her imaging (computed tomography abdomen/pelvis with oral contrast) suggested distal small bowel mechanical obstruction at midileum with significant free fluid in the peritoneum. Emergency laparoscopic exploration was done. Peroperative V-LocTM 180 suture tail end barbs were found anchored to the mesentery of midileum causing a loop that led to compression and occlusion of distal bowel. The barbed suture tail end was detached from the mesentery and thus relieving the compression. No additional procedure was required for the bowel wall. The excess barbed suture tail end outside the peritoneum was trimmed. The postoperative course was uneventful. Conclusion: Bowel complication is an uncommon but serious issue following the use of barbed sutures. It should be used with utmost caution as none of the preventive measures are completely safe. The patient was surgically treated with a small bowel resection, and eventually made a full recovery.</p><p><strong>Discussion: </strong>The pathophysiology of COVID-19-induced SBO can be explained by the prolonged inflammation and coagulation activation in the bowel's vasculature system. Under these circumstances, microthrombosis occurs in the bowel's microvasculature; the affected intestinal tissue becomes ischemic and infarcted. Von Meyenburg complexes are often asymptomatic and thus typically discovered incidentally on imaging or autopsy. They can also be encountered at the time of surgery where they often appear as scattered white liver lesions concerning for malignancy. Here, we present a case in which white hepatic nodules were found incidentally during laparoscopic cholecystectomy in a 36 -year-old female. Pathologic analysis confirmed the diagnosis of von Meyenburg complexes. The operating surgeon proceeded with laparoscopic cholecystectomy without complication. We report this case to encourage awareness of this benign entity. The finding of scattered hepatic lesions found intra-operatively can create concern for metastatic neoplastic processes. {"title":"Effective Port Placement for Left Sided Gallbladder Cholecystectomy.","authors":"Joel Braverman,&nbsp;Kristine Makiewicz","doi":"10.4293/CRSLS.2022.00061","DOIUrl":"https://doi.org/10.4293/CRSLS.2022.00061","url":null,"abstract":"<p><strong>Introduction: </strong>Left sided gallbladder (sinistroposition) is a well described congenital abnormality that can pose an unexpected challenge for the surgeon, especially regarding port placement for safe and effective dissection.</p><p><strong>Case description: </strong>In this case, a 36 -year-old woman with biliary colic was taken to the operating room for elective cholecystectomy and found, after port placement, to have sinistroposition of the gallbladder. The operation was completed with relative ease using our typical port placement of a 5 mm port at Palmer's point, a 12 mm port at the umbilicus; and two additional 5 mm ports, one in the right midclavicular line, and one in the right anterior axillary line.</p><p><strong>Discussion: </strong>Multiple port placements for safe and effective dissection of a left sided gallbladder have been discussed. Identification of sinistropic gallbladder often occurs after ports are already placed in position for right sided cholecystectomy. In this case, our typical port placement where the operating surgeon's right-hand port is located at Palmer's point provided excellent positioning for dissection. No alterations to the surgeon's left-hand port or the assistant port were necessary. The dissection was able to be completed from familiar angles, so dissection and identification of anatomy was performed with relative ease.
