Our objective is to study the relationship between the presence of NIDp with the total number of NMA in the feet and the rest of the body, racial phenotypic characteristics, and other risk factors for melanoma.</p><p><strong>Material and methods: </strong>A cross-sectional observational study was carried out that included 255 patients, ≥18 years old, who attended our Dermatology service from September 2020 to February 2021. All NMAs ≥1 mm from the feet and ≥ 2 mm from the rest of the body. The association between the variables was studied using a univariate and multivariate logistic regression model.</p><p><strong>Results: </strong>The presence of NIDp was significantly and independently associated with the presence of plantar NMA and with body counts of ≥50 NMA. However, currently, there are few articles to systematically analyze the efficacy profile of topical drugs for rosacea. This study aimed to investigate the efficacy profile of widely used topical drugs. To acquire appropriate information from related literature, we looked into 4 databases. Efficacy was appraised with the Investigator Global Assessment, Clinician's Erythema Assessment, Patient's Self-Assessment and Subject Self-Assessment of Rosacea Facial Redness scales. Treatment-emergent adverse events and dermal tolerability were also recorded. According to 21 randomized controlled trials included, a total of 6 topical drugs including minocycline, ivermectin, azelaic acid, metronidazole, brimonidine and oxymetazoline were reported. These drugs are well-tolerated and safe. Ivermectin is more effective than azelaic acid and metronidazole. Azelaic acid has a better efficacy profile than metronidazole according to included studies. Minocycline turned out to be effective improving the symptoms of rosacea. {"title":"Unraveling the Link Between Bullous Pemphigoid and Neurological Disease: A Single-center Study of 257 Patients.","authors":"L M Nieto-Benito, R Suárez-Fernández","doi":"10.1016/j.ad.2025.06.002","DOIUrl":"10.1016/j.ad.2025.06.002","url":null,"abstract":"<p><strong>Background: </strong>Multiple studies have corroborated the association between bullous pemphigoid (BP) and neurological diseases; patients with both diseases (BP-N) have been associated with a worse prognosis and specific clinical and immunological characteristics, defining a different subtype of BP.</p><p><strong>Objectives: </strong>We aimed to determine the prevalence and characteristics of BP cases with neurological comorbidities (BP-N) and review the related published literature.</p><p><strong>Methods: </strong>We conducted a retrospective, observational study of BP cases treated at a referral center for autoimmune blistering diseases from January 2000 through June 2020.</p><p><strong>Results: </strong>We collected epidemiological, clinical, histopathological, progression and laboratory data from a total of 257 cases, 102 of which were BP-N. Senile dementia, Alzheimer's disease and vascular dementia were the most frequent neurological comorbidities. Compared with cases without neurological comorbidities, BP-N cases had more intense tissue eosinophilia (P=.012) and higher concentrations of circulating eosinophils (P=.000), and anti-BP180 IgG antibodies (P=.007). However, as far as we know, data on this association for brodalumab are lacking in the currently available scientific literature.</p><p><strong>Objectives: </strong>To analyze the association of 180 polymorphisms with an optimal response to brodalumab in real-world clinical practice.</p><p><strong>Methods: </strong>A total of 119 patients with plaque psoriasis on a 24-regimen of brodalumab recruited from 11 Spanish hospitals were genotyped for 180 polymorphisms. Optimal response was evaluated as absolute (PASI) ≤ 1 at 6 and 12 months. Polymorphisms with false discovery rates < 0.25 were included in a multiple regression model.</p><p><strong>Results: </strong>A total of 68% and 62% of patients achieved PASI ≤ 1 at 6 and 12 months, respectively. Patient weight, history of biological therapy, disease-modifying anti-rheumatic drugs, and psoriatic arthritis were identified as risk factors for failing to achieve PASI ≤1. At 12 months, polymorphisms rs495337 (SPATA2), rs6311 (HTR2A), and rs4085613 (LCE3D) were associated with achieving a PASI ≤1 regardless of previous use of biologics and DMARDs, psoriatic arthritis, or weight. The genotypes CT-TT for rs6311 (HTR2A) and GT for rs4085613 (LCE3D) were identified as risk factors for lack of optimal response at 12 months, while genotypes AG-AA for rs495337 (SPATA2) increase the probability of response. Their diagnosis is predominantly clinical, but its laboratory confirmation is increasingly encouraged. We conducted a 6-year retrospective, observational study of polymerase chain reaction (PCR) confirmed mucocutaneous HSV infections in the largest Portuguese tertiary teaching hospital. Throughout this period, a total of 947 PCR herpesvirus tests in mucocutaneous swabs were performed, 448 of which tested positive (266 for HSV). Regarding the PCR-confirmed mucocutaneous HSV infections, HSV-1 was the main cause of orolabial herpes and HSV-2 of genital herpes. A significant proportion of patients were immunocompromised, demonstrating the risk for herpesvirus infection and reactivation in this setting. HSV-1 genital herpes was more common in women, younger individuals and had a lower rate of recurrences.
