Currently, we have new non-invasive diagnostic methods to complement the clinical and trichoscopic findings of patients with alopecia; these are high-frequency ultrasound (HFU), ultra-high-frequency ultrasound (UHFU), reflectance confocal microscopy (RCM) and optical coherence tomography (OCT), which are capable of identifying structures under the surface of the scalp, while maintaining the ability to monitor hair shafts. In recent years there have been multiple advances and changes in the management of psoriatic patients that justify a review and update of the use of topical therapy. Experts from the Spanish Psoriasis Working Group (GPS) of the Spanish Academy of Dermatology Venereology (AEDV) have developed a set of recommendations for the treatment of psoriasis based on the best available evidence and experts' opinion.</p><p><strong>Methodology: </strong>We followed the methodology of nominal groups, with the help from a systematic review. A coordinator was designated, and a group of experts was selected based on their experience and knowledge on topical therapies for the management of psoriasis. Afterwards, the objectives and key points of the document were defined and agreed upon, and with help from a documentary specialist, a systematic review was conducted across Medline, Embase and Cochrane Library. Systematic reviews, meta-analyses, clinical trials, as well as observational studies were selected. Published clinical practice guidelines and related documents were also reviewed. With this information, the coordinator generated a series of recommendations that would be later evaluated and modified by the experts themselves. After several review processes and an external evaluation, the final document was drafted.</p><p><strong>Results: </strong>Practical recommendations on the use of topical therapies for the management of psoriasis are presented in line with other documents of the AEDV, including the use of topical treatment as the main therapy, their adjuvant role when using systemic therapies, treatment selection, treatment strategies, their use in special locations and severe psoriasis, and the patients adherence and preferences to the topical therapy. {"title":"Home Self-Applied Daylight Photodynamic Therapy: Current Map in Spain and Management Proposals.","authors":"T Gracia-Cazaña, Y Gilaberte","doi":"10.1016/j.ad.2024.10.064","DOIUrl":"10.1016/j.ad.2024.10.064","url":null,"abstract":"<p><strong>Objective: </strong>To determine the current practices of Spanish dermatologists performing home self-applied daylight photodynamic therapy (dPDT), and their experience with this treatment.</p><p><strong>Methodology: </strong>We conducted a retrospective, multicenter, cross-sectional observational study with participation from 100 dermatologists from public and private Spanish hospitals. Their practices were collected through an online questionnaire and prepared based on a literature review, including experience, climatology, advantages, disadvantages and satisfaction with the self-applied dPDT.</p><p><strong>Results: </strong>A total of 38.4% of dermatologists have >5 years of experience using dPDT and 72.7% alternate consultation and home self-application. A total of 61.1% of dermatologists do not have specific protocols for self-application and 69.4% explain instructions to their patients personally. A total of 50% prefer to apply dPDT in months of greater solar activity and 70.8% recommend treatment during the morning. A total of 95.9% of the patients are satisfied or very satisfied with the treatment.</p><p><strong>Main conclusions: </strong>Self-applied dPDT is an effective and well-tolerated option to treat AK, especially in patients who adequately follow instructions and have family support. Studying the characteristics and prognosis of Caucasian patients is crucial to understand the distinct features of this tumor.</p><p><strong>Objectives: </strong>To analyze the epidemiological, clinicopathological, and prognostic features of acral melanoma in Caucasian patients.</p><p><strong>Methods: </strong>We conducted a retrospective, multicenter, cohort study of acral melanoma from a database across 20 hospitals from South Europe from January 2000 through December 2019.</p><p><strong>Results: </strong>A total of 733 acral melanomas were identified (median age, 67.5 years; 95.2%, Caucasians; 77.5% of which were located on the feet). Overall, 77.5% of cases were invasive melanomas. Foot melanomas had a higher proportion of invasive cases (80.8% vs 69.8%; p = 0.003), stages III and IV at diagnosis (24.8% vs 11.7%; p < 0.001), thicker Breslow depth (2.8 mm vs 2.0 mm; p = 0.021) and a higher rate of positive sentinel lymph node biopsy (SLNB) (30.7% vs 15.7%; p = 0.012). Thicker Breslow depth and later age of onset were risk factors for melanoma-specific survival. Thicker Breslow depth and ulceration were independent prognostic factors of relapse-free survival. Melanoma location and histopathological subtype were not associated with worse prognosis. Recurrences were a common finding (27.7%), with distant metastases appearing earlier than locoregional recurrences (1.32 years [IQR, 1.12-1.87] vs 2.14 years [IQR, 1.68-2.70]; p = 0.015).</p><p><strong>Conclusion: </strong>This study, the largest in a predominantly Caucasian population, underscores the unfavorable outcomes of acral melanoma. Foot melanomas exhibited delayed detection, increased invasiveness, thicker Breslow depth, increased SLNB involvement, and higher AJCC stages.
