{"title":"Viability determination of M.leprae: comparison of normal mouse foot pad and bacillary ATP bioluminescence assay.","authors":"U D Gupta, K Katoch, M Natarajan, V D Sharma, R K Sharma, C T Shivannavar, V M Katoch","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Correlation between viability assessment by mouse foot pad and ATP bioluminescence was studied in biopsy specimens from multibacillary leprosy cases. Biopsies were processed for inoculation into mouse foot pad and estimation of bacillary ATP levels by bioluminescent assay by earlier established procedures. ATP content as pg/million bacilli was estimated and correlation was assessed with growth in the mouse foot pad. It was observed that when the ATP content was > 36 pg/million bacterial cells, (> 1% probable viables) there was growth in the mouse foot pad from all the specimens. Similar results were observed when the ATP content was in the range of 3.6 to 35.99 pg/million cells (0.1 to 1% probable viables). The positivity rates in the mouse foot pad decreased when the ATP content decreased further. No positive growth in the specimens below 0.04 pg/million bacilli (< 0.001% viable organisms) was observed. These findings show an overall correlation between viability assessed by mouse foot pad and ATP bioluminescence. {"title":"Prevalence of HIV1 in leprosy patients in Rio de Janeiro, Brazil.","authors":"V L Andrade, T Moreira Alves, J C Regazzi Avelleira, M Bayona","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>The purpose of this study was to learn if HIV1 infection was associated with leprosy in Rio de Janeiro, Brazil by comparing the prevalence rates of 1.016 leprosy patients tested on a voluntary basis and 78.482 blood donors. A cross-sectional survey of anti-HIV1 antibodies was conducted in Rio de Janeiro, from 1990 to 1992 for this purpose. HIV1 prevalence found among leprosy patients was (3 cases) 2.9 per 1000, and among blood donors was (282 cases) 3.8 per 1000. Such difference was not significant (OR = 0.79; p = 0.69). Since HIV1 cases were only found among male leprosy patients, further analysis excluded females. Male leprosy patients showed a slightly higher prevalence of HIV1 than blood donors before and after age adjustment. However, this result was not statistically significant (adjusted odds ratio = 1.38, 95% CI 0.35-4.5; p = 0.83). These data do not provide evidence that leprosy and HIV1 infection are associated in the State of Rio de Janeiro. A disability survey was conducted in April 1996 with a half-randomized sample of five hundred patients. This survey showed that 28.73% of the patients had got grade two disabilities in WHO scale. 12.9% of the non disabled patients at detection had developed leprosy impairments during or after treatment. Plantar ulcers (12.2% of the patients) appeared very frequent comparatively to the findings of a similar survey in Burkina Faso in 1995 (0.9% of plantar ulcers). Considering the important number of patients showing a PU, the directors of national leprosy control programmes are determined, within the programmes on prevention of disabilities (POD), to treat the PU in the field. Therefore it appears to be essential to codify and simplify their treatment thus enabling it to be effective. The healing of PU being the only criteria of effectiveness of the technique. Four clinical stages were defined, each corresponding to a precise way of treatment using only essential and basic products at low cost. {"title":"[Prevention of disabilities and physical rehabilitation of leprosy].","authors":"W C Smith","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>A survey of the prevention of disability policy and activities in a random sample of 200 ILEP assisted projects was conducted in 1995. This was followed by a workshop of field experts in different aspects of prevention of disability who work in different geographical regions. The survey findings and state of current knowledge on prevention of disability were reviewed during the workshop and recommendations on the planning, implementation, and evaluation of simple and effective prevention of disability developed. Prevention of disability includes complex activities, such as nerve decompression and reconstructive surgery, however these recommendations focus on the simple techniques and approaches which can be implemented through leprosy control programmes, primary health care and community-based rehabilitation. Calcification was most common in the ulnar nerve followed by the lateral popliteal nerve. All eight patients were males with significantly longer duration of illness before start of treatment. Patients with late onset of nerve abscess were found to be more prone to calcium deposition in the nerves. {"title":"[Leprosy differential diagnosis in 1996].","authors":"A Mahé","doi":"","DOIUrl":"","url":null,"abstract":"<p><p>Due to a decrease in the prevalence of leprosy, discussion of its differential diagnosis assumes increasing importance. Research on the early diagnosis of leprosy, before the onset of nervous lesions, the appearance (or improved definition) of certain diseases and the expansion of intercontinental migration are some of the factors which have induced changes in the conditions of leprosy diagnosis. The most frequent and most sensitive differential diagnosis of leprosy have been reviewed: hypochromic diseases, tropical infections diseases, systemic diseases, deforming diseases, cutaneous lymphoma and AIDS. The difficulties posed by forms of leprosy revealed by reactional manifestations are stressed. Today, as in previous times, positive and differential diagnosis of leprosy is based on the following simple practices: study of lesion, sensitivity, bacilloscopy and the examination of the peripheral nervous system.
