{"title":"[Gastric adenocarcinoma complicated by subarachnoid hemorrhage and cerebral infarction: a rare case of Trousseau syndrome and literature review].","authors":"J Zhao, F Qiu, J L Feng","doi":"10.3760/cma.j.cn112138-20240827-00538","DOIUrl":"10.3760/cma.j.cn112138-20240827-00538","url":null,"abstract":"<p><p>Trousseau's syndrome is a thromboembolic disorder associated with malignancies, with cerebral infarction and hemorrhage representing common central nervous system complications in patients with cancer. This report details the diagnosis and treatment of a patient with gastric adenocarcinoma at our institution who concurrently developed cerebral infarction and subarachnoid hemorrhage. We performed a comprehensive literature review in the Wanfang and PubMed databases, searching for relevant studies on Trousseau's syndrome, cerebral embolism, and subarachnoid hemorrhage. A total of 42 patients (23 male and 19 female) with complete clinical data were included, including the present case.Our analysis showed that these patients had 10 different types of tumors, with lung cancer, gastric cancer, and ovarian cancer being the most prevalent. D-dimer levels were elevated in 38 cases, and 38 patients exhibited cerebral infarction affecting three or more vascular territories. Anticoagulant therapy was administered to 40 patients, 22 experienced a recurrence of cerebral infarction within a short timeframe, and 28 died within one year. Cases of Trousseau's syndrome associated with subarachnoid hemorrhage are rare and, to our knowledge, have not been previously reported in the literature. Patients experiencing strokes due to Trousseau's syndrome tend to have poor prognoses. Patients usually have complex health conditions, while the treatment process involves a wide range of content, along with high requirements for doctor's professionalism and multi-specialty teamwork, as well as a great demand for time-sensitive treatments. However, this is not matched with critical care professionals and the current state of medical care in China. Telemedicine, which shortens the distance of medical professionals and the gap of disease diagnosis and treatments in various regions through electronic information, can effectively solve the current problem. Previous studies have found that influenza infection can cause cardiac damage through various pathways, and patients with cardiovascular diseases are at relatively higher risk of adverse disease outcomes. Influenza vaccination has been proven to provide protective effect on patients with cardiovascular diseases. Currently, there is insufficient emphasis placed by cardiologists and cardiovascular disease patients on the prevention of influenza infection, leading to a low influenza vaccination rate in China. The patient had intermittent fever for 2 months with a maximum body temperature of 39.3 ℃ and elevated serum creatinine levels for 1 week. He had no other suggestive symptoms or positive signs. Laboratory test results suggested acute kidney injury and a sharp elevation in serum lactic dehydrogenase levels. Abdominal enhanced computed tomography (CT) revealed multiple low-density lesions, and further biopsy pathology demonstrated chronic inflammation. Thereafter, positron emission tomography (PET)/CT showed abnormally elevated uptake value for the bones throughout the entire body, in addition to the liver and brain. Repeated bone marrow biopsy finally confirmed metastatic bone cancer, which possibly originated from the kidney according to immunohistochemical staining. In this rare case of fever of unknown origin, the primary lesion was a renal tumor with bone, liver, and brain metastases.
