Methods: Retrospective analysis of medical records of 25 patients with ulnar positive variance’s various symptoms were treated in our hospital with transverse shortening osteotomy of the ulnar from July 2011 to May 2020, the preoperative and postoperative ulnar variation, wrist joint activity, radioulnar deviation range, forearm rotation degree, pain degree at wrist activity, handgrip strength, modified Sarmiento wrist score and change of upper limb function DASH score were compared to evaluate the clinical effect of the surgery. Results: 25 patients were fully followed up. All patients achieved bone healing. The changes in the preoperative and postoperative ulnar variation, wrist joint activity, radioulnar deviation range, forearm rotation degree, handgrip strength and upper limb function were compared. The positive variation of ulna was significantly improved after surgery. The mobility of the wrist was improved to some extent, especially hand grip strength and pain significantly compared with that before surgery. The range of motion to the healthy side gained limited increments from (75.1 ± 10.1)% to (89.1 ± 10.3)% for wrist flexion-extension, from (62.3 ± 7.9)% to (87.8 ± 11.2)% for ra-dioulnar deviation, and from (76.3 ± 8.2)% to (91.8 ± 8.1)% for forearm rotation. The grip strength to the healthy side increased from (62.1 ± 10.1)% to (90.2 ± 9.5)%. The pain VAS score reduced from (4.6 ± 1.2)% to (0.7 ± 0.5). At the last follow-up, the modified Sarmiento to wrist score averaged 89.1 points, with 16 excellent, 6 good, 2 fair and 1 poor. The excellent and good rate was 88.0%. Besides, the DASH score of upper limb function after surgery was obviously decreased compared with that before surgery (55.1 ± 9.5) points vs. (25.2 ± 8.7) points. Conclusion: Ulnar transverse shortening osteotomy in the treatment of ulnar positive variance’s various symptoms can alleviate wrist pain, improve wrist joint function and enhance handgrip strength. It’s an effective treatment.","PeriodicalId":67890,"journal":{"name":"外科(汉斯)","volume":"136 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"136053381","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

At present, mul-tisegmental LDH is mostly seen in the middle-aged and elderly population, and is less common in younger patients. Conservative treatment for patients with multisegmental onset is ineffective, while surgery is dominated by multisegmental fusion or endoscopic management of a single responsible segment, and endoscopic treatment of multisegmental is less common. Unilateral Biportal Endoscopic technology (UBE) is the use of arthroscopic system to carry out treatment, with a clear field of operation, flexible operation, decompression is sufficient and other characteristics, endoscopic surgery in the endoscopic treatment of multisegmental lumbar disc herniation has a unique advantage. Methods: Based on FMEA, 540 surgical patients from January to June 2020 were selected as the control group, and 540 surgical patients from January to June 2021 were served as the observation group. By analyzing the failure modes of the perioperative safety verification process of surgical patients, 13 failure modes were identified as the key points for improvement and corrective measures were developed. The Risk Priority Number (RPN) before and after failure mode and effect analysis was compared, as well as the accuracy rate of surgical safety verification and surgical identification, and the effectiveness of rectification was evaluated. Results: The mean RPN value before and after FMEA management decreased from 184 to 48.5; the accuracy of surgical safety check increased from 95.45% to 99.42% with a statistical difference (P < 0.01); the correct rate of surgical identification increased from 98.84% to 99.90%, but no statistical difference was observed (P ˃ 0.05). Conclusion: As used for perioperative safety verification of surgical patients, FMEA is very helpful in identifying potential defects in process improvement, significantly reduces the risk priority index and increases the accuracy of surgical safety check.","PeriodicalId":67890,"journal":{"name":"外科(汉斯)","volume":"63 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135748994","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It is a method adopted by many researchers that they use mixed liquid of different proportion of lidocaine and epinephrine hydrochloride to penetrate into the nasal mucosa of the surgical area before surgery to maintain a clear visual field and reduce surgical bleeding. However, because of rich blood vessels in the nasal mucosa, it remains unclear that whether it will affect the stability of intraoperative hemodynamics, postoperative recovery and have complications. Accurate diagnosis of appendicitis in children remains challenging in many cases. Acute appendicitis was divided into acute uncomplicated appendicitis (UA) and complicated appendicitis (CA) according to the intraoperative condition and postoperative histopathological classification. Appendectomy has always been the gold standard for AA treatment, but current studies have shown that conservative treatment in some children with UA has a good effect, so it is necessary to distinguish different severity of appendicitis before surgery. This study explores its application in differentiating the severity of acute appendicitis based on blood indicators.
