Gökyiğit","doi":"10.4274/jarem.galenos.2021.87597","DOIUrl":"https://doi.org/10.4274/jarem.galenos.2021.87597","url":null,"abstract":"Objective: Comparing refractive values determined by two photorefractory devices (Plusoptix® A09, GmbH, Nuremberg, Germany and SureSight®, Welch Allyn Co, New York, USA) with refractive values obtained by skiascope after cycloplegia with 1% cyclopentolate hydrochloride in healthy children. Methods: Cases aged between 48 and 132 months were evaluated for this cross-sectional study. Cases with no ophthalmic pathology in both eyes and uncorrected visual acuity of 1.0 were included in the study. In all cases, refractive measurements were performed first with Plusoptix® and then with SureSight®. After providing cycloplegia with 1% cyclopentolate hydrochloride, skiascopy was performed. The correlation between the measurements obtained was evaluated with the Pearson correlation coefficient, and the compatibility was evaluated with the Bland-Altman analysis. Results: The mean spherical values of 52 subjects measured by cycloplegic skiascopy, Plusoptix® and SureSight® were 0.88±2.07 dioptry (D), 0.69±1.78D and 1.64±1.06D, respectively; mean of cylindrical measurements are -0.73±0.68D, -0.92±0.67D and -0.83±0.70D, respectively. Comparing Plusoptix® with skiascopy, SureSight® with skiascopy, Plusoptix® with SureSight® measurements, a high positive correlation was found between spherical values (r=0.861, r=0.736, r=0.721, respectively); a positive correlation was also found between cylindrical values (r=0.602, r=0.675, r=0.901, respectively). Skiascopic spherical and cylindrical measurements with Plusoptix®, and skiascopic spherical and cylindrical measurements with SureSight® were found to be compatible with each other (within 95% confidence interval, lower limit: -2.65, upper limit: 1.89; lower limit: -1.35, upper limit: 0.95; lower limit: -3.56, upper limit: 2.06; lower limit: -1.20, upper limit: 0.98). Spherical and cylindrical measurements obtained with Plusoptix® and SureSight® were found to be consistent with each other (within 95% confidence interval, lower limit: -3.39, upper limit: 1.49; lower limit: -0.67, upper limit: 0.51). ÖZ Amaç: Sağlıklı çocuklarda, iki fotorefraktör cihaz (Plusoptix® A09, GmbH, Nuremberg, Almanya ve SureSight®, Welch Allyn Co, New York, ABD) ile saptanan refraktif değerleri, %1’lik siklopentolat hidrokloridle siklopleji sonrası skiaskopiyle elde edilen refraktif değerlerle karşılaştırmaktır. Yöntemler: Kesitsel çalışma için, yaşları 48 ile 132 ay arasındaki olgular değerlendirildi. Her iki gözde oftalmik patoloji saptanmayan ve düzeltilmemiş görme keskinliği 1,0 saptanan olgular çalışmaya alındı. Tüm olgularda önce Plusoptix®, ardından SureSight® ile refraktif ölçümler yapıldı. %1’lik siklopentolat hidrokloridle siklopleji sağlandıktan sonra skiaskopi gerçekleştirildi. Altıyüz kırk iki hastadan 179’u RV enteriti (%27,9), 22’si adenovirüs enteriti (%3,4), 14’ü Entamoeba histolytica enteriti (%2,2), 11’i Giardia enteriti (%1,7) ve 413’ü tanımlanamayan AGE patojeni (%64,3) nedeniyle hastaneye yatırılmıştır. Ruuska-Vesikari skoru hastanede kalış süresiyle ilişkili saptanmamıştır (p=0,31; r=0,04). Kış mevsiminde (%26,8) ve Nisan ayında (%16,8) diğer zamanlara göre RV enteriti daha yüksek görülmüştür. RV olgularının çoğu (%36,9) 12 ile 24 aylık yaş aralığında anlamlı derecede sık raporlanmıştır. AGE’li çocukların klinik şiddeti hafif, orta ve ağır klinik olarak sınıflandırılmıştır (sırasıyla %14,5, %35,5 ve %50). For this reason, as in all surgical branches, neurosurgery organisations have recommended postponing or limiting surgeries during the pandemic to reduce the risk of transmission. In our literature review, no study reported the number of neurosurgeons infected with COVID-19 following surgeries performed during the first 3 months of the pandemic. This study examined surgeries performed during the first 3 months of the pandemic in Turkey and COVID-19 transmission to neurosurgeons involved in these cases. Methods: Records of 188 patients who underwent surgery in our neurosurgery clinic during the first wave of the pandemic in Turkey (March 11-31 May 2020) were examined retrospectively. Characteristics of the operations performed, COVID-19 tests performed and results, intraoperative measures taken and frequency of COVID-19 symptoms among neurosurgeons after the surgery were determined. Results: Of the 188 patients included in the study, none had a definitive diagnosis of COVID-19 at the time of surgery. However, 25 patients (13.29%) had a history of unsafe contact or symptoms suggestive of COVID-19. Moreover, 9 (36%) patients had a positive result from the COVID-19 polymerase chain reaction test. A total of nine neurosurgeons who participated in surgeries were included in the study and none of them exhibited COVID-19 symptoms during the 3-month pandemic period. Conclusion: Although many surgeries were performed during the first wave of the pandemic in Turkey, with some simple precautions, none of the neurosurgeons developed COVID-19. FAI is increasingly used in the definitive diagnosis of patients with hyperandrogenism. Although the reference range of FAI in adult females has been identified, there are no adequate studies on the reference range for adolescent girls. We determined the diagnostic value of FAI in adolescent girls diagnosed with obesity, idiopathic hirsutism (IH) and polycystic ovary syndrome (PCOS). Methods: Patients aged 12 to 21 years and diagnosed with obesity, IH and PCOS were included in the study. FAI was found by calculating the ratio of total testosterone to SHBG. Results: According to receiver-operating characteristic (ROC) analysis results, FAI level under 3.45 indicated that the participants were healthy. ROC analysis was also used to determine the usefulness of FAI in distinguishingbetween healthy participants and those diagnosed withPCOS, obesity, IH. It was determined that patients with FAI above 6.15 should be evaluated for PCOS. {"title":"Targeted Follow-up of Incidental Lung Nodules: Will the New Nodules in Unscanned Regions Be Missed?","authors":"T. Gümüş, G. In order to apply targeted tomography safely, it is important to know the frequency of newly appearing nodules in unscanned regions of lung. We aimed to evaluate the frequency and importance of new pulmonary nodules that appear in patients followed-up for nodules according to Fleischner society guidelines. Methods: A total of 117 patients (women: 54; men: 63; mean age: 55±14 years; range: 30-88 years) who were followed-up for 265 lung nodules were included in this study. The inclusion criteria was presence of at least 1 nodule that was followed-up for at least 6 months with CT. Patients with calcified nodules or known malignancy during initial CT were excluded. Results: The median follow-up time was 22 months (range: 6-80 months) and the median number of follow-up CT scans was 2 (range: 1-5). New nodules appeared only in 6% (7/117) of the patients, of which 5 had nodules that disappeared or decreased in size during follow-up. Conclusion: For patients who were followed-up for pulmonary nodules, the frequency of new nodule formation was low. This should encourage the use of TCT for nodule follow-up. Few of the teams prefer not to use the instruments and postpone the surgery, whereas others may decide to remove the hair and the instruments in contact and continue using the remaining part. Evidence is required to determine a standard approach in such practices, which leads to negative consequences. Methods: Overall, 108 surgical clamps were sterilised using autoclave (n=36), hydrogen peroxide (n=36), and ethylene oxide (n=36). One third of the instruments in each group were packed along with a free hair strand, another third with a strangulated hair strand, and the last third were packed alone as the control group. Microbiological specimens of the instruments were collected with swabs. Hair samples were inoculated on thioglycolate broth. Growth was evaluated after 24 and 48 hours. Results: No growth was observed among the groups after 24 and 48 hours. Thus, all the instruments were considered sterile. It is a rare and challenging diagnosis, but it should be taken into account in the differential diagnosis of peripheral neuropathy, particularly in patients with a documented history of haematologic malignancy. Magnetic resonance (MR) neurography is very useful in diagnosis and especially when correlated with positron emission tomography/ computed tomography (PET/CT). In equivocal cases, nerve biopsy can be considered when the benefit outweighs the risk. We aimed to report a case of a 30-year-old male patient who was in complete remission from T-lymphoblastic lymphoma, presenting with clinical findings indicating initially ulnar entrapment. However, with the demonstration of brachial plexopathy with axonal loss in electrodiagnostic studies, MR imaging neurography dedicated to brachial plexus was carried out and revealed pathological enhancement associated with mild fluorodeoxyglucose (FDG) uptake on PET/ CT. Moreover, NL, due to the relapse of T-cell lymphoma, was diagnosed through incisional biopsy, showing diffuse infiltration of blast cells positive for terminal deoxynucleotidyl transferase, CD3 and CD10.
