It is 3% of all bone tumors. It may result in growth disturbance, bony deformity, joint swelling and contractures. Long bones especially of the lower limb are the most commonly affected regions. During the initial period, the resection of nidus was performed by open surgery where a bone block was removed. This was followed by development of computed tomography (CT)-guided procedures for complete removal of nidus using various percutaneous techniques. These include CT-guided radiofrequency ablation (RFA) or trephination. If these lesions are near any important vascular structures, it becomes difficult to use RFA due to risk of injuring them. While the results of RFA are well studied, percutaneous trephination is still not widely researched. Objectives: We evaluated the results of 14 cases of osteoid osteoma treated with percutaneous drilling resection of the nidus under CT guidance and discussed the positive and negative impact of the same. Material and Methods: We included 14 patients who presented, from October 2017 to September 2020, at a tertiary level health care center. The diagnosis was established by clinical as well as imaging modalities. The patients underwent CT scan and the exact site was localized. Safe zone for percutaneous drilling was identified using contrast for vascular bundle. Percutaneous drilling was done using 8 mm reamer and complete lesion was removed under CT guidance. Patients were followed up for 2 years after the procedure and the final results were obtained in terms of pain, functional ability of limb and recurrence. Results: 13 out of 14 patients were pain-free at the end of the follow-up. One patient did have some pain after 1 year and on repeat imaging, there were changes suggestive of recurrence. The success rate was thus 13/14 patients (92.8%). The mean pre-operative pain score improved from 2.3 (range, 0–4) to 4.85 (range, 4–5) after procedure. There was no functional deficit of the limb recorded at the end of final follow up. Conclusion: CT-guided percutaneous trephination is a cost-effective alternative to available treatments protocols for Osteoid osteoma. Although newer techniques such as radio-frequency ablation are available, they are neither easily accessible nor affordable for the masses in developing countries.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":"57 12","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-02","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135875327","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We present a case of osteoblastoma of the scaphoid in a 58-year-old adult male patient, its diagnostic challenge, and his treatment pathway from curettage and autologous bone graft until scaphoidectomy and four-corner fusion. The tumor showed an aggressive behavior and local destruction in an unusual location. It appeared in a patient outside the expected age range for this specific tumor, rendering a correct diagnosis extremely difficult to achieve. We pretend to emphasize its treatment evolution and highlight the importance of follow-up and surveillance of these tumor lesions.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-06-27","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"47636731","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

It occurs more commonly in spinal canal Stenosis and the surgical decompression of the spine in this condition is rarely required. Nerve tissue may develop dynamic dysfunction such as adhesion and neural fibrosis can impair nerve mobility. Neurodynamic mobilization methods may enhance the neural tissue mobility and enhance physical function. The aim of this study was to explore the role of neural mobilization on the pain free walking distance (PFWD) and walking time among cases of spinal canal Stenosis presenting with neurogenic claudication. Methods: Forty eight patients diagnosed with neurogenic claudication were randomly and equally divided into any of the four groups: the “tensioner neural mobilization”; the “one ended slider neural mobilization” group; the “two ended slider neural mobilization” group; and the placebo group. The distance walked before pain onset and this pain-free walking time was recorded immediately before and after the neural mobilization intervention as per group’s protocol. Result: The “two ended slider method” of neural mobilization was more superior to the “one ended slider” or the “tensioner method” for improving walking capacity. Neither “single ended” slider neural mobilization nor placebo intervention caused any statistically significant improvement in the walking capacity among patients of neurogenic claudication. Conclusion: [Formula: see text]he “double slider method” of neural mobilization is most appropriate followed by “slider method” of neural mobilization and the “single ended slider method” for enhancing the walking distance and walking time among patients suffering from neurogenic claudication.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-06-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"41869550","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, no technique could cater to all AS-related deformities well regarding the various mobility of calcified discs and the severity of deformities. Therefore, a thorough understanding of the strength and weaknesses of each method is essential. Then, a strategic approach could be formulated to select the most appropriate technique for a specific patient so as to have a correction operation with effectiveness, safety, and surgical simplicity. Among all preoperative examinations, the evaluation of disc mobility and the severity of kyphotic deformity is mandatory, which constitutes the base for the selection of osteotomy techniques. Since posterior column osteotomy (PCO) is the simplest technique, it should be used as much as possible. If up to three mobile discs are preserved, and the severity of the deformity is only moderate, e.g. the global thoracolumbar angle is less than 60[Formula: see text], contemporary poly-segmental PCO is selected. Staged PCO (SPCO), proposed by the authors, is the enhanced PCO with more excellent correction capability. If the disc mobility is not favorable enough, SPCO can be considered instead. PSO is the most versatile technique, capable of correcting all kinds of deformities regardless of disc mobility and severity of the kyphosis. This technique can be used in every case if surgical complexity is not a concern to the operator. Especially, PSO is the exclusive indication for those firmly ankylosed deformities without any mobile disc.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":" ","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-06-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"42336936","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
