The most common reported neurologic complications after lumbar posterior decompression and fusion are implant malposition, contralateral foraminal stenosis, hematoma/seroma, arachnoiditis, and nerve root oedema or vascular underlying causes. The current study reported that the patient developed unexpected left side neurological deficit after conventional lumbar posterior spinal decompression surgery for degenerative spondylolisthesis. No intraoperative dural insult. The computer topography (CT) scan and magnetic resonance imaging (MRI) of the spine after index surgery show adequate canal and foraminal decompression. No hardware-related complications or compressive hematoma could explain the patient’s symptoms. The patient recovered completely after removing the redundant posterolateral bone graft. The theory behind that is aggressive posterolateral dissection over the transverse process and abundant application of the bone graft might compress or injure the lumbar plexus. So, we suggested a delicate dissection posterolaterally and only a reasonable amount of bone graft could be applied to prevent such a devastating neurological complication.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":"81 8","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-12-20","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"138954265","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Metastases commonly occur in the liver and peritoneum, with very rare reported cases of bone metastases, hence, no therapeutic standard has been established for the management of bone metastases. Modern targeted therapy has improved patient survival, and although there are only a small number of reported GIST bone metastases, the incidence will likely continue to increase. Further understanding of GIST bone metastases is required for the implementation of an accurate and effective treatment plan. Case presentation: We report a 56-year-old patient who has a history of bleeding duodenal GIST resected with a Whipple operation. Post-operatively, Imatinib was started, but she developed liver metastasis 6 years later, requiring segmentectomy. She reported pain over the left humerus 2.5 years later and sustained a trivial injury to the left arm, resulting in a distal humerus shaft spiral fracture associated with a radial nerve palsy (Holstein-Lewis injury). Images showed a lytic lesion with cortical erosion and soft tissue extension at the fracture site. Biopsy confirmed GIST bone metastases. She underwent an open reduction and internal fixation with plating and curettage of the lytic lesion and made a good functional recovery afterward. Conclusion: This is the first reported case of a GIST bone metastases pathological Holstein–Lewis fracture. It highlights the potential for bone metastasis to present a long time after the diagnosis of a GIST. Although bone metastases are rare, they are now increasing due to the prolonged survival of patients and the introduction of targeted therapy. It is important to conduct long-term surveillance in specific patient demographics and future studies should be carried out to facilitate the development of a surveillance protocol in patients with GIST.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":"12 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-11-29","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"139214835","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, there are limited data about the application of these clinical tests for severities determination. Methods: A total of 64 patients with confirmed CTS were included. Three groups of mild, moderate, and severe were then defined based on the electrodiagnostic (EDx) test. The time to turning positive for Phalen, reverse Phalen, and compression tests were recorded. Initially, the percentage of positive results were compared among three groups of disease severity. Then, test positivation time was categorized into two groups of below and above 30 s, and comparison was made among groups with regard to this threshold. Finally, correlational analysis between mean positivation time and EDx quantitative parameters was conducted. Results: We found no difference between the three groups of disease severity, and there was no correlation between the time duration for three tests to become positive and EDx severity parameters. Conclusions: CTS clinical provocative tests are not of use in evaluation of disease severity.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":" 55","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-09","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135191692","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Methods: A cross-sectional study of 207 consecutively recruited T2DM subjects to assess and characterize MSP as neuropathic or nociceptive with the use of Nordic Musculoskeletal and S-LANSS standardized Questionnaires. Results: Of the 207 participants, there were 147 (71%) females with mean age of 59.7± 11.7 years. About 94.2% and 79.2% reported pain in at least one joint within 12 months and 7-day preceding the study, respectively. Ankle/foot is the most common site/region of reported pain in both the preceding 12 months and 7-day (58.5% versus 49.5%) while the least over the same period was at the elbow (22.6% versus 12.6%). Only 48.3% of the participants had restrictions in their activities of daily living (ADL) with 62.3% received treatment for self-reported pain. MSP was characterized predominantly as neuropathic among 27.5% of the participants which was highest among participants (36%) whose ADL were limited because of MSP. Conclusion: High prevalence of MSP was found among Nigerians with T2DM, more commonly reported in the ankle/foot region of the body with one in every five classified as neuropathic pain.","PeriodicalId":55895,"journal":{"name":"Journal of Musculoskeletal Research","volume":"16 5","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-11-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135585233","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
