Buzinschi","doi":"10.37897/rjmp.2022.3.5","DOIUrl":"https://doi.org/10.37897/rjmp.2022.3.5","url":null,"abstract":"Viral infections and vaccinations produce immune responses mostly through B cells (plasmocytes/ antibodies) and effector T cells (helper and cytotoxic). After removing the antigen, 90-95% of the effector cells disappear, but the remaining ones turn into T cells with long memory. The maintenance of cellular memory, the mode of information storage and the lifespan of T cells are insufficiently known. After measles, resident T cells in the plasma will offer protection only against the measles virus, generating a long period of immunodepression (immune amnesia).After the flu, memory T cells generate immune protection for 1-2 years for secretory IgA and longer for serum IgG. In SARS-CoV-2 infection, memory T cells (B and T) respond quickly to reinfection for 8-10 months. In conditions of intense stimulation in SARS-CoV-2, SARS-CoV, MARS infection, marked leukopenia occurs with lymphopenia generating immunodepression and high mortality, disorders similar to septic shock. These are BC patients in young age, hereditary (BRCA-associated) BC patients, inflammatory BC patients, synchronous cancer patients etc. In case of a large size of tumor or diffuse form of BC we often need to use extended surgery techniques in order to perform it radically and minimize a risk of recurrence. The aim of the study to evaluate the effectiveness of surgery approaches in inflammatory BC patients. Materials and research methods. Тhe first stage of our research was to evaluate extended surgical treatment using techniques by Handelhail and Beck on the risk of local recurrence. The study included 39 patients with breast cancer T4b-dN0-3M0 who in the period from 2014 to 2019 received complex treatment at National Cancer Institute of Ukraine. Besides neo- and adjuvant chemotherapy, an extended radical mastectomy using a technique by Handelhail and Beck was performed in 19 patients. In 20 patients a traditional (Madden) modified radical mastectomy was done. Results. When studying the data of a group of 20 patients we received a very high percentage of recurrence. Recurrences were classified as locoregional if they occurred in the ipsilateral breast or the axillary or supraclavicular lymph nodes, and as distant metastasis, if they occurred at any other site. Radical extended modified mastectomy consisted in the removal of the mammary gland and adjacent skin along the perimeter with subcutaneous tissue at a distance of 2-3 cm outward from the macroscopic edges of the gland, subclavian-axillary-subscapularis lymphadenectomy. The resulting defect was closed using a technique by Handelhail and Beck. The calculations for the serum average concentration/lot/date, were made, subsequent, using the values of the tested parameters regarding MPO (admission and discharge) the mean concentration/lot was made, regardless of time of determination. The calculated average values analyses of the three MPO serum parameters were constantly elevated and permanently matched the myocardial contractility deficiency class. Although they are mostly cutaneous, sometimes they can develop at visceral level, the liver being the most common localization. Objectives. Estimating visceral hemangiomas (VHs) prevalence, and identification of risk factors for VHs in patients with infantile cutaneous hemangiomas (ICHs). Materials and methods. 6 years cross-sectional study (2012-2017) including children diagnosed with ICHs, admitted in I.N.S.M.C “Alfred-Rusescu“. All patients underwent an ultrasound screening for the detection of VHs. In order to identify possible risk factors, we collected demographic and perinatal data. Outcomes. 138 patients diagnosed with infantile cutaneous hemangiomas (ICHs) were included, with a slight predominance of girls (58%). The prevalence of the VHs in our study was 7,24% (10 patients). The liver was the most common visceral localization (7 patients). Conclusions. Female gender, preterm birth, low birth weight, and multiple gestations were described as potential risk factors for IHs. There is no consensus for the effective vitamin D dosage nor for the vitamin D normal serum values. Both vitamin D deficiency and vitamin intoxication are dangerous for children. Pancreatic β-cell secretion is altered by hormonal changes during pregnancy. It appears, however, that patients who develop gestational diabetes have pre-existing insulin resistance. However, there are other risk factors to be considered, such as obesity, age, ethnicity, and polycystic ovary syndrome. Screening for gestational diabetes is very important to avoid maternal and fetal complications.
