We aim to compare multiple AI models with national human averages on SBCOC entrance exams (2021-2023), evaluate answer accuracy and citation reliability, and contrast models.Methods: Five models-ChatGPT-4 (standard and literature-trained), ChatGPT-o1-pro, Gemini, and Meta Llama 3.1-answered official SBCOC exams (50 items/year) individually using a unified prompt and identical images. Each response included one choice (A-D) and a cited source. Outcomes were accuracy and source reliability (peer-reviewed articles/textbooks versus websites). Statistics used chi-square tests and one-way ANOVA with Tukey post-hoc (p<0.05).Results: Across 150 questions, ChatGPT-o1-pro led (66%, 62%, 68%) and exceeded human national averages each year (p<0.05). Gemini (40%, 28%, 38%) and Meta Llama 3.1 (32%, 44%, 50%) underperformed relative to humans, while both ChatGPT-4 versions hovered near the ≥50% pass threshold without significant differences. Regarding sources, ChatGPT-o1-pro and Llama predominantly cited articles or books, ChatGPT-4 alternated between literature and websites, and Gemini did not specify references.Conclusions: ChatGPT-o1-pro outperformed the national human average and mostly used credible sources; ChatGPT-4 matched humans, while Gemini and Meta Llama 3.1 lagged. Patients diagnosed with benign lytic bone lesions who had a surgical indication for curettage followed by cavity filling with bioactive glass were included. In the postoperative period, patients underwent imaging examinations, which were qualitatively evaluated by a radiologist who determined the radiological diagnosis and the quality of osseointegration. The presence of imaging abnormalities was also assessed, and statistical analysis was performed.Results: Nine patients were included, 7 males and 2 females. The mean age was 25.8 years. The initial diagnoses varied, with enchondromas being the most frequent. The mean follow-up time after surgery was 8.4 months. All patients underwent preoperative radiographs. Most patients showed good clinical progression and good to partial osseointegration.Conclusion: The results reinforce that 45S5 bioactive glass demonstrates good performance as a bone substitute in benign lytic lesions, providing adequate osseointegration and a low complication rate. Its increasing incidence has driven the demand for new, effective therapeutic alternatives. This study compared the efficacy of platelet-rich plasma (PRP) and corticosteroid injections in the treatment of chronic low back pain of facet joint origin.Methods: This was a randomized clinical trial involving patients with chronic low back pain of facet joint origin, selected through a positive response to medial branch block. Epidemiological data and clinical outcomes were analyzed, including disability questionnaires (Roland-Morris Disability Questionnaire) and the visual analog scale (VAS) for pain. Assessments were conducted at baseline, one month, and three months post-procedure.Results: A total of 59 patients were included, with 32 in the corticosteroid group and 27 in the PRP group. No significant differences were found between the treatments in terms of disability scores or the Roland-Morris Questionnaire. However, a significant interaction between treatment and time was observed (p = 0.038). Regarding pain scores, no significant differences were found between groups at any of the assessed time points.Conclusion: The study found no evidence of superiority of platelet-rich plasma over corticosteroid injections in terms of pain reduction and functional outcomes for the treatment of lumbar facet syndrome. Proximally, it is composed of three heads: long, lateral, and medial, and presents a single tendon. Its insertion in the olecranon region remains controversial. Magnetic resonance imaging (MRI) assessment and detailed knowledge of this insertion may assist in orthopedic surgical reconstructions.Objective: To evaluate the anatomical aspect of the insertion of the triceps brachii tendon (TBT) into the olecranon regarding tendon insertion.Methods: In vivo MRI studies of patients were retrospectively evaluated. Two radiologists assessed 44 MRI scans from patients aged 20 to 50 years, with no previous triceps brachii tendon injuries or other TBT tendon pathologies.Results: There was agreement between the observers in the MRI scans evaluated, and the TBT insertion was found to be single.Conclusion: The triceps brachii tendon presented a single insertion into the olecranon. Swabs were collected from 54 tourniquets immediately after surgical use and before disinfection, covering an estimated area of 10 cm². The samples were cultured and identified using automated methods. The microbial load was described by median and interquartile range (IQR), and comparisons between public and private hospitals were performed using Fisher's exact test and Mann-Whitney test.Results: The prevalence of contamination was 70.4% (38/54). The median overall microbial load was 101 CFU per device (IQR: 0-153), corresponding to approximately 10.1 CFU/cm². The predominant microorganisms were coagulase-negative Staphylococcus (48.1%) and Staphylococcus aureus (18.5%), with isolation of Pseudomonas aeruginosa, Bacillus sp., and Candida sp. The contamination rate was 78.6% in public hospitals and 61.5% in private hospitals (p=0.081), with no statistically significant difference in the median bacterial load between the institutions (p=0.412).Conclusion: There is a high prevalence of contamination by clinically relevant pathogens in reusable tourniquets, regardless of the type of hospital. The results indicate systemic failures in reprocessing and suggest the need for high-level disinfection protocols or the adoption of disposable sterile devices to mitigate the risk of cross-contamination. Both Kirschner wires (K-wires) and cannulated screws are widely used for fixation, but the optimal technique remains debated. This study aimed to compare these methods in terms of functional outcomes and complications.Material and methods: A systematic review and meta-analysis were conducted in accordance with PRISMA guidelines.Results: Thirteen comparative studies involving 1,230 pediatric patients were included. Outcomes were analyzed using random-effects models, with risk ratios (RRs) and 95% confidence intervals (CIs). Meta-regression and prediction intervals (PIs) were applied to explore heterogeneity. Screw fixation significantly reduced the risk of postoperative infection (RR = 0.27; 95% CI: 0.12-0.60) and was associated with better functional outcomes (RR = 1.09; 95% CI: 1.01-1.18). No significant differences were observed in overall complications (RR = 0.89; 95% CI: 0.66-1.20), lateral overgrowth, range of motion limitation, or nonunion. Meta-regression did not identify age or fracture severity as effect modifiers.Conclusion: Cannulated screw fixation may be preferable for reducing infection and improving outcomes, but individualized clinical decisions and further high-quality research remain essential. After adaptation, the Brazilian version of the scale (URAM-BR) was administered to 40 patients with Dupuytren's contracture. Internal consistency was tested using Cronbach's alpha, reproducibility was analyzed using the intraclass correlation coefficient (ICC), and construct validity was analyzed by correlation with the Brazilian version of QuickDASH.Results: The URAM-BR showed excellent internal consistency (α = 0.930-0.938), significant reproducibility (ICC > 0.89), and moderate-to-strong construct validity (r = 0.69, p < 0.001). The adapted version proved to be clear, reliable, and appropriate to the Brazilian cultural context.Conclusion: The URAM-BR is a valid, reliable, and reproducible instrument for functional assessment of patients with DD in Brazil. The analysis considered synthesis material positioning, achieved angle (Reimers' Index), joint congruence on postoperative radiographs, and clinical outcomes recorded in medical charts after the use of the customized abduction orthosis.Results: Postoperative outcomes were satisfactory, with only 3.8% of hips presenting a new dislocation episode, requiring reoperation, and 7.6% showing superficial wound complications. All medical records reported comfort and practicality with orthosis use for both patients and caregivers.Conclusion: The use of customized abduction orthoses in the postoperative period of varus derotational femoral osteotomy in patients with cerebral palsy showed comparable effectiveness to traditional spica cast immobilization, while providing greater ease of home care and reducing the physical and emotional burden on caregivers. Early diagnosis and appropriate treatment are essential to recover knee function and avoid complications. This study analyzed updates and innovations in the treatment of ATF in adolescents, highlighting recent therapeutic approaches and their efficacy. A narrative review of the literature was performed in the Lilacs, PubMed and Capes Periodicals databases. Treatment should be individualized, with a conservative approach for stable fractures and surgical fixation for severe cases. Advances such as fluoroscopy-guided percutaneous fixation have optimized recovery, allowing better joint assessment and less invasiveness. Postoperative management emphasizes early mobilization and physiotherapeutic rehabilitation, favoring functional recovery and a safe return to sports. The prognosis is generally favorable, provided the diagnosis is accurate and the follow-up is rigorous. Rehabilitation is essential to restore muscle strength and prevent relapses. Despite advances, the scarcity of long-term studies highlights the need for more research to standardize protocols and improve clinical outcomes. {"title":"INFECTION PROFILE IN HIP ARTHROPLASTY AT A REFERRAL HOSPITAL.","authors":"Flávia Rachel Nogueira de Negreiros Freitas, Danilo do Nascimento Viana, Isânio Vasconcelos Mesquita","doi":"10.1590/1413-785220263403e299587","DOIUrl":"10.1590/1413-785220263403e299587","url":null,"abstract":"Objective: To analyze the occurrence of infections in patients undergoing hip arthroplasty at a tertiary hospital in Teresina, Piauí, Brazil, between 2020 and 2023.Methods: A retrospective, longitudinal, and observational study conducted at Hospital Getúlio Vargas, Teresina, Piauí, with patients who developed prosthesis-related infections during the study period.Results: A total of 1,420 surgeries were performed, with an infection rate of 2.0% (22 cases). Female patients predominated (59.1%) with a mean age of 68.8 years. Trauma was the main surgical indication (77.3%). Total arthroplasty was performed in 68.2% of cases, with 59.1% done without cement. The most common pathogens were Escherichia coli (22.7%) and Acinetobacter baumannii (18.2%). The main antibiotics used were ciprofloxacin (31.7%), clindamycin (25%), and vancomycin (15%). Hospital discharge occurred in 86.4% of cases, with no significant association between variables and outcomes (p > 0.05).Conclusion: The findings highlight the importance of preventive protocols and surveillance to reduce infections in hip arthroplasty.
