His course was complicated by severe hypertension and a number of acute life-threatening events necessitating periods of time on the intensive care unit, where he received ventilatory support and underwent renal replacement therapy and treatment with a monoclonal antibody therapy.We take the reader on a stepwise journey from presentation through to final diagnosis, discussing important biochemical, haematological and radiological features where learning points are discussed. The global incidence of Lyme disease is rising, and children are more frequently affected. The disease can manifest in various organs causing non-specific symptoms. The lack of sensitive and specific diagnostic tests makes the management of Lyme disease challenging. The prevalence in children in the UK is approximately 4 per 1000 children. It affects 1 in 220 children and young people in the UK with on average two children with epilepsy in every primary school and nine in every secondary school. The National Institute for Health and Care Excellence (NICE) defines epilepsy as a ‘condition in which a person is prone to recurrent epileptic seizures’ and epileptic seizure as a ‘transient occurrence of signs and or symptoms as the result of a primary change to the electrical activity (abnormally excessive or synchronous) within the brain’. Allocation: 1:1 allocation, stratified by age, HIV status, treating centre and ethambutol use. Blinding: Single blinding of diagnosis and endpoint tuberculosis (TB) review committees. Study question Setting: Uganda, Zambia, South Africa and India. Patients: 1204 children (aged <16 years) with symptomatic, nonsevere, smearnegative TB due to the start of firstline treatment. Intervention: 8 weeks of standard treatment as per local guidelines, followed by isoniazid/rifampicin for 16 weeks in the control arm, and 8 weeks in the intervention arm. Primary outcomes: ’Unfavourable’ status by 72 weeks, including treatment failure, loss to followup or death. A modified intentiontotreat analyses was carried out. Secondary outcomes: Adverse event of grade ≥3 and economic analyses to estimate costs and health outcomes. Followup: 72 weeks (18 months) from the start of treatment. Main results The % of participants that had a primaryoutcome event was 3% in both treatment groups (adjusted difference, −0.4 percentage points; 95% confidence interval, −2.2 to 1.5).
