Management varies with severity; mild cases respond to topical treatments, while severe cases may require systemic therapies or surgical interventions. We present two tumor-stage AKN cases successfully treated with electrosurgical excision and loop cautery, followed by secondary intention healing. In Case 1, a 38-year-old male with extensive keloidal plaques achieved significant improvement after electrosurgical excision. In Case 2, a 19-year-old male with recurrent pustules saw notable cosmetic and symptomatic relief from loop cautery. This study reviews the literature on therapies for specifically facial scars following Mohs surgery, emphasizing their mechanism of action and evidence of efficacy. Non-invasive options such as topical silicone and pressure therapy have shown benefits, while advanced techniques like dermabrasion, microneedling, or energy devices like the pulse-dye, CO2, Erbium, and Nd:YAG lasers, show promise in improving scar pliability and visibility. Intralesional injections of corticosteroids, botulinum toxin, and dermal fillers also have supportive data. Conversely, treatments like topical onion extract and vitamin E lack evidence of efficacy. Managing hypertrophic granulation tissue (HGT) also remains critical to reduce aberrant scarring from second-intention healing (SIH). In some subjects, this phenomenon is exacerbated by sun exposure, bad diet, genetic factor, pollution, etc. This remains a great concern as it impacts physiologically and psychologically on patient quality of life. The intradermal solution based on bio-fermentative HA is indicated for correction of skin damages including rhytidosis, photoaging and skin dehydration. The present clinical investigation was designed for obtaining further clinical safety and efficacy data for the SKIN R intradermal solution for the treatment of skin damages. The efficacy of the treatments was assessed by measuring the pH, assessment of sebometry, and hydrometry, Global Aesthetic Improvement Scale and Subject satisfaction. All 15 screened and enrolled subjects were treated with the investigational device at least once. All subjects who were assessed had a neck photo taken at visits 1 and 2. The subjects' judgment of appearance after treatment changed with time, however, by the termination of the study on Day 60, 33.3% of subjects still considered their appearance very much improved, and 46,6% still considered their appearance much improved. The assessment of \"satisfaction with treatment\" was very high by the majority of subjects. No major adverse event was recorded. Fractional lasers have been utilized to improve the appearance of this condition.</p><p><strong>Objectives: </strong>To compare the efficacy and safety of picosecond fractional 1064-nm Nd:YAG (PS Nd:YAG) laser versus fractional 10,600-nm Carbon Dioxide (CO2) laser in treating abdominal striae alba (SA).</p><p><strong>Methods: </strong>Thirty-two women with Fitzpatrick skin types III-V and abdominal SA participated in a split-abdomen study, receiving four sessions of fractional PS Nd:YAG and CO2 laser treatments on each side at four-week intervals. Striae texture, atrophy, clinical improvement, and satisfaction were assessed for each side at one and three months, with striae length and width compared pre- and post-treatment. Adverse reactions were noted at each visit.</p><p><strong>Results: </strong>Abdomen treated with both lasers had significantly improved skin texture, atrophy and clinical improvements (assessed by physicians and patients) at both follow-ups (p < 0.05), with no difference between the two. Striae length and width did not change significantly (p = 0.203 and p = 0.558). Patients reported greater improvement on the CO2-treated side at 1 month, but not at the final follow-up. High satisfaction was noted for both lasers. Adverse reactions noted after both lasers include erythema, edema, crusting/scaling, pruritus, pain and post-inflammatory hyperpigmentation. RF energy applied to a specific skin layer leads to either dermal remodeling or fat reduction. The multi-polar RF handpiece was used once a week for a treatment course of 3-8 sessions. The clinical evaluation of 40 patients showed no significant adverse and some degree of circumferential reduction with moderate positive correlation to BMI in 100% of patients. The blinded evaluation demonstrated a 35% improvement in \"skin laxity grade\" with an average rate of \"moderate improvement.\" The in-vivo histopathological analysis showed no acute heat injury as well as power-related progressive increase of fibroblasts proliferation and neocollagenesis. While the literature has primarily focused on dermal reactions as side effects of PDLLA injections, ophthalmic complications are rarely mentioned.</p><p><strong>Case description: </strong>We describe a presentation of severe visual loss and ophthalmalgia in a patient after injection with PDLLA subcutaneous filler at the nasal region who had been diagnosed with right eye iatrogenic ocular ischemic syndrome combined with central retinal artery occlusion. Despite treatment with ocular massage, intravitreal Aflibercept injection, and Hyperbaric Oxygen Therapy, her vision remained persistently impaired.</p><p><strong>Discussion and conclusion: </strong>This appears to be the first reported case of iatrogenic ocular ischemic syndrome combined with central retinal artery occlusion following PDLLA subcutaneous filler treatment. The study also evaluated adverse reactions associated with both methods.</p><p><strong>Patients and methods: </strong>Twenty patients with AGA were selected according to specific inclusion and exclusion criteria. The scalp of each patient was divided into two treatment areas along the midline, each randomly assigned to receive PRP either via intradermal injection (<i>n</i> = 20) or microneedle delivery (<i>n</i> = 20). The treatment comprised three sessions spaced 1 month apart. Scalp photographs and trichoscopic measurements were obtained before the first session and 6 month after the final session. Patient outcomes were assessed 6 month after the last treatment using the Global Aesthetic Improvement Scale and a self-satisfaction questionnaire. Pain levels during the first session were measured using a Numerical Rating Scale, and adverse reactions were recorded throughout the study.</p><p><strong>Results: </strong>Six month after completing all treatments, no significant differences were observed between the microneedle delivery and intradermal injection groups for hair density, hair diameter, single hair follicle ratio, or trichoscopic findings (<i>p</i> > .05). However, the microneedle group reported significantly lower pain levels than the intradermal injection group (<i>p</i> < .001). No adverse events were observed during the treatment period.</p><p><strong>Conclusion: </strong>The clinical efficacy of microneedle delivery of PRP was comparable to that of intradermal injection for AGA treatment, with the added benefit of significantly lower pain levels. The thulium laser combines characteristics of ablative and non-ablative lasers, standing out for its high precision in epidermal penetration, promoting cell renewal, pigment removal and stimulation of collagen production, with good tolerability. This study conducted an integrative review on the safety and efficacy of the 1927 nm fractional Thulium laser in the treatment of aesthetic dysfunctions. Searches were conducted in the databases MEDLINE/PubMed, Science Direct, Scopus, Google Scholar and Capes Journals between 2014 and 2024, to select studies that demonstrate the results of the application of this treatment. The studies carried out showed its efficacy in conditions such as melasma, scars and skin rejuvenation, with low adverse effects and high patient satisfaction. Although comparisons with other technologies, such as Er:YAG and Alexandrite lasers indicate some differences in results, the thulium laser offers advantages such as shorter recovery time and lower incidence of adverse effects. However, methodological and sampling limitations suggest the need for more robust clinical trials to consolidate their results and applications.
